I'm a referee. If you are wondering why some holds are called or not it depends on whether it gave an advantage to the offense. If there's a hold on the left side and the back swept to the right side, its not going to be called. If the OL holds 2 yards past the LOS and the back goes down at the LOS, it will not be called. But all safety fouls are called. Any kind of illegal material restriction that has an affect or could have had an affect on the play should be called.
I played 1AA and had a few seasons starting at oline. My technique was grab the pads or jersey at their chest area. I only got one holding call the entire time and the oc was upset with the ref, because the oc couldn't see holding on the multiple video angles we had.
It is not a hold if the defender does not wanna get off. If he stays in your grasp, good for you. That is never called. If he makes a move and you don't let go, that is obvious holding.
This is one of the reasons I quit watching football, other sports too, because there is no integrity or sportsmanship anymore. The it's not holding, clipping or unnecessary roughness or any penalty, because it didn't happen if you didn't get caught. If you're not going to follow the rules, why not just lose the pads and call it Rugby

Most of these lineman couldn't have succeeded with pre 1978 NFL rules; when offensive linemen were not allowed to use their hands.

The holding rule should be time over touch. If one holds for at least 4 seconds, it should be a penalty. If it is less than that, than no penalty.
What referee going to have time to count on one players and they have to focus on many
4 seconds = a year in QB time
And the things the refs would miss elsewhere on the field during that 4 second count..........
