Deciding Which Organisms to Treat: Chronic UTI Treatment, with Dr. Stewart Bundrick, Part 1

  • Chronic UTI is considered to be a persistent infection that has not been effectively eradicated with treatment. Many of those who experience this, report symptoms that are always present, and these symptoms have a significant impact on quality of life. Research into the area is still in its early stages, and clinicians are often in a position of having to recommend treatment for organisms that have previously been considered uncommon in urinary tract infection.
    In this video, Dr. Steward Bundrick discusses how he decides which organisms to treat and the tools he uses to support these decisions.
    ● As a starting point, Dr. Bundrick will typically undertake a urinalysis test, a urine culture test and a MicroGenDX DNA analysis.
    ● If the tests return no insights, he disregards them and considers the patient’s symptoms, he will recommend treatment for the most common cause, E. coli.
    ● Because E. coli is resistant to penicillin, he will recommend Augmentin or Bactrim. Or, if a patient is allergic to sulfur, he recommends Trimethoprim.
    ● DNA-based testing methods can return 15-20 organisms, Dr. Bundrick focuses on those organisms known to cause UTI. E. coli, klebsiella and enterococcus are the most common.
    ● Dr. Bundrick explains the importance of taking a thorough patient history to establish whether a UTI is an embedded infection or a re-infection as symptoms may be mild and relatively manageable.
    ● It is rare to require testing for physical abnormalities. However, if there is blood in the urine or persistent pain, further testing can be done.
