The Horizon Signal is triggered by a black hole system, not a wormhole. You can't control the chance of The Worm spawning, it's a flat 1/20 chance that a Horizon Signal black hole will spawn in your galaxy. Your save gets a specific flag if you have a black hole that can trigger it.
You can use a save file viewing program to check every save and restart if the flag isn't there. Just viewing the file doesn't flag the save for disabling achievements.
My partner has been having issues with steam taking up to a day for it to recognize that you've gotten certain achievements, so you're not alone in it being weird.
I remember the luckiest acheivement I ever got was invading Sol III in the middle of WW2. I wasn't even looking for it, but I haven't seen it since. At around 13:37, you got a pop up for Zro, but didn't go for it. It'd be cool, next time you have victory on, if you also try to trigger a machine uprising, and win the game by playing as the uprising instead of your starting Empire.
Ah! First contact war requires you destroy ten ships before first contact finishes. I got it off of crystals before :) The wiki has more details on what does or doesn't trigger things

I'm a little unclear on what counts as a full fledged first contact war, because I got it by mistake while exploring with my military ships as an aggressive empire once. However I can tell you, since you mentioned the Pre-FTL achievements: "With Great Power" ("Have ten observation posts over pre-ftl's as a necrophage without invading any of them", there are other ways to do it, but this is the quickest) is most easily done by setting pre-ftl's and habitable worlds (guaranteed and general) to max. You'll have a bit of a slow start because most of the worlds you find will be pre-ftl's you can't invade, but once you have all ten you can nom them up at your leisure and make up for the slow start with a big spike in productivity. Be aware that if the pre-ftl's reach the Empire stage before you have all 10, refusing to give them back their system (and auto-conquering them as a result of controlling the starbase) does NOT ruin the achievement, but you do lose one of your observation posts so it can slow it down. You can also set your species to Necroid for this, and force spawn a fanatic xenophile pacifist plantoid empire to get the plants vs zombies achievement. Note that when combining going for these two achievements it's quite important that the plantoid empire is pacifist fanatic xenophile, as these traits mean they won't invade the bounty of pre-ftl's they can find and gain a big advantage in production while you're still stuck observing them - you're essentially applying the same nerf to them that you start with to eliminate the chance that they race ahead in a way you struggle to catch up to.
I don't think I've tried this on pc yet but I think I've read that it should still work. You should be able to open a relinquary, see what you get and then reload to last save without closing the window to avoid saving. I could get multiple tries per minute with this. I'm not happy to use this technique but it might be among the quicker ways to get the Galatron. It might also take a hour or two.
After trying a few runs to get a few more achievements, the galaxy just explodes or I unplug and start anew…so…the same effect. I “salvaged” Bubbles, but it wasnt fully grown so…whoopsie. Then as you did, i murdered the crisis too quick for the brood queen. Menace…intensifying…
The Fallen Spiritualist Empire, gets mad if you colonise a holy world without their permission..... I wonder how miffed they might be if you were to World Crack their Holy Worlds ?
Yup, as other commenter said - the reaction to that is coded into the game. If you crack a Holy World, the Fallen spiritualist will immediately awaken and declare war on you. From a RP perspective you outraged their entire population so much that they collectively decide to get off their padded meditation cushions and rebuild their fleet just to make you (and probably everyone else) pay for it. If you're aiming for a war in Heaven, and you're ready for a war in Heaven, it's a great way to trigger one. Just keep in mind that cracking a planet also has a diplomatic penalty with almost everyone else, so you need to consider if that's going to mess up your search for allies against the sleeping giant you just kicked between the legs.
At some point, you just can buy Reliquaries for (increasing) Energy cost and without cooldown. It's just a matter of time and large pockets, in the end... BTW, it was entertaining to see ya going mad on your Achievement's Hunt. Cruel to say, but still very very true :P
I really wish this achievement would specify "Own a fallen empire's four holy worlds. I can't count how many games I tried consecrating my own 3 planets into holy worlds only to them be stuck on how to proceed. :p

Bro, stay on target is impossible. Not only is it rare, but they stop firing and run immediately. I’m going to try a stealth fleet and see if that can succeed. But I have failed so many attempts prior to stealth tech 😭
Yeah bro, I’m stuck at around 124 achievements and each dlc puts me further away.😢 Some are so specific and rng driven that it’s insane - looking at you galatron, 5 years and 1.5k hours of playing and it still eludes me! 🤬
None of my friends play stellaris 😢, but the humble pie trophy feels like a team thing as the ai just doesn’t go supremacy stance in any of my games ever - not even fanatical militarists 😢
On at least one occation, I didn't get an achievement, in frustration I quit. When I then loaded back in later in order to try to find out what I did wrong, I got it upon entering the save again. I dunno what stellaris and steam smoke together (literally no other game in my history have I ever seen achievement trouble like this).
The "old friends" is when you receive a gift from enigmatic observers or keepers of knowledge after saying yes to one of their requests

Wait, it's in the save file whether one of the black holes holds the Worm?

Couldn't you make your own holy worlds as a spiritual empire? Or do those not count?

You can't load saves in Ironman, the game autosaves and you just get to use your exit save.
