    Dr. Liz, - "catching a patient at a teachable moment" is a big key. Mainstream medical knowledge still puts the keto diet in the "weird" category. It's as if patients need to be at a desperate moment where they are willing to try anything before they will give it a serious try. Please, please continue. This is so important! Thank you.

    Many years ago I met this gentlemen on a flight. This was way before I had knowledge about T2D, carbs, injected insulin, etc. He informed me two of his toes were already amputated due to T2D, which made me very sad. He then asked the flight attendant a can of Coca Cola! He showed me how he adjusts the dosage on his insulin pump to counteract the (toxic) drink. So sad to think how many patients have gotten and continue to get bad medical advice.

      I have a very large friend (skeletal structure) who determines what bad things he wants to eat, tests his glucose, and adjusts his insulin dosage to minimize the sugar spike. He then loads his plate with potatoes and pasta, making sure that he has a good amount of salad to, again, offset the carbohydrate intake. Still, he consumes nearly twice the amount of total carbohydrates per meal than he should, having been convinced, as a child of above average height and weight, that he needs to eat much more than a smaller person. He lost his job as a truck driver due to neuropathy in his feet, and now sits around much of the day, getting even less exercise than he did as a truck driver. He is now nearly deaf in both ears, has poor eyesight, and needed to be catheterized to urinate.
      I am a type 2 diabetic who is approximately 20 pounds overweight, but I love to walk, and drink copious amounts of water to flush the sugars from my system when I cheat, which is too often. My daily glucose readings are typically 140 mg/dl 2 hours after consuming it typical low carb meal. My A1-C has been as low as 6.2. In the middle of the winter I eat more and exercise less, and it was 8.4 a month ago, but is on the way down again as I prepare to hike the entire Appalachian Trail (2,200 miles) in 6 months this year. I have hiked it in its entirety over the past 40 years. I hiked 275 miles in 2021, and 270 miles last year. I will be turning 70 this July, midway through the hike.
      Exercise and hydration are probably underrated.
      I take no medications for diabetes.

    I'm another T2D low carb success story. Before discovering the keto diet, under the care of an endocrinologist, I was on up to 120 units of Lantus a day and couldn't get my a1c below 7. When I switched to keto, I lost 65 pounds back to my 20s weight (I'm 76), was able to drop all my insulin, and my last a1c was 5.8. I asked my endo why he didn't recommend this diet to all his patients, and his response was that he didn't think patients could stay on it.
    More recently I had a similar experience with my thyroid. I had been on medication for hypothyroidism for over 20 years. After watching a video by Dr. Brownstein on iodine deficiency, I asked my doctor for an iodine test. In over 20 years no doctor had ever tested that - naturally it was low, so I started supplementing iodine. Now I'm off thyroid medication as well, and feel better.

      Good for you, man. It's difficult not to get angry with the healthcare system that has been completely in the sway of big pharma, wrecking health and shortening lives to make a profit.

    I'm continuously baffled at people risking poor health or death because, "I love my food too much". That's not a nutrition issue, that is addiction, but the situation is so rampant and widespread it has become a golden calf that it's not cool to talk about. I would venture to say that even those doctors that poo-poo the notion of low carb are probably addicted to eating garbage themselves.

      So very true! My spouse and I were headed to horrible health ourselves before we turned things around six years ago. Despite the dramatic turn-around witnessed by family and friends, "comfort food" abounds. My oldest brother dead now of T2D/kidney failure, another brother suffered a stroke, and the third has had a kidney transplant. I look at the people working in the doctor's offices and hospitals and at least half are morbidly obese. One was even the "dietician" for the hospital. Holy moly--this is insanity!

      @@tomg1247 It's heartbreaking. My next door neighbor is a nurse and looks to be about 300 lbs. She lives on fast food. I also made a big turnaround starting in 2015. People say I look great, much younger, etc. They ask what I am doing, then tell me they don't have the "willpower". People can not get through the first few weeks. I tell them the hunger and cravings go away but they just can't seem to do it.

      My health insurance provider is now offering Virta at no additional cost for members with type 2 diabetes. My wife and I have no need of it, but it's good to see this much sanity entering the mainstream.

    Very important to know that Hba1c does not measure fructose. So results although look great be mindful of fructose intake.

      As fructose is approximately 10 times more glycating than even glucose steer clear of it at all costs.

