The nearest major metropolitan area for me is Boston. My home lies 230 miles to the northwest, so I would be spared the direct impact of the nuclear warhead. The ones that are killed instantly after the warhead drops would be the lucky ones. They would be spared the agony of survival in a post apocalyptic world. Depending on how strong the blast is before it hits me, I may survive the event. If I am killed in the blast, I would welcome death with open arms. It would spare me from having to struggle to survive from day to day.

Not really, in reality the farthest the fallout would go would be about 200 miles. Plus even it did get to your city it doesn’t mean you will die, you would only die if you have gotten sick from the fallout and you don’t get treated in a certain amount of time.

most siren systems are controlled by the entire county, so i you live in the vicinity of both sirens, you will hear both of them, and if the city is big enough they would most likely have multiple sirens in the area.

The tones will activate a PEP receiver if the receiver can "hear" them. The loud beeps at the beginning of EAS emergency action notifications are actually bursts of digital information, and they're repeated three times to make sure the right information is conveyed to PEP receivers that are listening.
So am I, I'm probably at the least risk since I live really down south, so if realistically another nation wanted to nuke the U.S. I doubt they would want to nuke the extreme south cause it's likely some of Mexico would get caught up in the blast.
