Do you ever feel like your heart is too slow, racing, or skipping a beat? The awareness of your own heart beat is most times something totally normal, however in some circumstances can be a dangerous or even life threatening arrhythmia.
Hello there, I am Dr. Carlo Oller and I am and medical physician who specializes in emergency medicine and have been in practice for 16 years. In this video we are going to review an FDA approved device you can connect through your smartphone in order to detect at least ONE of the more serious arrhythmias. We are going to check the device against medical grade EKGs obtained in the ER on patients with ongoing heart arrhythmias. So lets see if this device lives up to the hype!
Palpitations is a common complaint for patients coming into the Emergency Department, but the problem is that many times the symptoms have faded by the time the patient presents for evaluation. So even after a thorough evaluation with blood work, EKGs, and cardiac monitoring, patients often leave the ER with no specific diagnosis and no closer to an answer than when they presented to the ER.
Patients will then have to follow up (hopefully within a few days) with the primary doctor to get a holter monitor / or event monitor to be hooked up and then analyze their rhythm over a period of time to hopefully detect the problem if the problem happens while they are wearing the device. Even when the event happens while wearing the device usually you won’t have an answer until the data is downloaded and analyzed by your doctor.
There are plenty of heart rate monitors out there (smartwatches, fitbit, garmin, etc). They are good at detection heart rate (how fast or slow your heart rate is). This information is useful to the evaluating physician but does not provide any information about the actual rhythm of your heart during the episodes. In other words, it might tell us that your heart rate shot up to 130s while you were lying in bed but it will not tell us if you were in sinus tachycardia, supraventricular tachycardia, atrial fibrillation or flutter, ventricular tachycardia, etc. But, hey…little information is still better than NO information right?
Recently I learned that the latest version of the iWatch (from apple) would be able to detect and then alert the user of a potentially serious arrhythmia called Atrial Fibrillation.
Now…lets make a parenthesis and talk about AF / (atrial fibrillation) for a moment. A-fib affects somewhere between 3 to 6 million Americans, most of them over age 65. In some people it’s silent, causing no symptoms.
AF is when the top part of the heart beats in a convulsion-like manner and the rest of the heart has a hard time trying to catch up. It usually results in a heart rate in excess of 150-180 bpm. This arrhythmia is dangerous because it can exhaust the heart to the point of damage or infarct, and because the pump itself becomes inefficient blood pools and then clots inside the chambers of the heart. These clots can then be dislodged into the bloodstream causing arterial emboli and occlusion of vessels, pulmonary embolisms, and even strokes. Whether or not a-fib causes symptoms, it quadruples the risk of having a stroke. Patients with AF will have weakness, lightheadedness, dizziness, and even near syncope (almost passing out spells) because as the pump of your heart becomes inefficient, the ability of the heart to pump blood to the rest of the body is compromised by up to 20%.
So…when I learned that the iWatch was going to be able to detect Afib I was pretty excited about that. I though it would deliver some critical information in some patients who we would otherwise be ‘on the blind’ when evaluating them in the ER. Not all ER doctors were excited, I must say. Some were actually quite upset thinking that many patients would end up in the ER because their watch told them they were in AF and they were not (something we call a false positive). I have a different take on it, yes I might get some people who are there for a false alarm…but what about those that we wouldn’t have known about their AF unless the phone/watch would have detected it? Patients who could have potentially blown off their symptoms or not known because they had no symptoms and then come in much sicker because they developed heart failure (flooding of the lungs) or a blood clot that caused a stroke or other embolism? THOSE…those are the ones where this type of device, if actually delivers on its promises, could help save lives!
So…you heard me right…IF IT DELIVERS ON ITS PROMISES!
According to this article a cardiologist used the AW4 to study his own patients with AF against the iWatch’s ability to detect the actual arrhythmia.

I have AFib and didn't even know it until a few years ago. After our second daughter was born, my heart kept feeling like it was racing so I mentioned it to my doctor and had a Holter monitor for 24 hours. He just said it was anxiety from having a new baby. I still see the same doctor. I went in for a pre surgical exam and the PA said something about having AFib. I was completely shocked. I don't know why my doctor didn't say anything before. This is a very interesting video. Thank you for explaining it, in normal people terms.

