    Dr. Fazal, excellent presentation! Thank you for the clear explanation. My mom is 89. Recently she has a very low DIA reading, in the 40-50, and high heart rate at 90+. She is feeling very tired. The doctor recommended her to do blood work and ECG. But she refuses to go because she is tired, fear in a room full of people, and she does not like all the trouble. In the last 2 weeks, she also has edema in her left foot. The one thing I feel hopeful is that her heart rate has reduced from around 90 to 75-80. As stated in the last part of the presentation, the goal is to slow down the heart rate so that the left heart has more time to fill in with blood. I hope I am understanding it correctly and that may indicate at least her situation is not rapidly getting worse. Thank you again!

    Just out of hospital. Felt terrible there, was given high dozes of Furosemide (diuretic) for days that appeared not to work for the odema in my feet and ankles and infact made me weak and get terrible kidney pains. Now I understand why at 14:00! You explained it very well. Thank you!

    Very good presentation. I would like to have clarification of the need for a BiV pacemaker with diastolic heart failure? Thought it was for reduced HF with a wide QRS. AICD also for systolic HF. Maybe meant if diastolic condition progresses? Thank you

      Biventricular Pacemaker (Cardiac Resynchronization Therapy, CRT)
      BiV pacemakers are typically used for cardiac resynchronization therapy (CRT) in patients with systolic heart failure, particularly those with a reduced ejection fraction (HFrEF), who have symptoms despite optimal medical therapy, and evidence of dyssynchronous ventricular contraction (such as a left bundle branch block on ECG).
      Automatic Implantable Cardioverter-Defibrillator (AICD)
      Iindicated for EF less than 35%), or those at high risk for sudden cardiac death, generally associated with systolic heart failure
      The use of BiV pacemakers and AICDs in HFpEF is not typically standard and is less clear. These devices are not generally indicated unless there are coexisting conditions like significant ventricular arrhythmias or other specific indications that warrant their use.
      I was wondering why it was mentioned as standard for HFpEF

    Pls help me out to understand , I have gone for CT abdomen while contrast istarted feeling Bee is biting and I woke up and fainted, docter given me CPR as they were saying u had low pulse and bp, after waking up I am ok, my all votes ok 2d echo, stress test all ok. Pls help to understand what happend to me it was syncope or other thing, is there any life threat in future. I feel like I am in post trumic stress. Feeling very scared. Thank you if any one help.     Left bazu me boht pain hai baqi chest koi pain nhi hai... Medicine beta blockers wali kha rehe hain kya krna chahiye???

    Beta Blocker or Calcium channel blockers or Ivabradine all these help in reducing heart beat.

      There is an increased sympathetic tone in a failing heart.
      Thus only beta blockers help but not others.

    Hi Sir, is it possible that someone has normal systolic (EF 65%) and diastolic function can have heart failure? My wife's Echo is normal but her NT proBNP is 450.
    She feels breathless sometimes and there are bigemni and trigemni in ECG.
    We are tensed due to this and doctor is unable to predict anything from it. Thanks in advance.

      Yes my friend he is 67 got covid 3 times and suffered heart failure he is lucky he found a good caring cardiologist who placed a device in him by his neck this has allowed him to live a more normal life, as for me I'm female and I have PPO plan and still cannot get a diagnosis for my condition I am end stage heart failure on oxygen from 5 years ago condition only worsenin and I sure to die sudden death thank you all doctor's who have charged my insurance Maxx but provided no care

    My husband is 42 and just got diagnosed with diastolic heart failure. His function is 50-55. Can a person live a long life with medication and life changes? Or is this a life sentence and life expectancy is very short? 😔

      Diastolic heart failure can occur due to restrictive cardiomyopathy as seen in amyloidosis sarcoidosis. It can also be seen in cardiac tamponade constrictive Pericarditis.
