    After my first sleep study, the technician there sat down with me to discuss the results. She started off using more common terms, but I explained that I had a rather high interest in the subject and there was no need to simplify it for me. She looked genuinely excited by that and went off into full jargon, pointing things out and explaining it all in depth. Turns out I spend 2% of my night in REM. Average is 20%.
    While meeting with a neuropsychologist, I brought up my insomnia that I've had since literal birth and he was more akin to the sleep medicine doctor in this video. Weirdest part of the interaction being that he'd make me repeat after him: The bed is only for sleep and intimacy. It wasn't enough for me to say it was for sleep. He'd always jump in, "AND intimacy!" Yes, thank you.

      @@Jakethesnake607 You're not wrong. It's too easy to lay in bed and scroll through whatever app you use. Even with a blue light filter and minimum brightness, or even a dark mode on a device, the light itself is going to keep you awake longer. Light interferes with circadian signals, so you just don't get the sign to fall asleep.
      It's mostly about conditioning the mind, though. You're training yourself to fall asleep better and more deeply when you sleep in the place your mind associates with sleeping. It works best if you can associate the whole room, but not everyone has that opportunity. I certainly don't. But you can train your mind to do a lot of different things, like keeping a dream journal if you want to better remember your dreams.
      Sleep hygiene is a really fascinating subject and there's a lot of things people can employ to get better sleep. It's not just about the firmness of your mattress and the thread count on your sheets. It's about the ritual of it all and how one goes to bed. By actually following proper advice, I went from spending an hour, sometimes up to three, to fall asleep down to twenty minutes. If I feel it's necessary, I can cheat my way into five minutes.

    Sleep medicine honestly saved my life. I have idiopathic hypersomnia among other things, and sleeping for 16 hours a day ruined any chances of productivity. My sleep medicine doctor gave me literature to take home the first time I met her. She let me discuss my treatment and have real back and forths. They really do incredible work.

      I hope you got your waking life back and are enjoying it. I struggled with hypersomnia for 6 years and it was really tough to get anyone to take it seriously. I'd sleep 12 hours/day, wake up exhausted, and drag myself through the day.

      @@janinawaz4596 Thank you, likewise. It is something that sucks the soul out of you. It isn't just you need more sleep, but the amount of sleep required is so much more. I have a comorbid parasomnia (PLMD) that also had to be diagnosed through a sleep study, and my quality of life is much better with stimulants and a sleep maintenance medication.
      My waking life is just so much more than I thought it ever could be :)
      And yeah, I will say that it has been hard due to my environment for people to take it seriously, it did take a long time, but fortunately not as long as yours. I couldn't imagine having to wait that long.

      With chronic migraines for a few yrs I would sleep 12-20 hrs a day. On the bad days I'd only wake to pee. Didn't eat, just wake, pee, maybe drink water, and sleep. Now my bladder holds a liter or more to accommodate my body not waking me to pee more frequently. I quickly drop into stage 3 sleep, but instead of staying there like it should, I quickly jump back up to stage 1 after 5-10 min, probably due to the pain or apnea. The cycle of exhaustion is real. Now I'm on a stimulant for ADHD, I had been on armodafinil for the OSA with limited success, and the new stimulant has given me somewhat normal sleep hours even if it hasn't totally fixed the problem. People don't realize how detrimental these conditions are to daily functioning. They just say, "Oh, you're a little sleepy? Just drink a coffee or energy drink and get over it!" Coffee makes me sleepy, so nah not gonna help.

      @@Matthew-cx9gj Glad you're doing better. I had systemic inflammation, asthma, hypersomnia, exhaustion, hypothyroidism, and frequent migraines all due to celiac disease. All symptoms started clearing up after I tried an elimination diet. It took me about 3 years to get my health in a good place, but every day was an improvement. So much was going wrong with my body and I was just treated like a lazy whiner because I was in my 20's and according to most health care professionals "I shouldn't feel so sick because I was young." They would laugh in my face when I said it took extreme willpower to just get through a 40 hour week. I think they may have assumed I was a party animal?
      Finally, a nurse practitioner looked at my daily diet and health symptoms journal that I'd kept for years. She was the first person who took me seriously, and gave me actionable steps, which finally led to a diagnosis.

