Why are simple direct restorations the most important in Biomimetic dentistry? 🤔

  • This patient wanted to replace her silver fillings but was told she would need a crown. Unfortunately, this is common in the dental insurance model, and it’s a crime! 😡
    When teeth like this get a crown, the number of problems and the chance of complications increases significantly, often leading to root canals and the loss of teeth over time. This is known as the tooth cycle of death. ☠️
    Replacing these silver fillings conservatively with a biomimetic approach drastically prevents these complications. This makes these smaller direct restorations some of the most important services we provide to our patients. With this approach, the tooth will likely never need an indirect restoration or crown, and root canals can be prevented. 🙌
    In a previous post, I shared how little details like anatomy make a difference. Why? Well, there are a lot of reasons. 🧐
    Taking your time to do it this way ensures the best marginal adaptation and minimizes polymerization stress for each increment. Natural anatomy also improves function, chewing efficiency, and minimizes overall tooth wear. 👄

    I’m not a fan of pigmenting anything. I prefer a completely white tooth, I fear it’s something another dentist in the future might mistake for a caries… but I respect the artistic choice. Great job overall.

      Thanks. i actually think when it looks good and natural it’s less likely to be treated again because it’s seamless and doesn’t draw attention. Also when it does get noticed, I have received emails and phone calls about the work asking about the material or technique so I think its less likely to be retreated when its deemed to be excellent. Either way, if someone goes to a dentist that makes that mistake, then the real mistake is going to that dentist. No competent dentist could make that mistake with radiographs, basic evaluation, and magnification.

    I just got a quote from my dentist to remove my tiny fillings I have 4. But the large fillings on molars he said no they’ll likely break. He said he’s only willing to remove the silver in the big ones if I get crowns.

      You don’t jump from a filling to a crown. I would do an inlay or onlay depending on how big the silver filling is or how badly the tooth is damaged (cracks, cavities, etc).

      I’m saying that’s not a good treatment method. I don’t recommend doing a crown unless that’s the only option which it does not sound like is the case.

      $200 around here gets you a crappy bulk-fill with no isolation and poor marginal integrity.

      Do you suggest a way for the patient to detox from the mercury filling being removed, so toxins aren’t released into their system? I’ve heard of natural dentists doing that with Glutathione, etc.
