Hi, you needn't need platforms like Ezoic to use Google Ad manager. Google Ad manager is an Ad server that you will use to manage display ads on your Website. If you don't have AdExchange, you can just enable Adsense at the Ad-unit level and this will ensure that Adsense backfills when you don't have any direct ads from advertisers.

Please share your exact requirements on what type of creative you want it to be done for and I can share the creative code for you. If you to code a little bit, you can use the setInterval function and below code to accomplish what you are looking for. Hey to serve Adsense Ads via backfill, all you need to do is enable: 'Maximize revenue of unsold inventory with AdSense' checkbox at the Ad-unit level. You will be able to see this check box while creating/editing Ad-units

By non-hosted, you should own the domain. Adsense approval on TH-cam doesn't give you a Google Ad manager account. Hey, there isn't a limit to the number of Ad slots for Google Ad manager so you can serve any number of Ad slots you want.