      I do too! I tell people the scary part isn’t falling asleep driving, it’s the waking up.
      Except mine pairs itself with insomnia.
      Sleep all day awake all night. It’s all managed suburb now.

    As an RN who was a night nurse for decades, I can tell you that sleep is essential to everything else you do.
    When night nurses greet each other one of the first things we all is, "did you get some 'good' sleep?" And, the family understand, too. I remember waking up once hearing my kids playing in the other room and one said to the others "Be quiet! Mom's sleeping!" It was sweet, but back then I could literally sleep through it when the roofers were working above me.

    I had to see a sleep medicine doctor when I was in my teens. I started to have seizures at night when I was asleep. Even after they were under control I was still terrified to go to sleep, sometimes staying up for days. My sleep medicine doc legit saved my life.

    I remember getting really stressed by this when I was a med student, all these things you were supposed to be doing to optimise your sleep. It turned out the solution for me was 'Eat enough food during the day and find a career where you aren't stressed at all times'.

      I've only just recently realized that I can't sleep when I'm hungry. If I'm not dozing off in a reasonable time frame, a midnight snack does wonders.

    Sleep medicine literally changed my life. I snore really loudly growing up, we just thought thst it was just a quirk of mine. Over the years, it has gotten to a point where i have constant headquache, could fall asleep anytime during the day, feels depressed all the tkme and have severe mood swings. I thought this is just how life is 😂. i finally brought up my snoring quirk to my family doctor. I was referred to a sleep clinic and it turnd out that i have severe sleep apnea, stop breathing 80+ times per hour when i sleep 😂. My CPAP machine is now the most precious thing in my life other than my wife.

      My Grandpa back in the 60’s and 70’s could wake the neighbors with his snoring in summer with windows open and my Grandma, Bless her heart, Had to sleep in another room to maintain sanity to go about her day. In hindsight I bet anything he needed that CPAP because the startling noises and all were unbelievable. I adored him. ❤

      amen! hubby would snore and stop breathing to the point where i was legit waking up myself to shove him because he stopped... and i was scared he would never breathe again.

    I have atypical insomnia, restless legs syndrome, and mild sleep apnea. The sleep hygiene stuff never worked for me, and cpap made my insomnia worse. I couldn’t work, was depressed and gaining weight. I nearly gave up on seeking help, but eventually was referred to a really good doc who helped me figure out that in addition to a mild sleep med, I needed a med to help me stay awake. This FINALLY allowed me to keep a normal schedule and get some good sleep. Plus, the wakefulness med also helped my ADHD. Doing well today.

      If you don't mind my asking, what medications did they prescribe? I have the exact same issues. I've been on Mirapex in the past for my RLS and it worked okay, but I didn't love it. Also curious what medication would help keep you awake, I've never heard of something like that before!

      It's not uncommon for ADHD meds to make people with ADHD sleepy. Could it be that your dosage for your ADHD meds was off? Assuming you had ADHD meds of course.

    AAAAA after 25 years of basically no sleep college friends finally helped me see my sleep was severely unnatural. Spent 3 years being passed from doctor to sleep specialist to neurologist and was finally diagnosed with 3 elusive sleeping disorders, then another 2 years of hard-core sleep hygiene (torture) routines before we saw hint of improvement. Now I’m 32 and actually sleeping and RESTING from it. All that to say, I have a special place in my heart for Sleep Medicine. Rest in peace.

      I only did six months of sleep torture before I decided I’d rather just have terrible sleep habits. I’m too tired and worn out to dedicate two years to it.

