On August 28, 2009, Mark Saylor and his family were killed in a horrific car crash. The culprit? A stuck gas pedal.
Shortly after, hundreds of others reported similar incidents. Everyone blamed Toyota, and the company ended up recalling 10 million vehicles in just two years. They paid a $1.2 billion fine to the US government and spent another $1.1 billion on a class-action lawsuit.
But what actually went wrong in Mark Saylor's crash? And was Toyota really to blame for all of these mysterious accelerations?

That seems like a massive oversight on Toyotas part. I mean pumping your brake is something that should not stop your brakes from working in any situation especially when your accelerator is stuck wide open. How scary.

Even if. There as a handbrake which is an different systen (for safety) and both automatic and manuals can be put into neutral. Putting an transmission into neutral disconnects the engine from the wheels. Meaning its basicly as if your accelerator is no longer stuck and the car will decelerate

He couldn’t stop the car, and as he approached the intersection, he drove into a ditch to avoid colliding with anyone else.
Here’s a quote from the video that pretty much explains it:
“In the real world, people will actually lift the foot off the brake- perhaps to pump the brakes, to get more power- maybe trying to fiddle with a floor mat.. but the point is, when you lift your foot off the brake even one time, you lose your power assistance, and it becomes almost impossible to stop it.”

Well IF that was your only 2 choices it would be easy. He probably figured if he slams into cars in the intersection him and his passengers are going to die anyway! SO.. why take others with you! Plus you probably have a better chance of surviving going off the road cuz you're not slamming into a bunch of cars or trucks!
NOW... that said, there's no reason they should've crashed in the first place when there were two or three other options that he had to stop the car without incident! Like put the car in neutral and use the emergency brake to stop! Or turn off the car then use the emergency brake! Or put it in neutral, the wind resistance is surprisingly stronger than you think! It will slow the car fairly quick, then use the emergency brake if the regular brakes don't work. If neither brakes work, after the wind resistance has slowed the car he could've got behind one of those big trucks that he passed and hit the back of it. Since they're both moving it wouldn't be much of an impact at all. And the driver if the truck would've most likely instinctively hit the brakes slowing both of them to a stop! There's even a couple more options but this text is long enough!
But realistically, THE MOST IMPORTANT THING YOU CAN DO IN SITUATIONS LIKE THAT'S IS.. "DO NOT PANIC!!!" Which I'm sorry to say it sounded like they were all panicking!

In regards to the brake override system.
The brake override system, also known as Brake-Override System (BOS) or Brake Pedal Override System (BPO), is designed to prioritize the brake command over the accelerator in situations where both pedals are pressed simultaneously. Its purpose is to enhance safety by allowing drivers to bring the vehicle to a stop in case of unintended acceleration.
Technically, the brake override system typically involves sensors and electronic control units (ECUs) that monitor input from the brake and accelerator pedals. When the system detects simultaneous inputs, it prioritizes the brake signal, cutting engine power and allowing the vehicle to be brought to a stop safely.
However, in the case of the Toyota recall incidents, a malfunction occurred in some instances where pumping the brake pedal during unintended acceleration led to a loss of power brake hydraulic support. This means that the brake pedal became less effective in stopping the vehicle, potentially complicating the driver's ability to bring the car to a halt.
The exact technical intricacies and the root cause of the malfunction can be complex and might involve issues with the electronic control systems, sensor calibration, or other factors that led to unintended consequences in certain scenarios. Investigations and improvements were made by Toyota and other automotive manufacturers to address these issues and enhance the reliability of brake override systems.

I had this happen in an 82 monte carlo. Got stuck in full throttle. With a 400hp engine. I shut down the ignition then put the car into neutral. Didn't have power steering or brakes. But just had to push harder. Thats why i don't understand why they keep changing technology that works. If a technology has worked since the 50s. Keep using it. Emergency brakes were on a cable. Almost fail proof. And the shifting linkages were manual. Not in the hands of a computer. Most newer cars are experimental. Always trying new things instead of improving what was already good. Because they're always trying to find cheaper ways to build cars. Do these fancy Toyotas not have neutral? Not once did i hear the dispatcher say try shifting the car. Or slowly put on the emergency brake. Every car I've ever owned has had one. And ive played with all of them. To make sure they work. Especially after fast m furious came out. I was emergency braking everything. It just seems like something else could of been done. Good learning experience for the rest of the world i guess. Test your emergency brakes. Even my van has one. That works. It weighs almost 8,000 lbs. And foot operated. With a cable. So doesn't require an engine. Good old days of 1997. I've never owned anything newer than a 98. But prefer older cars over newer cars. Spark and fuel. No dependence on a computer. Because steel cables outlast circuit boards.

