The real life Song was apparently able to retract his testicles and tuck his penis far back enough that he could create the illusion of a vagina and there was enough depth for shallow penetration. That's why they had sex so many times with the French guy not realizing. The fake baby also added to the "he's a woman" deceit. That and racist delusion.

M Butterfly is about a French diplomat who fell in love with a Chinese Peking Opera Singer, unaware of the fact that Singers in the Beijing Opera were all men. Song Liling/ Madame Butterfly used this as an advantage to deceive him since he sidelines as a spy.
