  • In this video Doctor Andrea Furlan, physiatrist and pain specialist, explains what disc bulge is. Because magnetic resonance imaging (MRI) is so sensitive to see tiny changes in the body, there are a lot of people who find out that they have lumbar disc bulges. But these are not always the cause of low back pain. There are many people, including children, who have disc bulging but they don't have any symptoms, they are asymptomatic.
    A bulged disk sometimes may be associated with lower back pain, if the disc is compressing the nerve root or causing narrowing of the spinal canal, which we call spinal stenosis.
    Doctor Furlan will explain that the most serious disc problems are related to disc herniation, and disc bulging may progress to disc herniation. There are various types of disc herniation: disc protrusion, disk extrusion and disc sequestration.
    When disc bulging causes symptoms, they are usually in the lower back area, and this is what we call degenerative disc disease. When there is disc herniation, the symptoms are in the area innervated by that nerve root, and then, the symptoms are in the buttock, thigh or leg. And they are usually on one side only, that is usually called "sciatica" but that is not a good name for this, as it is not a problem of the sciatic nerve.

    Excellent video.     Great video! This gave me a better understanding of my back and leg pains than 4-5 doctors, that I have seen these latest few months, have given even though they had my MRI results. Really complicated pain patterns explained so even I can understand. Thank you, doctor!

      Is the finger stiffness linked to ur back issues? I have the same problem. Body aches all over, especially fingers. No arthritis in the diagnosis. Only ddd, multiple disc bulges, stenosis, and spinal ligament sprains. And straightening of the spine (lordosis)

    NOVEMBER ,I'll be going for my second kneck fusion. First time my c5/c6 was fused, in less then 2 wks they are fusing c4/c5 and possibly adjusting c6. Personally the surgery was scary, but the best decision I made. This past June, had my 1rst stimulator replaced with a more updated one that works. I have bulging disks and was told I also have DDD. I had pain going down my legs arms and migraines beyond belief. So far, I'm happy I've had these surgeries and I'm starting to feel " normal" so to speak again.😀

    Good information. I had a bulging disc, but was in pure agony for a long time (a couple months). I can't describe the pain, it was overwhelming. Good to know most people don't have to suffer through that. I simply went to bed, and woke up next morning barely able to move my legs, so this can occur at any time. Just terrible.

    Wondered what your thoughts are on Internal Disc Disruption (IDD) - first coined Prof Crock back in 1970, with Emeritus Prof Nikolai Bogduk covering a lot of work on IDD in his books, Clinical Anatomy of the Lumbar Spine & Sacrum 4th Ed (2005) and the update, Clinical and Radiological Anatomy of the Lumbar Spine (2012).

    I had back surgery in 2012, I had herniated disks l5 and l4. I also have degenerative disk disease and stenosis. The surgery went well, but when the surgeon went in he said I had more damage then what showed on all the many xrays, mri and visits. I had let it go for many years. I had multiple injections into my spine which only masked the pain, then I had to go on pain meds because the pain was getting worse. I let it get to the point where I couldn’t walk and had to crawl to get to the bathroom. That’s when I said ok I have to have surgery this took my 4 years of pain to decide because I was so afraid of being paralyzed. I waited too long a lot of my nerves on my leg were damaged. To this day i still have no feeling in them. Well I’ve always heard if you have one surgery you’ll always need another at some point. I think I’m there now, my lower back is killing me and I have sciatica again.I have been doing the stretching exercises but they aren’t helping. I truly hope I don’t have to have another surgery 😔 your video was very informative so I guess I’ll be going back to the back dr again.     Thank you for this information. I had a CT Scan and MRI Scan and l have moderate central canal stenosis L4, L5. Plus a bulging disc. It's very painful. I'm waiting to see a Rheumatologist. Do you recommend l see another Specialist? I have had for 1 year now and pain is getting worse.

    Hi Dr. Andrea,
    The MRI result shows L4-5: Mild broad posterior disc bulge and ligamentum flavum hypertrophy. Mild degree of central canal narrowing without significant neural foramen narrowing.

    L5-S1: Mild bilateral facet arthropathy. No central canal or neural foramen narrowing.
    What do you advice me? I want to talk to you and is there any way to contact you?

      Yes. It is. The graph that I show with "disc protrusion" in normal asymptomatic individuals are all about disc herniation in MRIs. Disc protrusion is a type of disc herniation.

    I have lumbar compressed discs.
    Some bulging. Just started back decompressions last month. I have had about 12 so far. Starting to feel a little better. Hope this will work. I'm not in any real pain. Stiffness in the morning's.
    What's your take on back decompressions. Can this alleviate the problem, or am I wasting my time and money?

