You are the first person to really explain this issue of the external rotation of the tibia, that I have for 20 years, thank you so much for explaining that!
I have my right knee knocked in I didn't realize that the right pelvis is externally rotated After watching this video I realized that I wasn't using my right core . Now I can fully feel my core while walking😅. This channel is a great help to me❤
From birth, and genetically my pelvis is both more open and tilted. I have mild hip dysplasia right more than left. Severely Anteverted Femurs with Tibial Torsion. My knees are as you mention. I am 44. I have never had surgery to correct anything. As an adult at age 21 when I got the bone twists diagnosed when I went to a doc... The doc said bilateral Intertrochanteric femoral osteotomies. 6 months in a half body cast and relearning to walk. Even with my husband's insurance that would have been $50,000 outta pocket. Not able to afford, and got scared. The doc predicted that I'd have arthritis in the knees at 30. I tried to get surgeries later at age 33. But no doc would do it. But no arthritis at that age either. I am now 44. My knees have hurt since I was 12yrs old. They are the coldest part of my body. They go numb, throb, stabbing pain, creak, wobble, or refuse to hold my weight. I went on a 2 mile walk on Friday. I have not been able to stand up with out the feeling of them buckling since. They are creaky, constant pain, cold even in this 80°+ heat, and occasionally have a sharp pain. I see my doc Thursday. How long can I last on knees with the opposing forces that my bones have given them all these years? I used to be active, as in walking a lot. I once walked 14 miles in one day (off and on all day) but the last 4yrs I have been in a mental slump. I got up to 200lbs, though now am 185. The 2 mile walk is now far according to my lungs and legs. I am scared, and am worried about the knees structural integrity after 44yrs of this.
What a gold mine. Fantastic body mechanic awareness. Great explanation from a fellow PT! Edit to add: What resources do you use for learning biomechanics? Anatomy trains? Something else?
Hey Conor, your explanations are great. Does it make sense to train both? Just to give your hip a general mobility, for let's say you don't know your exact problem or you have a torsion, so one hip is forward and one backward rotated?
I think this is what I've been dealing with for almost 30 years and I fixed it a couple of times not knowing exactly what I did that fixed it but it always comes back. I've felt my femor pop as the quad muscles and patella shift over the femor and I have instant relief like my knee is normal again but I have trouble maintaining that position. When I tell my ortho's and PT's , they don't believe me and have told me so. Left leg femoral internal rotation with external tibial tortion with pain on the top left of the patella.
Ahh okay. So the first example fits my pathology in the leg itself structurally. I also have a large equinous though. So my pelvis is anteriorly dipped, but its also extended forwards so the legs are splayed outwards. I guess its not correct to say my legs are externally rotated, the left one is whereas the right is internally - to whatever degree possible both sides. Any advice on anything i can do to take some strain off my knee in the meantime? Im basically waiting for tendon lengthening surgery, but my right knee is takimg a battering in the meantime.
SERIOUS QUESTION!!!!!! what specialist will sit down and show me all this you are instead of telling me in person. is there a certain specialist or a certain word we have to use for our chiro/ Pt to SHOW us whats wrong with our body instead of saying " you have this so do these exercises "
A Chek practitioner level 1 or above will look at issues like this and coach on measures to correct it. I'm not sure about the title for others who do this his type of work, but it would likely be under the term corrective exercise
I have the first condition. huge internal hip rotation, knee goes inward and my tibia is going outwards with my foot. Can't balance on my leg I have an anterior knee pain and swelling under my knee cap. Bad tracking. How can I fix it? Is it ever curable? only Right side.
Hey Conor, question is in this video you mention that when the pelvis is in the forward pelvic tilt position the femur is internally rotated, but from other videos (left aic pattern) why is the femur externally rotated when the pelvis is in forward pelvic tilt?
who can i go to that can tell me what issue(s) i have like this? internally rotated, extternally rotated , hio alignment etc? I just KNOW my alignment is way off and the outter lower knee area is bulging and causing inflammation reallly badly, constant recurring knee effusion! :(
Im having a hard time tracking all the new words (English 2nd language) I have the first condition demonstrated in the video, what exercises should i be doing?
Dear sir... I'm 30 years old and my right keen hurts ... Looks like I've hip internal rotation.. my legs look like bow legs because of this... I'm really scared of having some serious knee issues ... Please help me... What should I do?
I've been trying to search some stuff about just this! I'm the first person and have struggled for a long time to feel my hamstring on the side of internal rotation of the femur. Just to clarify, when doing your staggered stance RDL, video is one arm db, the pelvis side you're trying to fix is forward?
As some with patella tracking issues/patellofemoral pain.. I have ER tibias, lack of hip ir/pronation, small athlete wide pelvis.. what do you think I need to work on?
Hey Connor, great info. I’m in the camp where my femur at the hip is extremely externally rotated and when I deadlift, both my femur and to a greater extent, my tibia externally rotate causing extreme torque. This is similar to your second example, however, I have extreme APT not PPT as you described in your example. Also, my internal rotation is pretty well non existent. Do you recommend I do the wall 90/90 and staggered stance dL even though I’m in APT?
Can somebody explain me why a tilt in pelvic/hip flexion results/requires internal rotation? Is it because the femur attaches in an angle and if the acetabulum faces more downwards the femur has to follow or why?
Hi Conor - do you have an email or website maybe a place I can purchase videos? Do you have a video about chronic hip internal rotation and perhaps tight and shortened glues piriformis and TFL . You have a video about lack of internal hip rotation , however my left hip is stuck in internal rotation. I looked for a video about this but I can’t find a video about relieving too much internal rotation (both my femurs are IR and tibia ER) :/ can you respond with a link? I found a comment about my request you said you’d post a vid but Ali didn’t find it. Also really interested in your biomechanics course. As you can tell my mechanics are not great 😂
Big time IR on my femur and big time ER on my tibia, only on the right side. My right foot turns completely outside when I run and then not only I have poor running mechanics but also have had problems with lateral knee pain and outside foot pain. Any suggestion of exercises I should be looking for? Just heard you on PJF Podcast, great stuff!
I've noticed my right foot isn't quite in line with my right knee would this be a result of internal rotation of the femur and what can be done about this? I'm 24 and wondering how much I should be concerned haha.
On your video about the pooch bellies, how do i fix it? I'm a skinny guy with a big belly. I'm convince my issue is posture related, i def have apt. I watched your pooch belly and breathing video but am still confused. What exercises do i do and how many times a day? Any idea how long it might take to fix it?
The first issue you addressed in this video is for sure what i've been dealing with for years. Always been having knee pain. Eventually got pictures taken and MRI and I already have starting decline on the medial side of both knees. 😢. PT made me do vmo strengthening to address the tracking of my patella. and other exercises but the knee pain doesn't go away. At this point I'm positive this doesn't help because of the stance of my femur and tibia apposed to one another. Would these exercises be enough to help me get rid of knee pain or am i done for? I am only 36 so the depression is real..

