Urinalysis: Preparing the 3 patient urine specimens for microscopic analysis

  • This video shows specimens being put into a graduated conical vial in order to get a great pellet during gentrification. The specimens are then centrifuged and viewed microscopically using the special urine slide.
    I did not like the video for the microscopic analysis of these urines. Unfortunately I am still working out the kinks on how to best make videos on the microscopes. Here were the results:
    1 Had Calcium Oxalate crystals with less than 1 WBC, RBC, and Transitional Epithelial cells= Indicating a kidney stone
    2 Had amorphous urates identified by lack of structure and being soluble in NaOH, there were also 1 squamous per hpf, less than 1 transitional epithelial cells, and rare calcium oxalate crystals= kidney stone also possible
    3 Had a pH of 9, many bacteria, 4 squamous epithelial cells per hpf, and triple phosphate crystals ie. coffin lid structures= old urine with need for recollection

    Beautifully explained and shown. So when you are getting the urine into those wells it should be sucked in automatically. My hand would have shaken. Thank you.

    Can you please tell me what is the g-force of the centrifuge? (I read once that it should be 400 g for 5 min, has it changed?)

      It depends on the rpms as to how long you centrifuge. Facilities adjust the time frame to maintain the best cellular component integrity.

      We use 2200rcf for 20mins

      g-force is a standard unit, while rpm is centrifuge-dependent(radius from the center..)

    I have a question for you if you just clarify for me. Do you still keep the condenser lower for the 40x or you raise it?

      The condenser is raised when more detail is needed in the view of the microscope. Many labs will set the condenser to a spot that achieves the desired details and leave it in that raised position since the work never stops (ie. It is never put away).

    Leukocytes seem to be measured as “ca cells/ul” (as noted on the dip stick keys). What does the “ca” stand for? Cancer??? Can these decrease without having cancer?

      I am not sure what you are referencing in this question. It would be WBCs per micro liter. You would not be expecting to see cancerous WBCs in urine.

    when do you grade color, before centrifugation or do you grade the supernatant? Thanks

      Make sure to resuspend the urine and look at the color and clarity in the tube. Centrifugation is after the chemical analysis before the manual confirmatory tests and microscopic.

      There is not an at home analyzer for dipstick and microscopic analysis.

    at 11:25, wouldn't it be a good idea to load the slide tray on the work top away from the condenser mechanism of the microscope to prevent a contamination spillage onto the scope and then transfer to the stage?

      Great question! You are concentrating the urine cellular components (into a pellet at the bottom of the conical tube) in order to view them under the microscope. Taking the supernatant down to one mL is important so the dilution factor is properly maintained for reporting purposes.

      @@MedicalLabLadyGill Thanks. Another question: Do centrifugation alter test result due to mechanical stress to the different casts,cells,crystals etc? I think test results will be more accurate in absence of centrifugation. I know this is wrong but please clarify.

    Wow this was a great video, thank you. I am a lehperson without a doctor and was looking into buying a microscope to analyze my urine, which led me to the centrifuge part and then I looked for a video to show me how the centrifuge works and what I'd need (1,800 rpm for 10 minutes, thank you lol) but I was glued to the screen for this whole video. I would have liked to see the specimens while you spoke but I read in the description that you weren't happy with the video so I understand. Thankyou

      Thank you for the support! Doing videos on the microscope while my phone is attached has proved very challenging. I am trying to figure out a way that works best for me. The urines I get don't really have much pathology in them so there are not many opportunities to make great UA microscopic videos.

    Trying to do a diy lab/homemade hospital, any advice on an semi affordable lab set up to check out urine, stool, etc?       @@olgathehandmaid not for labs, just emergency level standard prehospital stuff, got some clinical gear as well but nothing too advanced yet at the home. I live near so many hospitals it's kinda pointless but I want to have some kind of advanced setup for cardiac monitoring and fluid distro eventually to add to my doomsday kit lol 😂, might get into one on one care for money and do volunteer medicine for skill sets and practice but am trying to stay outta the system, it's completely corrupted in my opinion and I can't in good conscious keep up the bullshit hustle narrative they put on providers as they profit from suffering...

    hey ma'am. At specimen number 2 at 9:37 it indicates the patience have urinary tract infection. those residue. came from bacteria. the smell of it was so awful! because it happened to me too 3 months ago. I'm taking medication specially antibacterial until today.

      @@Ranveer_Singh_Kahani you can absolutely see the morphology of bacteria present in a urine specimen. Since the view of the microscope is a wet prep you can also see them move.

    how could you leave out the amount of a specimen you took how could you overlook that the first thing you need to say is we have taken a specimen of 20 cc's is 20 ml equals something you got to tell what it is

      I am sorry but am not sure what you mean. You need 10-12 mL of urine specimen in a graduated conical vial, centrifuged, and taken down to 1 mL. That information was said in the video.
