  • IMM 5476 is the 7th form we are filling in this video series. We have addressed all common issues such as who should act as a representative if you don't have one, how to sign this form digitally, how to write N/A, and what to do if a section does not apply to you.
This video is to guide you all who wants to fill IMM 5476 form without any confusion.
I am not any authorized or licensed IRCC official. I sponsored my wife to Canada recently , and this is how I filled my form IMM 5476. My wife became Canadian PR in 2022.

your competitor says you shouldn't use the hand signature in Adobe reader. It should just be your full name as per passport as there are clear instructions in the guidelines. Careful :)

The signature part in this video is absolutely wrong! Do not do virtual signatures because my form was returned because of that. For this specific form, IRCC wants you to physically sign the forms, scan it and send it to them.

This is really strange and I feel really bad after reading your comment. I have seen many people signing digitally even this form and had no issues.
Do you know where in particular IRCC says that physical signatures are required for this form? Because I see that IRCC says we don’t need any physical signatures for online spousal sponsorship application. :(

@@canadadreamers_ you feeling bad wont change the facts of what happened with me. my IMM 5476 use of representative form was returned twice because it was digitally signed and not physically signed. i digitally signed other forms but there was no issues. for this specific form, IRCC wants real signatures. when i consulted multiple forums and RCIC's, they said the same that digital sign is a no go. It does not matter if the application was submitted online. check other videos on TH-cam for use of representative form and all of them recommend physical signature for this specific form. Only after giving real signature and on the third attempt my IMM 5476 form was accepted. Stop replying to me and ask any RCIC and they will tell you the same.

Thank you for the video. In the scenario of having only a given name without a surname on an Indian passport, I'm unsure whether to input the name in the surname or given name section of the imm5476 form. I would greatly value your advice on this matter. Thanks in advance.

Hi, I wanted to start by saying your videos are great and all parts of the forms are explained pretty well. I had one question about this form. Part 7 your representative's contact information.
I am filling out this form as I am processing the spouse PR application for my wife. Now I have created an email account just for this application for my wife and have provided that email in other forms as my wife's email address.
Can I use just that email address here as I have created that email account only for the communication updates related to this application? or will I have to use my email here?

Hello, thanks for watching. Appreciate your feedback.
In part 7, they are asking for rep contact information, so you (sponsor) are the representative here. So provide your email id. Not Applicant's email id.

Internet response: please validate by yourself:
To fill out the Use of a Representative form (IMM5476) for your child who is less than a year old, follow these steps:
      1. Section A: Applicant Information
      • Question 1: Enter your child’s information as the principal applicant. This includes their full name, date of birth, and other personal details.
      2. Section B: Appointing a Representative
      • Question 2: Enter your information (as the parent) since you will be acting on behalf of your child.
      • Question 3: Indicate that you are an unpaid representative, as parents typically act on behalf of their children without payment.
      3. Section C: Your Representative’s Contact Information
      • Provide your contact details, including your address, phone number, and email address.
      4. Section D: Your Representative’s Declaration
      • Sign and date this section to declare your consent to act as your child’s representative.
      5. Section E: Applicant’s Declaration
      • Sign on behalf of your child in this section, as they are underage and cannot sign themselves.
Make sure all the information is accurate and complete to avoid any delays in the application process. If you have any further questions, feel free to ask!
      Hope this helps!
