    Truth is, it would ultimately be the son's choice. Even if the parents said yes, he'd have to say yes as well since he'd be the donor. If he's old enough, he could still agree to do it even if the parents said no.

      @@kalebgraffis5838 If we want to get technical, it does depend. If the son choses to give the lung, then the parents can veto that choice because they're legal guardians. However if the parents decide to give the son's lung, and the son refuses, then it doesn't matter what the parents' choice is because the hospital wouldn't force the son to do that.

    I can understand the parents plight. They have to sacrifice one child for the other. Of course the son only has a few years anyway, so by cold logic choosing the daughter they gain more, but it also means cutting their son's already short lifespan even shorter. It can feel like a betrayal towards him from the parents point of view. Kicking him when he's already down.

    3:28 - 5:00 - I really like this scene, normally House is the one to recommend unethical or crazy ideas that make medical sense, but are cruel in nature. He does this without hesitation and typically the patient's/parents resent him for it. However, because of Cuddy's request due to their new relationship, House doesn't do this, and instead the father comes and begs House for a solution, which he reluctantly gives. This time, because the father asked House for any options, he ultimately thanks him.

    I don't think the parents should make this kind of decision without involving their teenagers in the discussion. Especially not the one who would be donating the marrow and lung, because no matter what they decide, he could ultimately refuse, and no doctor would force it upon him. If he refuses, they don't have a choice to make. If she refuses, he's the only one who has the right to convince her to take it. The parents job is to make sure they both understand the consequences of either decision, as the doctors explained it to them, and ensure neither child feels pressured into either option.

    The thing I kept noticing throughout this is that Cuddy keeps talking about what it would do to their son, but she never seems to mention the daughter. "Do no harm" should apply to both of them. It's a really crappy choice to be put in, but harm is going to be done either way. Either they shorten the son's lifespan, or they let his sister die. And the only reason things have gotten this bad in the first place is because she has been continuously putting herself aside for the sake of her brother

      This is a long discussed flaw of the Hipocratic Oath-
      'Do no harm' and 'ALLOW no harm' are different. For instance, if a doctor sees a patient about to be murdered and could only kill the attacker, TECHNICALLY the Oath would dictate they allow it. We'd understand why they would't ofc, but it's a technicality. In advising them to hasten the son's death they DO harm. in not doing so they ALLOW harm.

      No, I don't think they do. What drives House, Why is he a Doc, What is his driving force that brings him to the hospital? He is there because he is an official drug addict. He is able to feed his addiction via the hospital. A seasoned white-collar addict never loses sight of his next fix, and thats why he is a prick his patients and their parents. They are not the priority. In his core you can see a little twinge of guilt, but it doesn't last. Real Doctors do not attend their patients impaired on drugs. People like him often commit suicide, because this is a game where there is no winner.

    I like how House handles pretty much confirmed death cases in the show. He was completely jokey and sarcastic with Taub right up to when he said she’s gonna die, and then you see it wash over House as well as he finally drops the act.

    Everyone is saying it’s sad, but take a piece from the doomed child in order to save the healthy one. The doomed child doesn’t have much time left anyway. I get it, the healthy one will live a normal healthy life at the expense of the unhealthy one’s very limited time span. But parents tend to be pretty irrational when it comes to the lives of every child they have. Think about it, to have a child you love more than anything in the world and risk their life in order to ensure the healthier one’s survival, regardless of the circumstances, is a pretty damn tall order to ask for a parent. Logically, it’s makes sense. Emotionally, it’s beyond devastating. 🤷🏽‍♂️

      CMD stands for Congenital Muscular Dystrophy. It’s a muscle disease where the chest muscles weaken which causes difficulty breathing and a stiffening of the ribcage. Technically it doesn’t affect the lungs so if he’s the a donor match for the sister and the CMD hasn’t made the ribcage so stiff that he’s punctured a lung or something like that, he could still donate a lung and not give his sister the condition.

    without having childs it would be rather difficult to understand why the parents are so reluctant on losing one, when the choice is between losing one or losing both. It’s not a toy, it’s not a prize, it’s not a watch or a phone. It’s not the same as “You either lose one of your phone or both of it, you either lose one of your watch or both of it”.     House was great, except everything they depicted as chronic pain and opioid dependence was wrong. There are drugs like clonidine that can relieve withdrawal symptoms and to see House always suffering was ridiculous.

      We've seen House escape the pain, and he's chosen to go back to it, because the pain is part of what makes him him. In one episode he felt being without the pain made him too nice and patient and friendly and it made him a worse doctor in his opinion, so he went back to the pain. In another episode, he refused to take a chance to get rid of the pain because he made the choice that led to his pain and so he felt he needed to live with it.