      @@jesuisamusante not gonna lie, I wanted to give up so many times. I wound up slowly and painfully working out a balance: preparing for this to be my life forever, whilst also doing all the things the professionals told me in the hope it could someday be less terrible.
      Between allowing myself to be ok without being fixed, AND working through a bunch of the Shame I felt at having a body and mind that was too exhausted to be and do what I wanted, that took much of the desperation away which was a gift… with that mindset waking up exhausted was no longer a “failure” but a day to give myself grace and patience.
      Change and healing was slow- I had struggled with it for most of my life, it makes sense it would take the rest of my life to learn to take care of myself. I still have rough nights, but I can usually leave the nightmares on the pillow now. I’ve practiced gentleness A LOT which allows for peace, which is priceless on days where rest didn’t happen.

    Sleep medicine doctors are awesome! My ability to sleep was ruined after a traumatic brain injury and near-death accident. My brain being messed up, PTSD, depression, anxiety, nightmares, and seizures all caused me to be afraid to sleep, and not be able to stay asleep. 5 hours was a long time for me to be asleep. I was diagnosed with insomnia and given a sleep hygiene handbook. I also started counseling to deal with the fear of falling asleep and process some of the things that gave me nightmares. It still takes me a few "rounds" of trying to go to sleep, not sleeping, getting up, doing something boring, trying to sleep, repeat. But now I can usually get 5-7 hours of uninterrupted sleep a night!

      ❤ there's a trick to teenagers. Let them sleep as long as they want and as much as they want. Don't complain how they're in bed all day. It's fine. They'll be in college and you'll still worry about their sleep.

    As a truck driver, I can attest to the important of quality sleep too. Driving tired is equivalent to driving impaired. I just started today with about 4 or 5 hours of sleep; rough night. Fortunately I take naps during delivery. Federal law mandates that truckers are required to be in Sleeper Berth (i.e., in bed) status for 10 consecutive hours before being an on-duty shift (which includes driving or other working tasks). Doesn't leave much for sleep when you consider I got a bunch of other things to do (like eat, poop, and shower, etc.).

      I'm also a truck driver.
      I find eliminating all carbs, except above ground veggies and nuts keeps me from feeling drowsy after a meal.
      I also lost a ton of weight doing this.

      As nurse mom of former otr, I was appalled when company woke him 3-6 times when sleeping, made him sleep in a chair calling it 32 hr reset while truck repaired, didn't provide reefer for his food, didn't allow breaks as bugge him on Qualcomm. Hell,just as bad as nursing

    As someone with mad insomnia due to severe mental health issues, I appreciate that this is genuine, helpful advice and not just "well get off your phone!!!" bc genuinely i use it for white noise and i can almost never sleep without that distraction to JUST keep my mind occupied enough to not spiral into anxiety.
    ETA: For fellow insomniacs, I put on something I've seen a thousand times at a volume so I can JUST hear it. Usually, it's a twitch stream VOD, but I've also used straight up let's plays, calming tv shows like bee and puppycat, and of course good old asmr.

      Hey, you might want to try brown noise instead of white noise. It’s a little less “severe”. In fact, I use a video on here that’s a smoothed out brown noise. It’s crazy how fast I fall asleep now. If that doesn’t work, try the other color noises. One of those may be a better fit.

      That's what works for me! I either fall asleep to a podcast or audiobook I've listened to before. If I look at the time stamp right when I start it up, I can usually figure out how fast I fell asleep by how much I actually remember hearing.

    Greetings, Sleep Bro!
    I wish he’d been around for my sleep study. There I was, lying in a strange bed, filled with performance anxiety because I’d been told to go to sleep (really, dude?) and I really wanted to be cooperative. The cyber hair-extensions were uncomfortable, there was too much light, and I couldn’t have a fan. Over all that was the question “Will data collected in these conditions be useful?”
    Seeing Sleep Bro in his cap with his silly little candle would’ve been a tension reliever.

      At my sleep study, there were a lot of lights on in the room. I flipped them off and on, trying to reduce the number of lights... except one was the sleep computer system ITSELF. It's a light switch! Next to the tv/window! How was I supposed to know? I got in huge trouble because it took like a half hour for the whole system to reboot, and he yelled at me for causing the program to start late.
      The next morning, the lady was super mad at the night-guy because he 'didn't give me enough time to sleep' so my test for daytime sleepiness was kind of invalid.

