- Correction- intrachain disulfide bond is there in A- chain, between 6th and 11th aminoacid.
Primary structure
Number and Sequence of AAs, peptide bond
example- insulin
Secondary
Alpha helix, beta sheets, bend, turn ,loop
Bonds- H bonds
example- Carbonic anhydrase, silk fibroin
Tertiary
3D, functional, enzymes
Bonds- hydrophobic interactions, H bonds, electrostatic forces, Van der Waal forces
example- alcohol DH, LDH, MDH, G3P DH, Mb
Quaternary
Two or more subunits together
example- Hb

Yes, myoglobin exhibit tertiary structure.
Insulin has two polypeptide chains but it does not have quaternary structure. In quaternary structure, the bond involved is primarily non- covalent bond. In insulin two chains are connected by disulfide bond which is a covalent bond.
