as a family medicine resident physician, my attendings always tell us “you’ll learn the business later”….I think if they taught us the business while teaching us the medicine, we would actually understand what we were doing and deliver a better service for our patients! I don’t understand why this has to be such a taboo topic in medicine. It seems like everyone wants to be so shady about it. Please continue delivering us content about these things, this is super helpful! Thank you!
Are we sure it’s not better to just keep the public and young people profession completely in the dark? I’ve always found it better when 3 people make decisions with zero accountability. That’s why our residency funding system has been perfect for so long!
You make it sound like the physician is being a bad teacher by not "teaching you the business." It's actually the opposite. He understands med students are super busy and doesn't want us to waste our time with "the business" instead of learning medicine. Unfortunately because of you're complaining, that family medcine physician might not be as nice to future med studets. You're giving me vibes of one of those insufferable med students who gets mad when they get told to leave early for night shifts, or brings up stupid garbage to admin about how we should have training for a bunch of random nonmedical stuff like finance, SDOH, etc, thereby making your classmates lives worse with idiotic assignments. If you wanna learn about "the business," do it on your own time with videos like this.
Please make that follow up video on the trend to value based care and how that will work and what factors will impact pay for different physicians and specialties. Thanks!

From your very good discussion of the topic, many might think that the reimbursement system is relatively simple and fair. Others, I among them, don't think so. Earlier this year I had occasion to inquire about a scan to be performed at one of our major medical centers, a 1000 bed teaching hospital. The insurance reimbursement to the hospital was approx. $1200. If the same study were to be performed on an uninsured pt. who would be expected to pay from his own resources, the charge would have been $16,000. When I asked my surgeon, an employee of the hospital, whether he knew what the private-pay charge was for that study, he said that he had heard it was about $64,000. So, even he was clueless. The CEO of the same hospital is paid $2.3 million/yr. exclusive of bonuses.
I'm a veteran who gets care through the VA. I don't have any CPT codes in my medical records, but of course, most of my doctors are full-time VA employees, so they're salaried. fwiw: I have personally been extremely impressed with the vast majority of medical care that I've gotten from the VA. Mental health care, not so much. That's not to say the medical care has been perfect, but it really has been quite good.
If you want good mental health care, go to a stand-alone psychiatric hospital. I recommend inpatient whether it’s a serious issue or not, as it’s usually not worth waiting for an outpatient appointment. But it can be hard to get into an inpatient facility if you’re not actively experiencing psychotic symptoms
I would be very interested in a video which describes how universal health care systems work in other countries and how being under one of these systems affects the doctors working in it.
Compared to what financial and business consultants make. Especially that what a physician is providing, considering the similarities in the concept of services. Physicians are compensated less …
I’m interested in how this produces very different incomes across different specialties in the US. How do other countries pay their physicians? It seems to me that most other places not only pay their physicians less, but that there’s less variability in physician pay across specialties.
