This song was a tribute to the singer and bassist's Mikey Ways Grandmother that died. They were very close to her and Gerard felt he hadn't been there for her in her last years like he wanted. So he felt guilty when he wrote this.
Yes, it was their grandmother Elena, known as "Helena" to everyone. She taught Gerard how to sing, how to work with make-up, etc. She even gave MCR the most vital piece of equipment for any young band; a van, so that they could haul themselves and their gear to gigs. IIRC, the white van in question is actually included in the video. I think it's in the background at the end, when the band is loading the coffin into the hearse. I've also heard that a lot of the lyrics came from letters Gerard wrote to himself after her death, in order to help process his feelings of grief and remorse. Others have observed that, Gerard's guilt notwithstanding, Helena would probably have wanted MCR to be out touring and "sharing their gift", but, of course, very little about grief is rational.
a couple of great facts about the video. the rain at the end wasnt planned, it just happened to be raining the day they filmed, and it worked out well for them. the sixth pallbearer with the bandmembers was just one of their fans who was invited there for the video shoot, just like the entire crowd at the funeral. and Guitarist Frank Iero is so short he couldn't actually support any of the weight of the coffin, he could only just put a hand on it.
