This is absolutely they way to go - have a centralised plan, build up rapport and set up relationships to allow for a coordinated collaborated team that are all on the same page and heading to wards the same goals - Keep going Gold Coast primary Health Network, the only was to fix this is to have a working relationship with the unwell and the health professionals mapping it out on a case by case bases - one peg does not fit in a square hole.
