Why was the father in law's not writing his mother's name a problem? A possible violation of the commandment to honor your father and your mother, or was it something else?

I am assuming the Rebbe wanted to pray for her and you should use the person's full name. Also in Judaism if you have two names you are meant to use them at all times.
