Just did this for a client yesterday free of charge and didn’t ask for it It’s just something I noticed and took care of it. The client was so thankful for it. The hole was worn over bore - so I used a big plastic anchor that fits into the hole and clip/break apart a small anchor to fit into the bigger anchor hole and reused the screw. So basically an anchor within an anchor.
I did apartment maintenance on an apartment we owned. This was an annoying problem for me. The bathrooms were small. And the lath and plaster was chewed up over time by the screws put into to mount the toilet paper holder. My solution might be seen as extreme but I didn't like fixing the same thing more than once. I installed recessed toilet paper holders. I didn't use the metal attachment plate that came with them. I put blocking behind the location and I attached the recessed holders with regular screws driven into the blocking.