This happened to me in a Toyota Corolla in 2003. I don't know what initiated it, but the gas was suddenly on full, and I couldn't reduce it. I pumped the gas a few times, and it wouldn't come up off the floor. I switched it to neutral, and it reved so high I thought the engine would blow, so I turned off the car to cut the engine and rode the break. I don't remember if it had power breaks or not - it wasn't a high end model, and it was at least 10 years old. I limped it into town by turning the car on in neutral and putting it into drive before the engine could rev too high, let it accelerate me, then coast in neutral with the engine off. I did that over and over until I got to the dealership in town, and they fixed it for free. They said that the cruise control got stuck. It was freaky, and it was lucky it first happened when I was on a country road, and I had a few seconds to try things, rather than in heavy traffic. It was pretty scary when it first happened, and you really do need a few seconds to figure something out. And there could be a lot of reasons it happens, and my solution might not work in all of them.

Coming at this as a non-professional, couldn't he in theory just put the car into neutral and quicky take the key out. I used to own a 2013 Toyota RAV4 and on that car you could engage neutral any time even if your foot is on the gas or the break. If you do that the car will immediately slow down about 5 MPH and you will have about 3-7 seconds to get your keys out of the ignition before the car redlines and blows the engine. Another Idea would be to immediately slide the seat back and attempt to dislodge the accelerator by hand with a passenger steering for you. I would only do that if I wasn't able to stop the engine though given how dangerous that maneuver is. Unfoutantely in the heat of the moment, these solutions may have been overlooked or weren't possible leading to a tragic loss of life. It stems as a prime example that drivers need to always be prepared for emergency situations in their vehicles.

If the brakes get too hot then the fluid will become vaporized, when that happens the brake pedal will become soft and the brakes will fail because the vapor is compressible and brake fluid is not. For those wondering why you would lose power assist brakes at full throttle. The majority of power assist brake systems use vacuum from the engine. At cruise and light throttle you have vacuum that is stored in the booster, but once they used their brakes and use all the vacuum in the booster and now that’s it’s at full throttle there is no vacuum anymore. This does not happen with vehicles equipped with hydro boost or electronic hydro boost systems that use power steering fluid pressure to assist the brakes

You lose the whole vehicle with that at least in neutral you still have control via power steering. And yes first reply is correct about ebrake, doing that at top speed is donezo. I looked at it and found out when the cruise opened the carb to maintain the speed the linkage had gotten stuck so even though the cruise had backed off the the linkage caused the carb to still be open to far

This happened to me about a month ago, in far less dramatic a fashion, thank God. And honestly, the outcome of my own situation, was due in large part to this family's tragic death.
Long story short- I entered my 2011 f150, started it, depressed the brake, recognized stiffness but didn't think much about it, shifted into drive and immediately lurched forward. I tried to brake. Nothing. I tried to stand on the brake. Nothing. Meanwhile I'm accelerating at a relatively slow, but constant rate. After my mind caught up, I shifted into neutral, my truck slowed, engine revved. I opened my door, got out as it slowed, and pulled out the floor mat. I was down the street headed toward a T intersection with houses everywhere. Scared the hell outta me. This whole ordeal lasted maybe 4-6 seconds. Estimate.
    Take-aways for anyone who cares:
    - Double check your floor mats- I thought I replaced mine (plastic, molded) correctly. Obviously, I didn't. Somehow, the corner or edge of the mat caught my accelerator and was wedged behind the brake.
    - Don't be complacent- I felt the stiffness in the brake but didn't do anything before I shifted. I should have.
    - Shifting into Park might not work- As I was accelerating out of my driveway, in my near panic, I passed Neutral and went straight to Park. The truck did nothing.
    - It can happen to anyone- I'm a firefighter. I'm used to making decisions under stress and bad situations. Mr. Saylor was a CHP officer. This can happen to anyone. Just keep your head, and shift into neutral.
    - This family didn't die in vain- I'd heard this story before. It was in my subconscious. While I wasn't in fear of my life, I was in fear of taking someone else's. Playing this scenario out in my head several times before, because of this tragic accident, is the only reason I was able to regain my composure and react.
I pray God brings comfort to their families and rest to their souls. Hopefully this helps someone out there. Take care everyone and be safe.

Knate, when you accidentally shifted into Park, nothing change? The motor didn't Rev? Am I correct in assuming you did that first and then you came back from Park and put it in neutral?
Also does that year of F150 have a key or the ability to turn the ignition off while you're driving? I understand in your Panic maybe you didn't think of that but it seems like that should be a common part of training to get one's driver license. Perhaps I'm wrong but I think every car ever made has the ability to turn off the engine while you're driving whether it's 5 mph or 100 mph. You might lose the power braking assistance because of lack of vacuum from the engine, but the brakes will still work
Also can you make it more clear about the mat bunched up under the brake? Was there so much of the mat shoved under the break that it was strong enough to physically hold the brake pedal up and keep it from being depressed even when you stood on it?
If that was the case then that's pretty serious and you're correct you should have paid attention to it, even turning the key off would not solve that problem, although at least it would keep the engine from continuing to make you accelerate.
In conclusion, after reading so many of these comments and listening to the so-called experts in the video it still boggles my mind why nobody thinks of turning the key off. If the people in the Toyota had done that, especially on a straight open stretch of highway, they could have easily pulled over to the right and coasted to a stop
