      Some people, such as myself are visual learners, so drawing as he explains is a better way to learn. Also, I do not think he explains as he draws, I think he draws and then does the voice over.

    I was so confused about the mechanism of breathing and when I came onto YT to watch videos, this was the first video recommended but I dismissed it to find like an actual video showing the lungs moving and a visualization.
    3 visualizations later, I was more confused and angrier, I eventually came back to this, it was worth every second. The way you explained patiently even diverting sometimes just to make it clearer, I love this so much.
    Thank you 🧡

      Enxhi Tepshi I agree with you

    • @govindnair5152
      @govindnair5152 9 ปีที่แล้ว +82

      Enxhi Tepshi You can look at it this way.
      1. Pleural pressure is the vaccum created so that the lung will be forced to expand. So as the lung expands it automatically draws in air via the respiratory tract.Thus we are able to breathe.
      2.The resting pleural pressure is negative 5mmHg. It is kept so as to help in the next inspiratory cycle. View this as a continous cycle,if you look at this concept phase wise it will be harder to grasp.
      3.Normal change in pleural pressure: At end of expiration (--5mmHg) TO At Inspiration 0mmHg (As air rushes in to abolish negative pressure alongwith expansion of lung to compensate and occupy the vaccum created) TO -5mmHg on expiration.
      4.So imagine if the pleural pressure has changed from its normal negative 5mmHg to a positive value of lets say 10mmHg. This is equivalent to someone pushing on the lungs to prevent its expansion. If there is no negative pressure,the lung has no reason as to why it should expand as the pressure in the pleura is already positive.Hence you will also not be able to draw in air from outside.Why should it..when there is no no negative pressure to be matched.
      5.This is what happens when we have conditions like Pneumothorax(Air in pleura) or Hemothorax (Blood in pleura) etc. Irrespective of the material occupying the pleura, all have the same effect...they destroy the normal resting negative pressure of the pleura. Therefore such a patient will be seen gasping for air,with the help of accessory respiratory muscles as he struggles to force air in by creating at least a little negative pressure with the help of repiratory intercostal muscles.
      I hope this helped. Cheers

    • @shteenaM79
      @shteenaM79 9 ปีที่แล้ว +7

      +Govind Nair wow!!!!! this is an amazing explanation, you have enabled me to visualize this like never before! thank you God for smart ppl like you. I understood mechanisms of breathing but that negative pressure in the pleural cavity was so hard for me. so happy I finally get it, you have no idea what you've just done for me!!! thank you

    • @ColinBlumenthal
      @ColinBlumenthal 8 ปีที่แล้ว +30

      +Govind Nair This explanation was incredibly helpful!
      One quick correction though. The intrapleural pressure doesn't reach 0 during inspiration, it actually becomes more negative. Intrapleural pressure goes from around -5 mmHg at the FRC (airway pressure is 0 mmHg) to around -7.5 mmHg at maximal inspiration and then back to -5 mmHg. This decrease in intrapleural pressure allows you to create a slightly negative alveolar pressure that draws air into the lungs.
      Stated in another way, since the alveolar pressure is 0 when the intrapleural pressure is -5 mmHg, the intrapleural pressure must become more negative or you will not have a negative pressure gradient in your airway to allow in the influx of air. This decrease (more negative) in intrapleural pressure is essentially transmitted to your alveoli to allow your lungs to fill.
