For those confused, on April Fools daily runs (and I believe one challenge), the item’s appearance is random, and you don’t know what they are until you pick them up. I assume in this clip, the game saw the pedestal as an active, dropped his active to replace it, but then the April Fools function activated, and took the hidden item he just picked up instead.
im a bit late, but no not in this one on april fools the daily challenge makes it to where all items and trinkets are randomized on pickup you can reroll your item by putting it in a pedestal in any way if you have a trinket you can keep rerolling it by dropping and picking it back up until you find a tick, the new version of the trinket retains golden if the rerolled trinket was

pro tip seeing an item in the gold room pool reduces it's weight by 100% meaning you can not find it again in the run even if you don't pick it up this is not true for every item pool but it's true for gold rooms, shops and devil/angel rooms for example