    Mam, I have a disc bulge at L4-L5 and L5-S1. ND inflammation in sarcollic joint. I have done physiotherapy and chriopractric treatment but my pain is still in lower back nd in left thighs. I have taken snrb injection but still having pain..plz suggest me something to get rid off pain

    I have Spinal stenosis, I went to pain management got the epidural shots they didn't help. I went to physical therapy it did help but pain is coming back. I had this problem for over 20 years, should I see a back surgeon? I'm tired of the pain.

    Hi Dr. My mom complained of pressure on her back about a year ago. About 6 months ago, she slowly lost her ability to walk (without a walker), stand up or use her arms effectively. We had numerous radiographs taken except an MRI. All of which indicates osteopenia. We have also seen a physical therapist who suggested narrowing of the spine due to muscle tightening, when my mom said she lifted very heavy stuff in her occupation. She also has a history of Gout. With exercise, she seems to be getting bit by bit better, but still far from it. What would you suggest her most likely diagnosis would be and any recommendations?

      All of these indicate osteoporosis and osteopenia. Do not forget at night the body decomposes the bones with osteoclasts and in the morning it synthesizes them with osteoblasts. She need daily calcium vitamins with D3 K2 and of course magnesium with vitamin c. In summer, leave it in the sun until it bursts with sweat, it is extremely beneficial for the bones.Excellent presentation!!!

    Hey guys I’ve had 3 laminectomys on l5s1 which did work then I had a fusion on l5s1 and I came out worse my foot is numb still still extreme pain in back and leg I would hold off as long as possible for a fusion at least in my experience

    I have all 5 of my lower lumbar discs along with myelopathy spinal stenosis arthritis that is getting worse in my facet joints. So what’s your medical opinion? I’ve had this for 10 years due to injuries from my former employer. I also now have a very good Doctor now

    Is this problem common after a car accident. A city bus hit my car on the passenger side back fender, one year ago. Since then my right leg is still hurting all the way down into the foot and toes. It is like deep stabbing pains. I am finally going for an MRI in a few days. I also developed a “capsulitis” of the second toe.. Recently my left ankle is starting to hurt also. I am in my sixties, but l never had back or such leg pain before? Prior to this accident l use to walk a lot and exercise almost daily . I am no longer able to do this.
    I would very much appreciate your opinion. Thank you so much.

    Im quite confused my MRI said i have diffuse disc bulge with superimposed central disc protrusion wich causing severe bilateral foraminal stenosis abd mild spinal canal... May you please enlighten me is this a bulge or herniation? Thank you

    Can you address cervical and lumbar disc degeneration?
    I have 2 places in my neck and 2 in my low back that are near bone on bone. And Scoliosis in between.

    I have a herniated disc L2/3,which has pushed onto my nerve leaving half my R leg numb with some shooting/burning pains,Naproxen helps but would like to cease this painkiller,any ideas on this?

    Excellent asusual admiring one .disc bulge and very common in asymptmatic individuals ...is the evidence because it might affect psychological rather than physical more with noceibic effect never accept treatment ...so due to that unnecessary spine surgeries are prevalant .if you see my last video MRI catastrophisation ..great madam ..i admire your passion

    I have got 2 bulging discs but i have also got something pressing on my spinal chord. I had an mri scan to look at my back because of major back pain at the bottom which prevents me to walk. As soon as i sit down its relief and as soon as i stand up its painful. The doctor told me that he doesnt know what it is that id pressing on my spinal chord so i am being sent back to physio. It effects my life on a daily basis. I had a rupurtured annurysm in 2017 which caused a bleed on my brain so i am wondering if this is an after effect from then. Any information you have for me i would be grateful. I am wondering wether to go to acident and emergancy or wait for an appointment for physio.

    My mother is in so much agony from collapsed L5S1 that she said if there is no pain relief solution she will commit suicide😔 she’s had two failed back surgeries. I don’t know what to do

    Good video.i suffered severe back pain for 25 years I found out auto amune daseese was causing it.i stoped eating bread all grains and sugars after a year and a half my back stoped hurting over 95 percent.and I have returned to normal life.mri said i had 9 bulged and deterated disc . They called it stage 4 spine deasese.i would love to have another MRI to see if there's a change.

    I get migraines from degenerative disc disease and Ehlers-Danlos syndrome causing probs with my spine. They always kick off from my spine nerves.

      In fact, buldging discs are usually not the cause of the pain. How do we know that? Because there are hundreds of thousands of people with buldging discs who do not have any symptom of back pain.
