Often times the insurances have their policies listed online. Medicare has what they call the IOM or Internet Only Manual which is the huge compilation of their policies. Often times the insurance companies also have newsletters you can subscribe to for important updates.

Can a screening colonoscopy stay as screening if they find polyps, but do not remove them?
So since it turned into a diagnostic procedure, you would still count the Z code for a screening?

It is, but often used as an exam strategy for saving time. It's up to you to determine your confidence in that risk.