This video is so helpful. Thanks for explaining so simple and concise
Thank you so much, this explanation has been enlightening. It has helped me so much with my left foot issues.
Wow that makes total sense. Thanks brotha
Ugh....wish there were links to these separate exercises in your videos, or in the caption. :-)
Superb explanation
Glad you liked it!
Brilliant! 👏
Thanks for explaining this so clearly.
Can you do a video on barefoot or minimalist shoes and their effects on these muscular/skeletal issues?
Extraordinary superb tnks
Super as always
This super helpful, thank you!
Hi ..could u throw more light on lateral knee pain ,causes and exercises to heal it
Yes please could u 🙏🏽
I have the lateral pain in my knee but I have the first one knee figuration you showed in the video the most common one
What a gold mine. Fantastic body mechanic awareness. Great explanation from a fellow PT!

This video is gold

thank you for making this!
Could you please release the video on tibia work and the foot you mentioned at the end🙏🙏
Please make a video on meniscus tear
If you can explain the femur internal rotation how it may effect the ankle or foot that would be more helpful.

My knees face inward and I have bow legs. What could be the cause of this and how should I correct it??
I would like to get an online coaching? I live in Sacramento California..
This saved me because I thought my knee rotation was because of my ankle
Same pfp lol
It could be related if u have pronation within ur feet I have pronated feet thus my knees rotated inward then my hips
Wow. Same here. Looking for someone to show this online.
You are not wrong, 90% of the time is because of the ankle
Ahh okay. So the first example fits my pathology in the leg itself structurally.

Can u give us a excercise to help this
Finally a video that explains my problem
I have IT band syndrome on my left leg too , is it related to that? Internal rotation I mean?
Most likely yes, but obviously hard to say without assessing you.
@@conorharris thanks
Bow legged?? A lot of knee pain. What exercises?? Thx

A physiotherapist should help you, both describe what you have and teach exercises to fix it
Do you have an approach for varus knees, please? I have knee pain and I'm bow-legged.
Im having a hard time tracking all the new words (English 2nd language)
I have the first condition demonstrated in the video, what exercises should i be doing?
I would try some of the drills mentioned for improving external rotation/posterior orientation of the hips
Hey there
Amazing information. What are you thoughts when this happens in the right instead of the left side ?
Dear sir... I'm 30 years old and my right keen hurts ... Looks like I've hip internal rotation.. my legs look like bow legs because of this... I'm really scared of having some serious knee issues ... Please help me... What should I do?

The side I am trying to target for IR would be the back leg in the staggered stance RDL.
@@conorharris What exercises do you recommend we do?
My left leg facing this issue has excellent external rotation but lacks internal rotation a lot; why is it still happening to me then? I am confused.
As some with patella tracking issues/patellofemoral pain.. I have ER tibias, lack of hip ir/pronation, small athlete wide pelvis.. what do you think I need to work on?

But how do I know which degree of rotation is normal and if my assessment of the degree of the rotation is correct?
No demonstration?
what to do for internal hip rotation, I have very bad internal hip rotation

I fracture my left femur and had a rod place in it from my hip. Now I notice my knee is rotated inwards.
May I have a question on instagram or Maybe here? I am starter want to learn

Tight quad muscle and weak glute may be cause of knee pain.
