Trepopnea clarified after referring many sources : Dyspnea more on lying on same side - ipsilateral severe lung pathology seen (lying on same side increases perfusion due to gravity but ventilation is not increased leading to V/Q mismatch and hypoxemia and feeling of dyspnea) Dyspnea on lying on normal side - contralateral pleural effusion seen ( lying on normal side splints the normal lung between the bed and weight of the body causing dyspnea ) So,Trepopnea on Right side may be due to right sided severe lung pathology or left side pleural Effusion!
Sir, can you please explain…, u have told that the only lung pathology that symptoms releives on lying on pathology side is pleural effusion and again u have also mentioned that this patient is having left sided pleuritic pain and symptoms relieving on lying on right side typical of left pleural effusion , I didn’t get it sir Can anyone clarify it please
He is talking about breathlessness as a symptom.. If pain as a symptom then it relieves on same side of pathology.. We consider breathlessness as major symptom so we talk respect to that.. While pain is secondary symptom as per disease
