I rock the waders and rain coat but I haven't went out into anything crazy. I would want a dry suit for sure going out on the Columbia or bigger bodies of water.
PFD always / every time, dry suit on if I wouldn't want to go swimming; which is basically all the time except for summer. I also keep a safety knife and radio on my PFD as well.

Paddling jackets and paddling pants will have neoprene around the waist and ankles to prevent water from getting inside.
