The # EP39YR CTS 3 Tuner is designed for GM vehicles, so it won't work with your F150. You'll want to use # EP59YR. The only thing that will fix your gear ratio is changing your rear gears, but the CTS 3 will allow you to recalibrate your speedometer to compensate for the taller tires. Would this tuner work on a 4.6 2009 f-150 I recently got a 6.5 lift 35s and 24x14 I wanted to see if this tuner would fix my gear ratio and gas mpg

No you need a dyno tune but you should still get one to monitor trans temps, read and clear codes.