    I'm a respiratory therapist. In school, when we did our sleep disorder unit, I found out about delayed sleep phase syndrome. It was me to a T. After graduation I went on night shift. Best decision of my life. I never knew what it felt like to get good sleep! I can wake up on time now too! Getting enough sleep is life-changing - ask any of my patients who wear their CPAPs faithfully. I remember one guy with COVID who got a (relatively minor) pneumothorax and we had to take the CPAP off him till it could be treated. That CPAP had been blasting dry, cold, high-concentration oxygen into his face at an obscenely high pressure to keep him oxygenated, but he was *so upset* about me taking it off him because "How am I supposed to get any rest like this?" He wanted nothing to do with the quiet, warm, humidified nasal cannula. The poor fellow died in the end. Delta was a rough time.
    (If you're afraid you need a CPAP and it'll be awful, don't worry. Home units are much quieter and comfier, plus he was on especially high settings due to his COVID.)

      Having delayed sleep phase syndrome as well, the way sleep specialists constantly try to push CPAP machines on anyone with a sleep complaint, without actually investigating all the possible options (DSPS, Non-24 Hour, Narcolepsy, whathaveyou) was so so so frustrating when I was first seeing specialists. XD

    My sleep specialist got his medical degree from the soviet union's premier medical university. He was genuinely one of, if not the best, doctor's I've ever had. Easy to get along with, knowledgeable, not super overconfident and willing to say he doesn't know how to help with certain issues.

    I took a class on sleep in college! I became incredibly concerned that my dad had sleep apnea based on what I learned, and he rolled his eyes until my mom forced him to get tested. It turns out I was totally right and might have saved his life!
    Edit: We also got extra credit for falling asleep in class.

    Having finally gotten in to see sleep medicine after a lifetime of sleep problems, I can attest that they really can figure out in under a few minutes what's wrong and just use tests to confirm and ensure insurance can't deny you treatment. For me, it was a diagnosis of narcolepsy, the genetic kind, which makes perfect sense as my symptoms are something that's run in my family for generations and been labeled all manner of things. Getting treatment didn't fix my sleep/wake cycle problem; I'm still more nocturnal than diurnal. However, it has made it possible to generally function better, get a full night of sleep AND actually stay awake through the day...😒most of the time. And at least I'm not having cataplexy attacks anymore! 😁👍 So there's definitely that!

    As someone with sleep apnea I have to say the tubes and cords hooked up to monitor me during my sleep study sucked but that was one of the nicest rooms I’ve ever been in and the bed was as comfortable as they get.

      Mine was like a low-end hotel room. The bed was too hard, the pillows were marginal, the decor was cheap. No problem, I could still sleep there, it was clean and didn't smell bad.
      But when I was in the same hospital for afib a few months later... wow. The bed was about as comfy as a hospital bed gets, automatic air support adjustments, decent pillows, and the room was high end hotel all the way, complete with electric windowshade I could control from the bed, beautiful tile work in the bathroom, room service ordering on the TV, nice light fixtures, huge room, nice guest recliner and guest sofa/bed.

    ah yes, sleep medicine, the most ignored of the health sciences (I'm mostly joking)
    Also, there's so many other sleep disorders besides sleep apnea and those diagnoses are usually delayed from onset by years and years (often even after seeing a doctor sometimes even after seeing a specialist)
    Sleep apnea is not always the only thing at play and I wish more people knew that!

      Yeah I went in for a sleep study looking for something unusual and the follow up call I got was "you don't have obstructive sleep apnea. Bye" and like. Yeah. I know. That wasn't in the list of things we were looking for because we know I don't have it. We were looking for anything else.
      ...
      Also their only appointment was midnight to 5, which are some of my most productive awake hours, so I couldn't actually sleep. And even if I could have, that's a maximum of 5 hours. Seriously?