    I swear everytime Cuddy opens her damn mouth, I get more and more pissed. She'd rather everyone practice "hand holdy lollipops and rainbows" medicine instead realizing how the world really works. Does she not realize that the brother wouldn't even make to 25 if his sister dies, knowing there was a chance of saving her. I'd give it 6 months tops after her death before he'd become so griefstricken that he'd unalive himself.

      I’ve always liked the dynamic of House and Cuddy- how the job should be done from an objective perspective vs how it should be done from a moral perspective.
      House is only ever concerned with cracking the case- even willing to literally break the law (which he does way more often than the show thinks). Cuddy, as the dean of an entire hospital, has to consider legality and morality. House always had a case of tunnel vision, his obsession with solving the case.

    House was right that they weren’t given an impossible choice but a crappy choice and they chose the crappier option. I get that the parents doesn’t want to sacrifice their son. But the difference between a few years and a few decades is huge. Their daughter will die then their son will die in like five years. If they chose to save their daughter, their daughter could even live past them. She has a longer and brighter future ahead.
    I believed their choice was really influenced by favouritism, since their son was constantly sick so they cared about him more in their heart which resulted in the daughter being neglected. You can tell by that the fact that their daughter had to put on a healthy and strong facade so that her parents won’t be worried about her and only focused on her brother. It’s a common story troupe where the sick one is cared for while the other sibling has to be mature for the sick sibling and is often neglected and treated unfairly.

      Cuddy wasn't wrong about it being an impossible choice, but you also hit the nail on the head. Both of these kids were in a horrible situations, but the daughter at least had a chance for recovery, while the son was, unfortunately, on a time limit. The parents saying they wouldn't take the lung from their son was the worst decision they could have gone with, since they would have ended up losing both of their children. With House's idea, admittedly it is harsh, but it would be able to ensure that at least one of these kids would be able to live a longer life.
      I can't even imagine what parents go through when situations like these would appear, but I could never see myself choosing to allow BOTH kids to die when an option, as morbid as it is, was present to save at least one of them.

At least this way their daughter would live and, as he said, a part of him will always be with her. It's a shitty choice but it's the logical one.

    Yeah no. House is not correct here. Judging their choice because “your son is gonna die anyway there’s no reason to not shorten his time” is insane. You can’t ask parents which kid they’d rather have live and that is exactly what he did. The girl didn’t want it to happen anyway, so if they did it what makes anyone think the guilt wouldn’t cause her to kill herself when her brother died. Do no harm includes mental health and house honestly was never a good person anyway.

    The whole thing is that while Cuddy is 100% right that it's not House's call to intervene in that drama (It's not any doctor's call), it's not the parents's either! It's the fucking boy's call. The parents need to sign off on that, ofc, but they need to understand that their own opinion is irrelevant compared to their son's. He makes the call and then the parents sign the forms.
    Mathematically, it makes sense for the boy to donate, the dilemma is "lose one son or lose both children", but I get why the parents are leaning towards the latter option: It's just heartbreaking to sacrifice your own child to save another. But there's the thing: The boy *wants* to sacrifice himself. He loves his sister, his sister got herself in that whole issue because she loved her family more than she valued her own health. The boy knows his days are already numbered, so if he wants to sacrifice himself, I commend him for being such a brave young man.
    (Before anyone says "It's just a show", I fucking know, that doesn't mean I won't emotionally engage with it while the clip keeps running)

      I don’t think it’s favoritism. In their minds, they are choosing between their daughter’s or their son’s life. It is so easy from the outside, but humans tend to see lack of action as more guilt-free than doing something. By taking away their son’s lung, they are directly hurting him. By not doing so, they are doing nothing to hurt him; they are also doing nothing to save their daughter, but that means less guilt than harming their son.

    I'm confused. I guess I'll have to watch this entire episode, because my understanding is that "sickle cell trait" means that you are just a carrier of one half of the gene that causes the disease. My 40 year old African-American son has the trait, and he has passed it down to both of his mixed-race daughters. None of them have any symptoms like this girl.

      Yes. If you have a homozygous dominant gene of the sickle cell traits you have the disease.
      If you have a Heterozygous genotype, you are a carrier. You don’t have the disease, but you have a 25% chance of your offspring having the disease. I’m not sure if haus said she has the trait or the disease, if she had only the trait it would be medically inaccurate but knowing haus almost all info in the show is accurate.