I was feeling heart palpitations, so I ordered this device on Amazon for same day delivery. I was not short of breath, nor was I having chest pain, which would be an immediate trip to the ER. The device showed pre-atrial contractions, as I spent the extra $9.99 to have the professional consult in-app purchase. After a small amount of research I determined caffeine can cause these irregular beats. I dramatically reduced my coffee intake and have not experienced these palpitations again. This device definitely raised my awareness of a necessary lifestyle change. Highly recommend.

Great and informative! Just what I was looking for to watch since buying my Kardia Mobile ECG device.
And a GREAT little device! I bought mine a few days ago, in consideration of my Triple Angio, Quad Heart Bypass operations. And, although it might be if no value for the subarachnoid aneurism I had two months ago, I just feel better that I’m way more equipped for problems down the road than I would be otherwise. And while I’ve still got my Saint John’s Ambulance Emergency First Aid ticket, there’s no better way to up the ante by keeping some ‘essentials’ in terms of high tech gear on hand. So I’m always looking for ways to be better prepared, so to that extent, I’m looking into buying a proper 12-lead machine (around $800+) for my home use, as well as a portable oximeter, and a Philips portable home defibrillator.
Since I spend a lot out in the deep back country, I find things like a sat phone, portable BP meter that integrates the Kardia device, a blood sugar device, and the Kardia device itself, to enable me considerably more freedom, and fewer strings for enjoying the great outdoors for as long as I’m able.
Since I’m about to turn 65 next January, and most of my friends are over 50, I like to think of it as extra insurance for all of us, should we need it. Considering that we’re often out somewhere past the ‘Golden Hour’ in terms of response time for ambulance service, I figured that this might buy us some all important time to get to the Emergency room.

But he said it’s not unusual for readings not showing anything strange either unfortunately. Not sure about the Alivecor as I haven’t tried it, may give it a go though to see how it compares. Apple have made it a right pain to send multiple readings to your doctor on an iPhone or iPad.

The last time my heart jumped up to 237 was in May so I decided to get the Kardia I don't go anywhere without it in fear i might get an episode like that again. I had that experience with two fmily practitioners and three cardiologists. All refused to provide an email where I could send the PDF for my medical records. Unless you are highly experienced in reading EKGs (I’m a retired ER nurse), it could be a waste of money.

Dr. Oller, this was very interesting for me to watch. For the past twelve years I have been having palpitations, but just in the past three months or so I have been noticing that it seems as though it is getting worse. It REALLY bothers me and most of the time I feel light headed when it is going on. I have an Apple Watch and when I feel it, I do the EKG and I see that it doesn’t look normal, but since I am not a doctor, I don’t know if it is normal or not. It definitely is not all one steady rhythm. I have recently been wondering if I should go and visit with my cardiologist, since I haven’t seen him for several years. I have been taking 50mg of metoprolol in the morning and another 50mg at night (prescribed for my palpitations) for several years now but I am feeling like it’s getting worse for sure. The elastic band put on the arm when drawing blood CAN cause high potassium. Not always but on certain people. More info and reasearch is needed. Always listen to your doc!! Just an fyi. Also bisoprolol caused potassium to be high in 1%. They cut it in half dosage, potassium returned to normal. Not all are effected.

It’s not that Kardia can’t detect and identify PVCs, it’s that the FDA has cleared the device to id only AF, bradycardia and tachycardia. As you know, even twelve lead ekg machines can misidentify arrhythmias so this software encourages the pt to send/take the ekg tracing to a doctor, who can interpret it. even if the reading is identified as normal, a doc can look at it and quickly dx pvcs or whateverever else might be going on. It’s only going to tell you if you are in AF, Brady or tachy. Otherwise it says “normal”, which in reality means it could be dozens of other arrhythmias or normal, just not AF.

PVCs are pretty obvious on a strip. Yes a 12 lead ecg is gonna give you a ton more information but if you know your heart rhythms you can probably see quite a bit with this thing.

The algorithm detects sinus rythmn. If the algorithm detects a rythmn that is abnormal, it will show as "unclassified" as the software is smart enough to know its an arrhythmia it just doesn't know the name of the arrhythmia. If it reads sinus rythmn, then no abnormalities were detected.
For example, STEMI can not get picked up by the algorithm, but you can just observe The ST elevation on lead II to diagnose it.

A single lead akg is not up to what I am looking for.
Why don't we have a device with several stick on wireless leads that we can place on our chest and relay to our doctors?

Whether or not the Doctor is being compensated by Alivecor is irrelevant. I have been reading and watching reviews for both the iWatch 5 and the Kardia product and Kardia has been recommended over the iWatch in several avenues.
