Iam not totally sure about my answer . So , please forgive me if I didn't do well.
I think this condition is a Vertebrobasilar disease which describes a variety of conditions affecting blood flow to the back of the brain via the vertebral and/or basilar arteries and it's common in elder people who have atherosclerosis . When this patient rotate his head suddenly , the vertebral artery is compressed by the action of some muscles of the back and suboccipital triangle and these arteries become narrower due to atherosclerosis . So there isn't insufficient delivery of the blood to the posterior structure of the brain particularly those responsible for movement and balance. And that's why he fainted .

عاشت ايدك يدكتور و اخيرا كملتنا الهيد و النيك🥰🥰🥰 الinteractive Q ما عرفت احلة بس اول ماخطر ببالي ان external jugular vein صار بيه خلل او تضيق او blockage شوية بسبب ان سوا rotation و حرك ال sternoclefomastoid اما التفسير الثاني الي لكيتة ان صار اكو خلل بال third part of vertebral aretery بسبب ال rotation من muscles الي بال subocciptal tringle ع اعتبار ان third part of vertebra artery يمر بال subocciptal فقلل كمية الدم الواصلة للدماغ لحظيا ف حس بشوية دوخة ...بلكي صح لان محتاجة يكون صح 😅

so the only finding I got is; Benign paroxysmal positional vertigo (BPPV) is an inner ear disorder in which changes to the position of the head, such as tipping the head backward, lead to sudden vertigo - a feeling that the room is spinning. BPPV is not a sign of a serious problem. If it does not disappear on its own within 6 weeks, it can be treated with a short, simple in-office procedure.
