NTA. But, I would actually get a hair follicle test done, then ask a lawyer to write a notice of action against your sister. Have him note that you will sue for defamation if she cannot prove that she has not taken drugs through a follicle test. This will raise chaos in the family, but as your claims are legit, it will prove to your family that she has been manipulating them. At the very least, you can drop the case after getting the peace of mind of them knowing why they won't be invited to the wedding.
If I recall a defamation lawsuit only works if you can prove that the defamation has caused you financial harm. As far as I know being disowned by one’s family would be considered a family matter by the courts.

ESH, OP isn't an AH but i disagree with keeping quiet and accidently enabling her substance usage, i understand the intent but they were misguided, the sister for lying, and the family is easily the worst party here OP is honestly better off without them at his wedding or in his life
I don't agree with the ESH. The sister and family do but the OP's actions make sense. OP wanted to helps his sister where as he stated his family has given up on addicts so, I wouldn't say it is enabling either if OP was truly trying to help. Also, if the OP did speak up there is no guarantee that if his sister simply said "No, it is actually OP" that they wouldn't believe her.
