This is one of my favorite Eminem songs of all time. Think Steve King or your favorite scary movie director. It's like a good book or movie in music form. He paints an entire movie in your head.

I feel like he's referring to the perspective of a man that feels he has nothing left to lose. One that's mind is so warped and twisted from trauma that it's now found unhealthy methods of outlet. Hence the serial killing and twisted fantasies
