    This comment might be off-topic. But watching your videos made me realize that this world is a better place because of people like you. Thank you for everything.
    We had no biostats course in our medical school. Your videos not only helped me ace my Step 1 and Step 2 Ck but also made me a whole lot more interested in Biostatistics. I will take Step 3 in a few days and prepping for Day 1 now.

    Thank you so much doc.! you don't really know how much impact you have already created in my quivery biostats world. I had difficulty through out med school and even on step-1 but, when ever I had to review biostats I straight away watch your videos. They really helped me and reduce anxiety in solving biostats questions. I don't think there are any other better ways to explain these confusing concepts. Thank you keep doing these videos. Don' forget there are many students like me who are willing to watch your videos.

      @@RandyNeilMD your videos have covered biostats and most of behaviour science topics. I have watched all of them and they are very useful.. Would highly recommend to others. One thing that I think can be added is ethics section. Since IMGs are new to this subject and I personally face issues solving ethics questions. So it would be great if you add one ethics video addressing the common ethics question. Thanks again for such great videos..♥️

    Dr. Neil... At 24:48 it says no CLINICALLY significant difference in BP REDUCTION b/w 3 doses. It doesn't require calculation. They seem to reduce BP, but they all bring it down by 8-12 mm Hg. The difference in reduction b/w them is 1-2 mm Hg. So, the difference of 1-2 mm Hg b/w them is not CLINICALLY significant.
    Statistical significance and clinical significance are different concepts.
    Please correct me if I'm wrong.

I also was looking for some coverage of the hazard ratio in your videos and from your comment, it sounds like you already covered this topic before, but I couldn't find anything when I searched "Randy Neil hazard ratios". I'm studying for step 3 so I'm very comfortable with a lot of the biostats basics and I was hoping I wouldn't have to watch every single biostats video. But it's hard to figure out which topics are covered in which videos to focus my time on the topics I need the most help in. It would be infinitely helpful if you would tag your videos with the biostats principles covered in each video so they'd show up in the TH-cam search better, or potentially list them in the titles/thumbnails like you already do for some. I know that's a lot of work and probably won't happen before I write step 3 but I figured I'd comment anyway in case it helps others. Thanks again!

    Thank yo for your video Dr Neil.
    I had one question of the Essepro question about the clinical significance. I’m unsure whether the Z value helps us with clinical significance. I believe that would help us understand the statistical significance. The clinical significance would require more data (to calculate the Cohen’s D or other routes), which isn’t meaningful since the difference of 2 in BP sound’s clinically insignificant. I believe it’s heuristic. I maybe wrong and would appreciate your insight here.

      I see what you are saying... When I see the large P-value I think they may be referring to the Drug vs. Placebo... Whereas the question is talking about between the medication doses themselves... I like your thinking though.

    Thank you for such an amazing explanation. I have a question regarding 2 problem part 3 (item#15).
    For excluding option C you applied the whole formula to get z value of less than 1.96. What if we just compare the p values of median survival for class B and class C? p-value of class C is 0.05 which is obviously greater than class B (

    My interpretation of the last question:
    (A) ↑sample size (N) correlates to ↑power and ↑statistical significance → comparing 4xx participants to 159 participants (placebo) exhibits relatively greater generalizability and validity
    (B) Doubling 2.5mg to 5.0mg of ESSEPR) brought about a 1-2mmHg reduction in DBP and SBP. Doubling it to 10mg may or may not have the same effect (vague)
    (C) Higher doses may or may not offer less/greater benefit (vague)
    (D) Graph itself shows very poor clinical significance BETWEEN the 3 doses (1-2mmHg of improvement based on each dose)
    (E) p value is high
    Hope I was right.

    Scan for math

    Exclude bad choices (power>.05 of CI of 1.0)

    Hi Dr. Neil! Quick question about the problem at 25:00. I'm wondering if _"there is no clinically important difference..."_ gets at the concept of *clinically* significant versus *statistically* significant. In this case, the range in SBP between the 1mg and 5mg group is only about a BP change of 2 mmHg from baseline, which isn't clinically important. In this case you proved that there is no statistically significant difference between the groups. So in this problem the difference isn't clinically OR statistically significant. But in other problems, I'm unsure if we are supposed to equate statistical with clinical significance.
    For example, in this problem, if the results were slightly different, you could theoretically get a Z score greater than 1.96 if a=7 and b=1. In this made up example, the BP change is still only 6 mmHg from baseline which, while statistically significant, still isn't really a meaningful reduction in BP. Just wondering if I am missing something or if you had additional thoughts. Thanks for your great videos! They are super helpful!

      Spencer, Great feedback... I am hoping that they (USMLE) don't live in any 'grey area' of what is meant by "Clinical significance" other than if it was impactful treatment result, which can be somewhat subjective (ie. 1 less day of symptoms for a cold may be statistically significant, but not clinically per se)... The formula that we discussed for statistical significance is a "quick formula" that does come with some disclaimers (equal distribution). From my take away the clinical significance would have to show impact to the treatment, which leaves it a little subjective and not as concrete as 'statistical', therefore we can 'eyeball it' imo. I am hoping for everyones exam it's easy to determine and something obvious... I hope this helps.

    In question 59, choice number 1 isn’t the best answer? Because they mentioned that on the last line of the question she can get 3 month supply of medication for free?

    Thank you so much, I hope we'll be seeing more of these. Regarding the second example, question 60: Answer choice D refers to "clinically important difference", as opposed to "statistical significance", which you calculated for that option. I always considered these two to be different things. As in, we don't really care if it's statistically significant or not, we are only focusing on whether the amount of reduction in blood pressure is likely to have an effect on their health, clinically. Could you please comment on this?

    Hi there,
    For question #60, it seems that you may have done unnecessary calculation... Answer choice C is referring to the clinical significance, not the statistical significance, so couldn't we just have eyeballed the graph, determined that the BP reductions are more or less equivalent across groups, and arrived at the same answer without doing any calcs?

    For the BP question, the answer choice you picked said no clinically significant difference. This means calculating the Z wouldn't be needed right? Because that's for statistical significance, no? Either way clinically, a numerical change in BP of 1-2 isn't clinically significant. Studying for step 3 right now and your vids are great!

      Amna, Thank you for the feedback. The hardest part for me in the process of "drug ads" is finding examples... Hard to create from scratch and I don't have access to Qbanks anymore... Biostats should be our guaranteed points on the exam...