    I think a medical professional is ethically and legally obligated to tell their patients all options. That’s what defines informed consent. For living donor candidates who are CONSIDERING it, there are living donor advocates who are supposed to support potential donors so they can make their decision without external pressure.
    It’s never easy, mind. And normally it wouldn’t matter because the definition of an option is that you don’t have to take it. What’s extra upsetting here is that, on some level, people understand there is no real option. It’s two dead children vs one dead child. There is no happy ending, no words for how gut wrenching and awful this is. Ultimately though, that’s for the family to decide, not the doctors.
    In the end, one of the biggest responsibilities of medical professionals is to disclose all information of the diagnosis, treatment options, treatment details, risks, etc. To not INFORM them that this is a viable option when it could save their patient is to take from the patient’s autonomy. TELLING them isn’t coercion, and it’s actually a major point that House actually DIDN’T initially try to coerce a patient or guardian for once.
    What he did AFTER with his rant could definitely be considered coercion, though hospital dramas tend to have a lot of it. But to be fair, Lisa then yelling and arguing back could also be considered coercion if this incident is reviewed despite her more sympathetic stance. It’s typically why medical professionals are supposed to take these kinds of conversations somewhere else and not do this shit in front of the patient/guardians. Like, screaming a private medical situation in a public hallway? Generally considered NOT GOOD. And then the son actually heard it and made his decision right after?

      He knows full well what he's asking them to do. But it's also something that happens every time there's a Mass Cas Event. Doctors have to choose who to save, and who to let go. The best case scenario for someone with CMD is that they live to see 30. They likely won't have children, and will be bound to a wheelchair most of their lives. Letting her go to save him ultimately means they lose both within a few years of one another. If she lives, she can have a full life where she could get married, and have her own children at some point.

      The situation was either they do nothing and end up losing their daughter over the next few days and get another 5-6 years with their son and his detreating health. Or they take the transplant from their son and get maybe another couple years with their son with the high chance their daughter will make a recovery and go on to life a full life. So....would you rather choose the option where you lose both your children over the next 5 years or get a few more years with the one child that is already dying so the other can live a full life?

      Because even though they are biological parents, there is still the chance that there isn't a match and the new tissue would be rejected. It happens all the time, a child needs a tissue/organ donation and even though the parents are willing, the compatibility is not able to allow it. The chances of a match are more likely in siblings than in parents, but even that isn't a guarantee; it is lucky that the daughter was a match with her brother.

I'm just pointing out that this wasn't thrown out as an option by the medical team to one of them and it might have given house a better idea of a way of throwing something back in the family's face. For instance, my son donated a kidney to me, he certainly wasn't a perfect match to me yet was able to pull off the process. Process. The genetics would work the same in reverse parent to son.

      Look as a transplant recipient, the only thing that needs to match is the blood type, you can give a zero out of six antigen organ and still have it work. I should know this as I've had a kidney transplant myself. I know that there's a lot of artistic license used on television programs like this, but I'm sort of a stickler for transplant portrayed accurately in TV programs. The chance of a compatible organ being from a parent is just as likely as it would be from a sibling. I don't know the episode well enough, but unless they are identical twins, clearly they're not as their opposite sex, there's really not going to be an advantage here. I know this is possible because my own father offered to donate a kidney to me at the same time that my son donated, my son turned out to be a better match. Even my ex-wife tried to donate, and she clearly has no genetic attachment to me. All I'm saying is that would have been the first thing as a physician that I would have suggested and I think it would have been something that house would have suggested as well if anything to get under the parent's skin. The sickle cell thing may be true. It's been a long time since I've seen this episode. Perhaps that's a plausible answer? I don't know? As I say, there's just a lot of artistic license done around transplant on these medical shows, also, the idea of some doctor running into an emergency room asking a car victim to be declared brain dead so they can harvest their organs and things of this sort. That stuff just doesn't happen

    I think people are missing that because this is House and a TV show, we know he'll fix the problem if he gets enough tries. Even with new information, it's not a guarantee, even with regular transplants carefully planned out. It's easy to do the math of sacrificing 5-10 years of the son's life to save a lifetime of the daughters, but what if it fails again? Then the daughter dies, and now your son is going to die much sooner. Even in cases where things are terminal, patients keep hope to survive on medical innovations, making death sentences livable, like with HIV.

    Don't get to play god she says, literally her & most of the team plays god anyhow when they stubbornly pick between which out of the 2 patients to be save in most episodes anyhow, no different to House other than he's rarely a hypocrite & is mostly a blunt introverted sociopath.
