Why Doctors Don’t Seek Help

  • I just wrote my first ever Op Ed in the Psychiatric Times about my decision to open up about my mental health as a doctor.
It would mean the world to me if you read it or shared it with a friend:
    www.psychiatrictimes.com/view...
This is a reality for many doctors in the US. Most states require docs to disclose if they’ve received treatment for a mental illness. The rules are different by state. Most states require docs to disclose this & they can refuse to give you a license bc you got mental health treatment.
My name is Dr Jake, I’m a doctor who treats mental illness, and I’ve sought mental health treatment. I have no fear of the repercussions.

However, if you _must_ elaborate, it should be phrased as something more along the lines of,

@@Havis_Princess that's not how depression works for most people. I've been depressed most of my life. I've never been suicidal. NOT medicating (long term) is the only thing that's caused a disruption in my employment. (In that I couldn't get up to go to work, not that I was suddenly incapable of providing a service) antidepressants don't work in a way that missing a pill effects anything. They take awhile to build up in your system so missing a pill does nothing. I know there are people who's conditions make them a hazard for certain work environments, but those kind of red flags can generally be sussed out with past medical and behavior history. IE by the time someone is a full blown physican with no supervision, said dangerous behavior would have been identified and dealt with accordingly. There are always going to be very few outliers just like with anything else, but depression is generally a slow slide, not a switch that turns on.. Symptoms don't just appear and are suddenly dangerous to others overnight. Catching stuff early is key. It might not be a perfectly perfect system, but dealing with this stuff on an individual basis seems to get the job done! Much love to you and all you do!

Well well well, hello everyone - I've been watching the attention build on my comment and I realise it could be read as harsh. What I meant by it is that, as a rule, everybody with a mental health issue will function better (and more happily) with treatment, and they shouldn't be put off from seeking treatment by concerns over their career, and all this will do is make people hide their problems until they can't anymore. I worry it sounded like I said people with mental health issues can't be good doctors, and that's not what I meant at all!

exactly, don't they know that people will just hide it? I think that instead, if you've declared you had a mental health issue in the past, then they should maintain that you are actively in mental health care while you're working - sort of as a preventative measure (which for most may not be needed but can prove a lifeline if they ever do relapse into that state after years when a particularly bad trigger comes along). They can either subsidise it, as a gift for all those loans you're now paying back to be in their field, or they can do regular check-ins to make sure you're still actively in care (like therapy).

I was severely depressed due to some issues in my uni and also not being able to keep up with studies, i got goddamn frustrated since i was always the best kid back in my HS yrs. My parents have never allowed me to go seek help and when i tried to do so on my own they threatened me to abandon me and that i need to get outta their house,, i didn’t have any financial sources to be independent so i had to give in, and that’s left me with 5-6 yrs of untreated depression that’s not gonna get any better,,

While understandable and I'm totally for allowing others to seek help, I believe certain mental health disorders that are incredibly disruptive- like schizophrenia, or psychotic episodes- and medications that have a side effects that can affect the doctor's productivity- such as making the doctor drowsy, or worse having hand tremors, unfortunately should disqualify someone from getting their medical license. However, mild conditions like anxiety, depression, or even OCD or ADHD, shouldn't immediately disqualify them. Especially if they no longer struggle with these disorders. The medications they CURRENTLY take may have an effect though (some are pretty hard core). It should be a detailed sign off process where a psychiatrist signs off that their disorders and (if any) medications will not affect their ability to be a doctor.
It's just like being a pilot- you're not even allowed to take Zyrtec for allergies because it can make you drowsy. Like a pilot has to work hours on end- a doctor has to focus even harder for hours of surgery. If they have a disorder that impedes on that, it can be a matter of life or death which isn't taken lightly. It's not perfect, but it's necessary- just life.

    • @tiffariff
      @tiffariff 2 ปีที่แล้ว +280

Also the reason I list above are why it's not illegal.discrimination. however they should he required to have a process in place if the person wants to appeal the decision and get an exception if they believe their mental condition will not effect their ability to practice medicine..

The doctor she was seeing jumped to a conclusion and if she hadn't been willing to go to another doctor and get a second opinion she would have been stuck taking medication for an illness she did not have. But if someone digs into her medical records it will always be there. So is it fair if she ever decided to join a field that decides if you've ever had a mental health issue you can't that she be black balled? Every single case and every single person should be a case by case review. Not a blanket policy. I personally would never have an issue going to a doctor who had been treated for a mental health issue today. And if one day I ever had a mental health issue, I honestly think seeing someone who had been there would make me feel better than just someone who had read about it in a book.