    It would be nice to bring on board and have a talk about carbohydrate addiction and, more importantly, how to deal with it. Many metabolically illed people is addicted to carbs and it is SO difficult to overcome it.

      Good luck in California. There are a couple in the larger cities. There is a LC and keto doc list I believe through diet doctor. Even a bariatric doctor I recently saw never used the words keto or low carb but sort of quietly said “you can lose faster eating protein and veggies!”

      Do it in your own. Do not need a Dr. Plenty of information out there . Drs do not want to fix up. They want their money coming in.
      It will take one month to six depending on how aggressive you do it. Or how fast you want to get well

      Dr Ken Berry has some great videos and there is one about which blood tests to get. I'm in Australia and changed my diet without doctor's help. Sten Ekberg has some great videos explaining insulin resistance which helped me to understand why carbs are bad. He also has a video or two about how to find a low carb doctor. Just search his name and low carb doctor. The links are in the video description. Dr Eric Westman's Page 4 (Adapt Your Life)is a good starting point for what foods to eat. Beat Diabetes! also has some good videos.
      My story: It only took me about 3 months to go from A1c of 7.0 down to 5.2 without drugs. My moderate fatty liver is now mild and my ALT liver function test has gone from 45 down to 13. I've lost 20% of my weight without even trying (it was an added bonus, my goal was to remit the diabetes). My waist went from 44" to 38". After the first couple of weeks I stopped craving carbs and I was eating as much of the permitted foods as I wanted so I wasn't hungry. As time when on I became fat adapted, which means that when there is glucose in my blood the body uses that first, then it will use my own fat stores to make the energy it needs. Like driving a hybrid car. I can now defer a meal for several hours whereas before low carb when I started getting hungry I would have to eat within the half hour.

      You don't need a personal low carb doctor. Just get the book "End Your Carb Confusion" by Dr. Eric Westman. I've lost 37 pounds so far and have dropped 6 of 6 meds. No more joint pain either. I'm 76 YO. I also walk 4 miles per day.

    Hopefully doctors will start being more inquisitive to learn about low carb when they see their patients taking control of their health by choosing diet instead of drugs. That's what I did and when I returned for my blood test results after 3 months of low carb I said to the doctor that I hope in the future you suggest diet before drugs. She had wanted me to start a statin, metformin and see a dietician (which I did after starting low carb and was told I wasn't eating enough carbs). I told the GP that you don't make more than one change at a time. I also contacted the dietician with the result of my blood test results. I had a CAC = 0 and because my LDL had gone up after low carb (but all other results were good; I'm not concerned about the LDL) the cardiologist wanted me to start low dose statin. I said no thank you and told him about LDL hyper responders and gave him the link to a Dr Paul Mason video which he said he would watch.

    What's the exact difference between remission and reversal of Type 2 diabetes. Can I eat rice without any medication after I have shed all my fat, built some muscle and followed keto for several years?

      The definitions of remission and reversal vary from one organization to another. HbA1c less than 5.7 and no drugs for blood sugar means you no longer meet the criteria for the diagnosis of pre-diabetes. Can you maintain this and even add some carbs? That's entirely individual. Many people experiment with it until they find a combination that works for them. Others discover that it is simpler for them just to leave starchy carbs, like rice, off the plate.
      A simple starting point is the one-page meal guide from Dr David Unwin or the one-page guide from Dr Eric Westman (both available online). After achieving the main goal (normalized blood sugar without medication, for example), a person could experiment with small amounts of carbs they've eliminated. Remember, the carb police are not the online pundits. The carb police are your A1c and your fasting insulin. They'll blow the whistle if your carb intake exceeds your body's personal carb tolerance.

    It’s amazing how simple it is to lower your blood sugar and insulin requirements by understanding what turns into sugar in the body and how insulin is driven in part by blood sugar. Once you understand what foods increase your insulin requirements, the rest is a cake walk… lol

    My brother has an IQ score of 186. He is a type II diabetic, now on insulin and dialysis. This summer I saw him eat eggs (yay)...along with a bagel, then a piece of lemon creme pie...because his blood sugar was low. Sigh. People can be misled so easily.

      It damn sure won't hurt, and from what I understand it will reduce inflammation and help your body catch up and regenerate. Like the right time to buy real estate being "20 years ago," the right time to start keto is ASAP.

      Low carb won't reduce your CAC, but it will reduce the burden on your arteries which make these damages, inflammed and not healing.
      BTW, I would more worry about the soft-plaque than the calcified one...
