Dr. Tollestrup, you have opened the door to a lot of unanswered questions I have been looking for. I had spinal surgery in 2018 with a diagnosis of spinal stenosis. With severe sciatica pain in my left hip. Since the surgery it has become more than 100 % worse. I had a spinal stimulator implant put in in August last year. It has not helped a great deal! I was told that it would not help it 100%. I have to adjust it periodically but at my highest level now. The worst problem I have is pain in my feet. Mostly at night when I go to bed. It is a throbbing, burning consistent pain until I fall asleep. The horrible pain is skin sensitivity down the front of my left leg. Feels like an opened wound with salt being poured into my flesh. Especially when I take my compression socks off. I have had a nerve conductive study done but incomplete because it was to painful. I have used pain relievers of most kinds…..creams, pills, supplements etc….nothing has worked! I had another surgery (abdominal) in October last year and the leg pain traveled to my right leg but not as bad. My pain management doctor diagnosed me as having CRPS. I’ve had physical therapy to water therapy and nothing helped! I’m at the end of my rope what else can be done. Thank you for your time!
I am in hospital right now post Op for sciatica surgery. l had 4 screws inserted fusions in L4 L5 S1. I researched Piriformis syndrome 4 year's ago, and approached a consultant orthopaedic surgeon with my findings, but as said it's like opening a black box to them. Since my surgery Wednesday gone, I am more sure now than before. I want to wait in hospital till Tuesday to discuss this surgery with Mr Singh.

My neurosurgeon is offering me a piriformis release procedure. It involves a 4 mm incision on the side of the buttock releasing the tendon of the piriformis from the greater trochanter. He does not use a scope and uses fluoroscopy. He claims there's no major nerves or blood vessels there to cause injury. Wondering if you feel it might be a bit of a less of a risk of scarring down then other piriformis releaes, considering he's nowhere near the sciatic nerve with very little bleeding?
Can an MRI see issues with the sciatic nerve issues? I fell over three years ago and wasnt able to get physical therapy because of the pandemic so i just sat alot and did some chaur yoga now this recently started flaring up. Can chiropractor care make this condition worse ? My pain is worse after a bowel movement or at the end of the day after i have walked some and i have hypothyroidism my pain radiates all down to my leg as far as my foot

If it’s been less than 3-6 months and the steroid is put right into the piriformis muscle than it could solve the problem. If the problem has been present for longer than that, it might give some temporary relief or do nothing at all.
I m having burning pain for 1 month (after form rolling my glutes and hamstrings for 1 week and some vigorous clamshells) in posterolateral part of my right buttock which radiates down my leg with tingling and numbness. Also lateral side of my hip and upper thigh is very sore to touch. No medial/central butt pain. Pregabalin helps but if I skip it the excruciating pains back. The doctors keeps giving pain meds thinking it’s sciatica . My spine MRI is fine. Could it be bursitis , piriformis or something else?

This is what I believe I'm dealing with, piriformis syndrome. MRI shows spinal stenosis, spondylosis, disc bulge, but doctors don't believe this is where my symptoms (mild leg inflammation and cool sensation on calf)are coming from. So they sent me to Pain Mgmt (they're just as confused). No shots or surgery is recommended. Just started experiencing buttock discomfort on left side when sitting. No real pain. So returning to doctor for follow up.

I’ve narrowed it down in my research to internal snapping hip or piriformis syndrome or both. I often think suicide is the only way out. I am a stage IV breast cancer survivor and wonder why I went through the hell of chemo, radiation and mastectomy only to spend the last part of my life suffering. I have commercial insurance, but I’m really interested in your techniques. I’ve had two back surgeries and had back pain down my right side after the first surgery it took care of the pain ,but after a year the rods broke they had to go back in and put reinforcing rods in. After about three months I developed almost the same pain on my left side. So what I’m getting at is that I would like to know my options as far as payment. I had to retire because of my back pain. But my municipality agreed to pay three years commercial insurance for me.I’am 67 and live in Pennsylvania . I’ve been stuck in pain management with no results. I had a thoracic Sympathectomy, and have had a phantom pain in the upper left part of my back ever since. Some days are worse than others, but it generally feels like a muscle that is fatigued but I can’t get it to release (tried with stretches, ball, trigger point cane). There are definitely some stretches I can do they can trigger the pain/sensitivity, and some stretches typically those that involve my hands arms going above my head or backward make me feel like I’m going to pass out. I also have intermittent pulsatile tinnitus. I’m curious if the tinnitus and nearly passing out from certain stretches may be a symptom of TOS. Also curious if a TOS can occur as a result of a sympathectomy. These symptoms can be super distracting, and are impacting my ability to focus at work.
If things can slow down and there was some way I would come to you for my back surgery. I have an MRI to see what is going on. I just got a playstation 4 and the bac pain I know a peice of my l4 l5 lumbar is not fused and I just have pain. Sometimes my feet will have a burning like sensation. As far as my playstation 4 I am in so much pain I can't sit through a gamee. I have a video of me doing that macarina dance and I fell because the pqin made my legs give out.

Can you help me pls I believe I have piriformis syndrome of SIJD since 21 now 41. I’ve been disabled from it since 25 and only got worse. I’ve been to at least 80 drs in 20 years and about 20 MRI’s and xrays. No one has been willing to figure it out and help me. My life has just been wasting away it’s so painful. Smack middle of left buttock feels so tight I want to stab it. I also have tight pain that goes down back if thigh/leg. Sitting, walking, climbing stairs or lifting anything heavy hurts so much. No massage hard enough will release the tight feeling. I also have an indent right where the tight pain is. It feels like I need to stab it or take a small plunger and pull it out. I’ve wondered if it could be meralgia because when I wear anything a little tight it’s hurts so much. I also have bursitis a lump on side of my hip does piriformis cause bursitis. I also get pain in my tailbone really my butt especially when sitting or laying on my back. I don’t understand why most Drs just ignore ppl’s cries for help. #Helpplease

If it’s pretty significant, then surgery is the only way. If it’s mild to moderate, then PT/stretching the piriformis muscle might help. The fact that it’s been ongoing for 30+ years means that most likely only surgery to remove the piriformis muscle will be effective though.

Is pain around the sitbones common with PS also or would that perhaps be another nerve in addition? How about trochanter pain ?

Peripheral neuropathy typically does not damage bone. Muscle “damage” depends on the degree of weakness and how long the problem has been going on.

Mine started with an indent and after five months is now a crator size right at the piriformis muscle ...

Is pain around the sitbones common with PS also or would that perhaps be another nerve in addition? How about trochanter pain ?