@@khtiderem7112 I think it can vary depending on school and country, but in med school you are treated the same regardless of specialty. Even in residency you may be subject to switch ups from what I hear. You train the same even as a psychiatrist and specialize further down the line. It's why any doctor is technically supposed to know what to do in an emergency for a person- given an ER doctor or EMT would be best for that situation but they don't ask "is there an ER doctor or EMT?" Most specialties also still require hour long shifts and procedures that still warrant steady hands and a good mind. A pediatrist still has to cut up the occasional foot just like an eye doctor has to do the occasional surgery. A psychiatrist still has to have the brain power to know what drug could make a person better- or worse, or hurt their health. For example, even something like a heart murmur or hypothyroidism, can make some mental health drugs really bad for you. If I was to tell the patient "hey this person has episodes that cause them to go out of reality (psychotic episodes- and at times similer to schizophrenia)." I don't think that they would feel comfortable with that doctor. I'm fairly certain in the irony of me using a psychiatrist as the exception to surgery that they would agree with me. lol. the medical system has a hard time as is keeping consistent systems. To make it more complex also wouldn't be a good idea. The standard also shouldn't be so high that they need to dive into "well this specialty should be fine." If they have a serious issue are on a hardcore medication, they just shouldn't practice. as blunt as that is

However, the licensing process for attorneys is similar as described in this video for doctors. Once you graduate, you first go through an application to get your license where you are required to disclose all past treatment for mental health issues and a variety of other things. What happens after that (i.e. refusal to admit until proof of being "cured" is presented, requirement that you attend state specific assistance counseling to evaluate you, etc.) will depend on the jurisdiction. Sometimes you will get lucky and a jurisdiction will not care at that moment, but most cases there are a lot of hoops to jump through.

Buissnesses also naturally compete & call eachother out on malpractices, even provide transparency of their own. They & private approvers build their own history & trustworthiness, that is just as good as the state, becsuse they have every incentive to. 1. It’s though to even find therapists that are accepting patients. And it could take several sessions with many different therapist to find one that is well suited to your needs.
      2. Unless you have coverage from work or some type of insurance plan that covers psychotherapy, it’s unaffordable. Therapy ranges from $200 - $600 / hour. Even if you have insurance it’s likely capped. You’ll be lucky if your insurance covers even 5 sessions per year.
It seems to me that the people that are most vulnerable or suffering the most from their psychological ailment are the least likely to receive any sort of help until they reach the point of suicide or self harm or drug addiction. There are large segments of society such as the homeless and the incarcerated that desperately need therapy but have little to no access.
The therapy industry is price gouging. The entire industry is supported and dependent on the insurance industry. The vast majority of these therapists have no right to change the absurd prices that they do but they can get away with it because the individual is not paying in most cases.

I am a human being, and I have struggled with mental health issues.” it genuinely gave me chills. This is so incredible to see. As a non-physician, I honestly wasn’t even aware of the prevalence of this issue, which probably feeds the problem because in that case they’re fighting a silent battle. Bringing awareness about this to non-physicians will absolutely help shift the culture as a support system for our doctors in addition to doctors being more comfortable opening up!

Like the basics of our course being short (have our MD by 22/23 so no ‘sacrificing’ our youth), government-subsidised so it’s cheap and we don’t have to pay upfront, and faculty who are super supportive and try to make sure we have good work-life balance (that might just be my uni tho), this is wild to me. A lot my cohort have mental health issues which we openly speak about, and we have subcommittees dedicated to supporting the mental health of healthcare workers. If you become a psychologist you have to see your own personal psychologist so you can handle what you go through with your patients. I’m not sure about psychiatrists. It’s baffling to me that is is different with medical doctors, they still have to deal with people in the psychiatric part of the hospital. I genuinely don’t get it.

I felt this when trying to enlist in the military. You can’t enlist with mental illness but once your in your able to seek treatment for mental illness. It was ridiculous. I knew a lot of people in the enlisting process with mental illness but they all lied and said they were fine so they could have a shot at getting in.

Same thing with lawyers. After all of the school and passing the bar exam, you can be denied character and fitness if you disclose any mental health issues even though you are sometimes dealing with very serious, traumatic events in others lives.

I agree with you. Don’t know why it has to be this way honestly. When more people have been on antidepressants in our world then ever before, you’d think that people would realise doctors are people too, and hell, they have to deal with awful stuff everyday. There is no way in hell no one wouldn’t get depressed after worker as a doctor

Yeah… imagine going through the most expensive, stressful, and academically rigorous program there is and expect students not to have occasional mental breakdowns/mental health issues. I think Doctors are a special type of human species considering all they can handle. But ffs, they’re still human before anything else.

It's like asking: "Have you ever gotten treated for a physical illness or injury?"
"Yes, well then sorry you can't be a doctor."
Not saying that everyone has a mental illness but a lot of people do. And getting it checked out probably could help your physical health as well.
    Thanks for the videos!

This might be why so many Drs say go for a walk and lose weight in order to treat my mental health issues. Maybe because they also learned the stigma. My issues stem from a mixture of genetics, abuse, and not getting help when I was young. Sure, it helps to be as healthy as possible, but mental illness is a real illness that sometimes we need medication and therapy for. I've needed therapy since I was a teenager, but insurance never covers it. Now I have a 6 year old and I'm trying hard not to mess her up. Thanks a lot twisted medical system... But thank you for these posts. ❤
