The reason killua isn’t as confident as before is because before he always could win. But his opponents are stronger than the opponents he faced before. That simple.
It's this. He was confident with any opponent that DIDN'T have nen. But once they started fighting stronger opponents, more of them qualified as being dangerous and triggered the radar/hypnosis
not exactly its the needle doing its job, he needed a powerfull motivation to overcome it, he was willing to die to let gon escape when they where captured by nobunaga.

I think the point is that he seemed so bad ass early because they didn't have opponents that truly threatened Killua until Illumi obviously, and then essentially anyone with nen until he learned it himself (like zushi), and after he learned nen it was anyone he would consider stronger. It may give off the notion of inconsistent writing, but it was really just the nature of the opponent power creep
Considering Killua doesn't remember when/how it happened, Illumi definitely implanted the needle when Killua was little. Remember Illumi's first reveal at the end of the Hunter exam? He was reaching towards Killua's head, probably using Nen to active the needle and manipulate him.

That's why a major theme throughout Killuas story arc is how he deals with his family and breaks away forging his own path while his family tries to control him and his actions to make him into the perfect next head of the family
Pretty sure Illumi planted the pin when Killua was much younger, which is why when he fights someone with a stronger aura than him, he kept getting scared. it didn't affect him against other opponents, such as in the hunter exam or in the battle tower or whatever, because he didn't see them as stronger than he is, but the pin was definitely there.

His entire family loves him and knows he will become the strongest and they want him to take over the zoldyck family, thats why they are very protective; illumi put the needle in order to protect him incase he encountered a strong enemy while he still growing.

I think Killua didn't try to kill Palm because he know she's not an enemy, she's just crazy lol but at the end, they're all after the same goal and fight for the same things. Besides, that would have put him in trouble with the Hunter Association AND Gon would have been upset. But yeah, you were right, it just didn't sit right

You can see his ears drop, the sad look on his face, and he even basically says, "f*ck it, lemme stop torturing this kid and kill him, once and for all" almost as though he's showing mercy.

This was always foreshadowed when killua felt overwhelmed for example with illumi/hisoka or the hunter that trained them nen and killua jumped into the corner the moment the aura became hostile. 2 reason Killua can't kill Palm. First, Gon already considering Palm as his friends. Second, Palm is an Enhancer which is stronger than Killua in terms of strength and physical defence. And more over, she is professional hunter who always doing her mission alone and always came back unscratched. If There's no condition for partnering with Gon and Killua,, she could handle Knuckle and Shoot herself

It's supposed to force him to run away because Illumi, as twisted as he is, still loves Killua but for his own, self serving reasons... Once Killua removes the needle, his confidence returns. Morel said a while back that nen battles aren't based on who's the strongest all the time. There are various factors that go into a fight. His urgency to run away from anyone stronger is one of the reasons Bisky wanted him to separate from Gon because she felt he'd leave him for dead, eventually.
I mean her being 22 trying to go out with someone who is 12 should have been the indication of something being wrong with her from the get go LOL. Her character would've been so badass as one of the few female characters as strong as she is on HxH but the writer kinda fumbled the bag with how he wrote her.
