One way to treat spalling concrete beam is to remove the concrete cement around the affected area, clean and treat the BRC before applying the cement on the beam. Will post follow up video soon.
How to treat it is one thing, what caused it is yet another thing. In this particular case it appears as though this incident took place on the inside of the structure. Or a least at an area not directly facing the elements. The problem in my opinion lies in the concrete itself. Where did the aggregate come from? Did it come from a gravel pit with a high salt content in the water?

How the hell did moisture get into the rebar in that place?
Either the tie wire ends outside the concrete mass or the concrete cover was too thin