      Next specialist I went to was, like, why did she give you a CPAP? It was obviously not your issue.

      Yes our local sleep specialists are pulmonologists. They don't see you ever, do a phone interview and you fill out a tick list. That list is about sleep apnea and snoring only. I can't sleep due to head injury and strokes, but neuro says "that's sleep mecicine". Sleep medicine, hmm u don't have sleep apnea. I know. So I gave up.

    Would love to see more sleep medicine videos, especially to draw attention to both obstructive sleep apnea and the under-studied field of sleep medicine at large.
    Shoutout to my sleep neurologist. Between my grandma and mom, I've met quite a few really good MDs, DOs, PCPs, RNs, NPs, OTs, etc. My sleep doc is the hands-down best healthcare provider I've seen. (And he's a DO, take that people who are jerks about DOs.)

    My neurologist wanted to do a test to check something in my brain. I was taken to a room like the one these doctors were about to enter.
    I got on a bed that felt like a cloud. The electrodes were attached to my head and I was asked to remain still.
    The lights were turned out and I was told that I could even fall asleep. Was it a coincidence that the test lasted 20 minutes? The urge to fall asleep was overwhelming but I couldn't stop enjoying the feeling of that bed! Floating on a cloud is how it felt with no pressure anywhere! I could have slept for hours until I tried to turn my head. 😮
    When the test was over, they quietly returned to the room, as if to not awake me. Only subtle light was used to remove the electrodes as they spoke softly.
    They informed me the test was concluded and I was free to go. I felt tears forming and they said I could take a minute to regain my bearings. I truly understood the meaning of sleep hygiene for the first time in my life.
    Even though this wasn't a sleep test, this was clearly the surroundings for one. Although they looked at me funny when I was leaving and asked if I could return every night at 10:00.
    I've since gotten blackout curtains and invested in a new bed. I sleep much better now and haven't had to return to the sleep room, one of the most enjoyable places I've ever been.💤
    If I ever have a restless night, I close my eyes and imagine I'm back in that room. Or on a beach while the sun is setting and the waves are rolling in. Because when you dream of being on a beach, there's no sand when you wake up! 👍

    Sleep medicine saved my life, that's no lie. Literally my first night in the sleep clinic, and they hooked me to the machine. It was the best sleep I'd had since I was a teenager sleeping through a growth spurt. I still wake up like that, twelve years on. Ready to roll, at least as far as the coffee pot.

    I have severe obstructive sleep apnea with residual hypersomnia. I use a bipap. It took me YEARS to get diagnosed. Both of my sleep studies showed that I get zero REM sleep without the bipap. I love my current sleep specialist!! He's awesome! We are working on my treatment plan ; I'm finally getting the care I need.

    In my high school, we had a presentation from some of Dr. Dement's Stanford students about sleep hygiene, sleep schedules, and the importance of sleep. This was extra funny because we got to hear all about how teenagers have later sleep schedules than adults, but the presentation was held during first period at 7:50am.
    Anyway, Dr. Dement was very cool, he pioneered a lot of sleep apnea research and he apparently let students sleep in class, because if they were sleeping during class, they must REALLY need it and he was literally teaching them about all the harms that can be done by lack of sleep.

    Thanks Dr. G for showcasing a need that has hit a nerve with many of your loyal followers! The discussions are spirited with what helps one sleep deprived fan over another!
    2 points: I participated in a double blind research study at Northwestern that was attempting to determine if medical grade marijuana helped with sleep apnea. I had to stay overnight every 2-3 weeks for a sleep study and then I'd get my nightly capsules for the next few weeks to take about 30 minutes before bed time. I also had to fill out a daily questionnaire of how I felt I slept, yadda-yadda-yadda! After it was completed we were supposed to find out If we got the real pot or the placebo. The study continued on for around 8 months. That was several years ago and I still don't know what I was given!
    2) I find that if I am having a hard time falling asleep if I take 3-5 DEEP breaths (expanding my lungs fully) inhaling through my nose and exhaling through my mouth it helps my brain to shut down enough to fall asleep! I try to picture positive energy with every inhale and expelling negative energy with every exhale.

