Great work but ONE CORRECTION, in case there is NO Documentation, we should approach Business Analyst (BA) or Product Owner (PO) , NOT the Developer as he might have a different perception. BA /PO are in direct contact with business so they can get the requirements correct. Ideally, without clear requirements, one should not proceed on assumptions, neither the developer nor the tester.
I was asked some of the challenges faced in QA I was also asked about the parts of a test plan simple and short, simple ask chatgpt to give you questions and answers to the toughest QA questions. They all picked them up from there
Defect may not be reproducible in the development environment due to configuration and other environment differences which are important to check, in addition to providing logs.

Speaking about the login form question - it is better to start with the happy path because it has the highest priority.
