The Roman Empire is traditionally called "das Römische Reich" in german. Reich means realm but can also mean Empire or Domain

Your HRE ia inaccurate because Prussia is outside of HRE. You can see it by the fact that Duke of Brandenburg is King in Prussia, not King of Prussia, just like the Habsburg, suppossed to be King in Croatia and King in Hungary, because Hungary-Croatia is outside the HRE.

The italian Navy isn’t crap... ok not super crap. It’s large and once you add like 3 dozen new 1936 destroyers with an AA focus and upgrading and adding 2 new battleships or more and some new light cruisers and heavy cruisers it’s really not that bad. But then again that basically doubling its size. It starts up as thick so that’s a plus

5:20 Reich = Empire, if you wanna be really specific you can say Kaiserreich (Empire) or Königreich (Kingdom). (HRE in german Heiliges Römisches Reich)
"Reich" can actually be translated in multiple ways. You're right that it firstly means Realm, but an Empire is still called Reich like in "Kaiserreich". It describes something like Kingdom even though it means literally the Realm of the King
It wasn't always so decentralized. It wasn't that way until it was on its last legs
