There's a series of events that has to happen for a nuclear bomb to become fully armed and explode with full nuclear capability. If any one of those steps doesn't happen, the nuclear device doesn't arm. Even if the conventional explosion goes off. This has happened several times in the u.s. actually. Since all of the steps and events hasn't happened, we've never had an accidental nuclear explosion.
I thought this was going to be about the Aeroflot plane that jettisoned an unruly cow that landed on and sunk a fishing boat. Not sure if it is a true story or not, but I hope it is.

Such would happen again. Mars Bluff, part of Florence, SC. A B47 accident where, yep, a nuke hit. USAF paid for that, too.
