    I mean you say that Dennis walking out of a doctor's office with a prescription isn't accurate to real life mental health treatment, but this was made in America where that is much more common. A good doctor may want to use other therapies to address the underlying issues, but a lot of doctors in america will go for the solution that is the least amount of effort with the highest price tag. Even if that is the worst case scenario, or an uncommon occurrence, that's the premise of this show. It showcases the very worst of america, and Dennis walking out with drugs was an intentional choice, a critique of our health care system. I would argue that it is not only realistic, but a valid critique.

      American here and can confirm

    • @Lucifronz
      @Lucifronz 3 ปีที่แล้ว +28

      I believe that's in part due to the heavy corporate sponsoring that goes on in America. Doctors are hounded to take "samples" from corporate representatives for pharmaceutical companies, which in turn leads to them becoming more financially tied with them and eventually recommending the "product" over the well-being of the patient. It becomes less about finding a specific mental or physical condition the patient has and treating it and becomes much more about filling a quota of how many pills they moved for a specific brand.
      Capitalism is a cancer and it should never have been allowed to infest the healthcare system in America. I think Scrubs does a really good job of both parodying and seriously critiquing that.

      A lot of psych meds do absolutely nothing unless you have the disorder they treat

      @@Lucifronz I completely agree. At 12 years old I was diagnosed with depression. You know what my psychiatrist prescribed me? An anti psychotic designed for adults with schizophrenia. Just because she got a nice pay check. How dare anyone claim to "help people" when they actively take advantage of mentally ill people especially mentally ill children.

    • @emobx02
      @emobx02 3 ปีที่แล้ว +9

      Maybe that's your experience but I call BS because I've been in the field and idk any doctors that wouldn't suggest a full range of treatments and recommend traditional therapy alongside drugs, "good" doctor or not. The issue is that a lot of mental health patients do not want to do therapy or go through the high effort treatment plans, and many times just straight up refuse, so leading them down the path of pharmaceuticals is better than doing nothing at all in many cases.

    I'm actually really glad someone specialising in psychiatry and mental health reacted to this episode. I do love IASIP and I did laugh a lot at this episode. The one thing that bugs me is Dennis being diagnosed with BPD... which I don't feel really fits his character and only adds to the stigma of "borderline = psycho", I always thought he was more of a charismatic sociopath. So THANK YOU. DBT also needs more attention - a lifesaver for many!

    • @ginao6810
      @ginao6810 ปีที่แล้ว +5

      I guess because they never started with a specific diagnosis for Dennis that they wrote towards.
      The character developed over the years and they took him in weirder and weirder directions as the show progressed. Whatever suits the story and whatever they find the most interesting or most funny.
      The writers say they’ve deliberately avoided a specific diagnosis for Dennis as that might restrict their story telling. Plus they are aware that Dennis is such a horrible person that they don’t want to taint anyone with a specific diagnosis, as that could stigmatise mental illness.

    • @reeve1991
      @reeve1991 ปีที่แล้ว +5

      @@ginao6810 Indeed. I strongly suspect the BPD diagnosis in this episode was meant to be a joke that wasn't delivered all that cleanly or clearly. Given the writers repeated insistence that they don't have a specific diagnosis/disorder in mind (which is probably why he's so all over the place with the traits and symptoms he exhibits), that BPD is to my understanding the single most misdiagnosed disorder in the world, and that this occurred after a single brief session with Dennis, I believe the INTENDED joke was about BPD being inaccurately/sloppily diagnosed, in this case probably brought on by the doctor just wanting to say/do something to get Dennis out of the office. But if that was the intention, it could have been made a lot clearer, because it's left an enduring impression on a lot of the IASIP fanbase that Dennis is meant to have BPD (at least as his primary disorder), when that just... doesn't really mesh with what we otherwise see, where he exhibits so many eclectic symptoms of Cluster B disorders and is all over the Dark Triad, but I think most people if they had to slap a single label on his core psychosocial dysfunctions it would be Narcissism more than anything else.

      @CheziahKatt 3 ปีที่แล้ว +2

      @@nthgth it does not! BPD stands for borderline personality disorder. Borderline personality disorder however does present similarly to bipolar disorder at times. There are wild fluctuating mood swings. Bipolar disorder is a chemical imbalance so they need certain medications, whereas borderline personality disorder is more of a case where someone didn't get the proper coping mechanisms growing up? So they do bad and reckless things, so directly they need therapy to work on the coping mechanisms - sometimes they do need to take certain medications however. People with BPD can often have things like anxiety or depression as well.

    I took abilify and was told that weight gain doesn't generally happen on that one and I went from 170 pounds to 230 in 2 and a half months. I have a serious fear of anti psychotic now even ones that say you don't gain weight or there is less of a chance.
    No point in helping my mind if the way I look now makes me feel shitty.

    I love that you actually pointed out that it's not bpd Dennis has. I prefer emotionally intensity disorder (eid) as it more accurately defines what I'm experiencing but I'm just glad that you tried to undemonise it a bit. There's a lot of stigma against it

    Really appreciate your commentary! I wanted to mention that I experienced weight gain on quetiapine and ended up trying Lurasidone on the way to finding the right meds for me. Unfortunately, Lurasidone was the worst experience I’ve had on any of the many medications I’ve tried. I had akathisia where it felt like a constant sensation of restlessness bordering on panic. I ended up stopping the drug after a few days and ended up finding a mix that worked better for me but I just wanted to share my experience because it was horrible and it’s not that uncommon a side effect.

    God when Dennis said he answered the questions honestly and got medication for BPD I was like "oh no no no no no no no" bc I have bpd and I do NOT act like Dennis lmao if anything maybe could show signs of it but not Dennis. I think he got that bc bpd tends to be misdiagnosed and you also don't get that diagnostic from one sitting that it just shows the psychiatrist was just not paying attention, especially after what Dennis had just said to Dee.

    What are your thoughts on the Rosenhan experiment, the 33 percent reproducibility in psychology, and the lack of cultural universality of treatment and diagnosis?

    • @DoctorElliottCarthy
      @DoctorElliottCarthy  4 ปีที่แล้ว +6

      Wow, very interesting questions and maybe I can address these in a Q&A vid sometime? Cultural differences are hugely underestimated and it's something I continually try and educate myself on but it's an enormous area. The Rosehan exp is utterly fascinating but horribly unethical (that and many other practices at the time). Another great topic for a video!

      @@user-uv2cp1qd1j ...yeah, so, other species act solely on emotion if you put it that way. Acting on instinct = more or less, doing just what you feel like at that moment, without regard or thought. That's fair.
      I just mean, since we're the only species that usually (at least most of us, most of the time) do things in a planned, _logical_ manner, we're superior. Because as a species, we're awesome at controlling our emotions, is my point. Compared to any other known non-fictional species.

    what in the world is a "junior doctor"? a first year?

    • @DoctorElliottCarthy
      @DoctorElliottCarthy  3 ปีที่แล้ว +3

      No it's a rubbish term used in the UK for any doctor who isn't a consultant/attending. Takes anywhere from 5 to 10 years after med school to become a consultant in the UK so a lot of "junior doctors" are very experienced. I've been a doctor for over 5 years but technically classed as a "junior doctor" because I'm not yet a consultant

    Psychologists can't prescribe meds in the US, they can only do psychotherapy. Cognitive behavioral therapy is a very common treatment used by psychologists here. It's only psychiatrists who can actually prescribe meds. Reading through the comments, it doesn't seem like a lot of other Americans know that though.
