The balck plague ravaged europe from the 13th to the 16th century, well before there was formal medical doctorates. Ergo the plague doctors as seen in their popculture depictions are not licensed professionals in any modern sense. They may have learned under either a church healer of some form, likely a nurse (which originally meant nun), or some village alchemist, and certainly had a greater than avrage grasp of what was considered medicine at the time, but it could in no way be compared tot he standardized courses provided by universities even of the 18th century.