    “Jack be nimble, Jack be quick, Jack jump over the candlestick”…love it! I went as Jack be nimble at Halloween in 1965! Grade 2, this is a tender memory for me. Plus the 20 minutes to fall asleep or get up & do some low light activity is something I should try.
    I like the residents who sneak in to sleep😂

    Sleep medicine was honestly a life saver for me. I could barely sleep through the night, was falling asleep at work to the point I was put on medical leave. Sleep medicine diagnosed me with severe sleep apnea and prescribed me CPAP and I'm sleeping and feeling so much better. Thanks for saving my career and giving me more energy in the day.

    I did a sleep study on a journey of sleep apnea discovery (dumb brain, not fat neck). I needed to take a urine a couple times. It was remarkable how when I said, hey-o, to the dark, they immediately responded. I mean, my guy was watching me the whole time, presumably. I was impressed. The Eye in the Sky.

    Sleep medicine saved my life. They let me wait 3 months for an initial consultation, during which a nurse reads my questionnaires response and some Wikipedia of sleep apnea. I then waited 3 months to just get the home sleep study test kit and another month for the result, telling me I can't get a CPAP due to insurance issues. Great experience.

    I met a person once who actually only slept for 2 hours per 24 hrs. I asked if they had something wrong and they said no, they just hit deep sleep really quickly and get the maximized amount of sleep that they need.
    They seemed to function really well, better than i do in fact, but i have always wondered what was going on there.

    My dad did a sleep study, and it was unfortunately entirely useless because the doctors said “you don’t have sleep apnea” (which he knew) and didn’t even think it was a problem worth investigating that he wasn’t sleeping for prolonged periods (which is why he got the sleep study done).
    It seems like in our area (Oregon) sleep study facilities just want to diagnose sleep apnea and sell sleep apnea aid devices rather than solve any other kind of sleep-related issues. I wonder if that’s the case in other regions??

      It absolutely is. I’m in Utah and I had the same thing happen with the first sleep specialist I went to. She also suggested I eat more probiotics and exercise more (which is decidedly not a treatment for insomnia).
      I ended up going to a specialist at the local university’s sleep clinic and she was able to actually diagnose me. I figured going somewhere that they actually do sleep research would have a better chance at identifying my non-apnea issue, and I was right.

Despite having a neurological and inflexible circadian disorder - all they want to talk about is sleep hygeine and chronotherapy, which nearly killed me multiple times (gotta drive to work - chronotherapy + driving to work means falling asleep at the wheel constantly) and almost cost me job due to on-the-job exhaustion, all in some vain effort to get people with sleep disorders onto a 'normal' rhythm. Well that and they try to hustle CPAP machines on absolutely anyone with a complaint.
    The only thing that improved my health was saying 'fuck it' and taking on the graveyard shift.
    Now I feel better than I have in years.

    I had severe insomnia from what turned out to be my crippling ADHD, and a sleep doctor yelled at me for not falling asleep in the sleep studies. Just gave me a cpap, used it 100% for a year, made the insomnia worse, i asked if he could do a test dose of ambien just to see if itd help.
    He said "oh i dont prescribe those kinds of medicine." Bitch sleep is your entire thing. Got my GP to do it, learned i could stay awake with two ambien, stoned off my ass. Thats how i discovered my ADHD was crippling me with insomnia so severe i slept 5 hours a week. By wasting over a year with a bad sleep doctor.

    I've spent most of my OT career trying to reconcile how I feel about Grandpa Joe's bed transfers and single point stick use. I'm ashamed to admit I'd never considered his sleep hygiene.

    I have non-24 sleep-wake disorder. It actually quite debilitating. And this sort of doc is the ONLY sort of doc who seems to know it exists.

    Bless you, sleep medicine, you saved my life ❤ My sleep doctor retired and now I'm seeing a neurologist instead for my narcolepsy and he doesn't really...get it.
