URGENT ADVICE: Bucket Handle Meniscus Tears

  • In this video you'll learn the 3 reasons why bucket handle meniscus tears require urgent attention - the sooner the better.
Dr. Erin Boynton has been an orthopaedic surgeon for 30 years, worked for MLB's Toronto Blue Jays for 10 years as their team surgeon and is the current Medical Director of the Rogers Cup WTA Tennis Tournament so she has expertise with athletes and people who love and need to move!
Whether you've been diagnosed with one or not, if you injured your knee and have trouble fully straightening it out or it catches or locks when you walk, you need to take care of it ASAP.
If you have a bucket handle meniscus tear and you do get surgery, then you'll want to get started on your rehab ASAP - Dr. Boynton often had meniscus patients doing specific exercises before they left the hospital.
The specific exercises and a program progressing you through the critical phases of recovery are all laid it in our program here:
    www.precisionmovement.coach/y...
It will create the foundation for movement you need to get back to your sport as quickly as possible, pain-free.
If you have a torn meniscus but not a bucket handle tear, you might be able to avoid surgery altogether with this program as Dr. B was able to divert 70% of her meniscus consults from surgery with the exercises and plan laid out.

I had bucket handle tear 6 months ago and just got surgery. I unknowingly played football on it for 5 months, after it was wrongly diagnosed as a strained muscle. In surgery the doctors said all the cartilage around it was very torn up from me playing on it. But I’m on the road to recovery and it’s going well. Thanks for the video.

Great video! Anyone concerned please listen to their guidance! I had a bucket tear that would intermittently lock my knee, but erroneously I delayed treatment for a month. I had a long road after surgery due to the amount of inflammation, atrophied muscles, and adhesions from surgery. Thankfully I found a great chiropractor that targeted my issues and I have been fully functional for years 😊 your education style reminds me of him, thanks for what you do on this channel!

I had a bucket handle meniscus repair and I’m so pleased with the results. Thanks for sharing this important message. Is it possible for most people to get to surgery in less than 2 weeks in Ontario? I managed to but I tore my ACL a few months before and happened to experience my bucket handle tear the day before my MRI and a few days before my consultation with my surgeon.

Hi @precision Movement and Coach E, thank you first of all for such a great video!
I just received my MRI results with a bucket handle tear of medial meniscus. The injury happened 3 weeks ago and I don't think I can get the surgery for at least the next 4 weeks if I follow the NHS process in the UK.... Is it possible to still get the meniscus sewed or would the surgeon shave the tear because it would be at least 7-8 weeks after the incident?

I'm presently experiencing limited knee flexion movement at either end particularly in my heel reaching my bum. Is this definitely a bucket handle tear? My doctor didn't bother to check, just said it was an external knee injury and that I had arthritis and to expect stuff like this but not to stop exercising. I'm a bit worried about doing damage. My daughter fell and hurt her leg in June of this year. She went to the ER two days in a row. She told them she couldn’t straighten her leg out. They finally sent her to a orthopedic dr but drs appointments were scheduled far apart. She finally got to get a MRI appointment but they said she was having muscle spasm’s in her knee and that she would have to come back. Three weeks later she took a muscle relaxer and had the MRI done and found out she had a bad meniscus bucket handle tear. So now three and a half months later, she is scheduled for surgery this coming Thursday. I hope it is not too late to repair it. I wish the doctors would have listened to her when she told them something was wrong.

    THANK YOU.
THESE QUESTIONS GO OUT TO ANYONE WHO CAN ANSWER THESE. AND I HOPE EVERYONE HAS A SPEEDY AND GOOD RECOVERY.
10 weeks after your meniscectomy were you able to bend your leg all the way back as far as your good leg bends back? Or are you only able to bend it about 90°? And on bending it about 90° did you feel pressure in the knee? I can bend it about 20° past 90°, but that's putting some force into it like with PT exercises Did you still have some swelling of the knee? Were you easily able to walk up and down steps or did you still have a limited range that made it difficult?
I am in my mid 60s and had a partial medial bucket handle meniscectomy. My surgeon showed me the pics. There's no wear or damage to my cartilage. He showed how smooth and undamaged it is. And I was walking around with the bucket handle meniscus tear for 5 months until the surgery! And my job is a physical one. I'm just very lucky.
This is my situation. I'm trying to get an idea of all of this through searching online different resources.
    I'm going through PT now.

Hey! I had a meniscus tear in the year 2022 December while skiing. I couldn’t move it basically at all, but decided not to go to a doctor. It got better. Then i had a reoccurring injury in march while playing volleyball and another one in April while jumping. After the second injury we went to the doctor and they told us to go to the MRI. They told me i had a lateral bucket handle meniscus tear. After the second injury though, my knee got better within a month. I can walk, ride the bike and swim, but still too scared to try running. I also have full mobility of my knee. When we went back to the doctor after the MRI (yesterday), they told me that i need surgery. I’m not personally sure i want the surgery, but at the same time i don’t want to risk anything worse happening to my knee.

Hello Doctor, its been a month since my injury, originally it was determined that dislocated my patella but after my MRI I was told I have a lateral bucket handle tear. I am able to straighten my knee but not full extend it. My knee is still swollen above and around the patella. Is it critical for me to get the surgery?

    • @PrecisionMovementCoach
      @PrecisionMovementCoach  2 ปีที่แล้ว

Thanks for reaching out to us!
Can you please ask your doctor to clarify the diagnosis?
If you did dislocate your patella there would be signs on the MRI.
If the injury caused the bucket handle tear then the prognosis depends upon your age and symptoms.
If you are in pain and have swelling then it is likely you need the meniscus fragment excised.
If you are older and have no symptoms then you could observe the tear (i.e. don't seek repair) but there is a risk of damaging the cartilage in the knee.
If you are young then there is a possibility that the meniscus can be repaired.
I would go and discuss these issues with your surgeon. You definitely need a surgical consultation.
Let us know if you have any more questions :)

Or do I see how I go after seeing the orthopaedic tomorrow as of writing this message. I can’t fully extend my leg. But I can put a little bit of weight on my leg. Thank you for making this video. Very helpful. I'm 45 years old man. The MRI report of my left knee mentioned large medial bucket handle tear. I twisted my left leg while playing cricket 7 weeks back on 12 June. After 4 weeks of physiotherapy I can straighten my injured knee almost same as other but I feel blocked/locked at the last stage of straightening the leg. I can walk well with balance for 20 mins but with intermittent pain in the knee, especially medial line of meniscus. I didn't try running. I'm careful not to bend my knee or twist. The physiotherapist says my knee movement is in functional range to walk, run, do my regular activities without surgery by strengthening exercises and diet plan to reduce weight(h 167cm / w 74kg) and healing internal injury due to tear in 3 to 6 months.
I consulted 2 orthopaedic doctors and they clearly advised to go for miniscus repair at the earliest.
I'm confused as I believe conservative approach can help here based on progress. However docs are very clear about meniscus repair surgery. Can you help me with your comments to make decision?

@user-dk4hw9of1k I met 6 surgeons and 3 physiotherapists in my area. Except for one physiotherapist all advised to get meniscus repaired.. however they also mentioned in case meniscus is not repairable, they will remove patialy. I had to make a choice of having restricted active life or go through surgery and rehab of approx 6 to 8 months and get back to an active lifestyle where I can run to the least.
I had my surgery in mid September this year. It's been close to 3 months now with physiotherapy advised by the surgeon. From last one month i have started walking with a intermittent pain but it's improving week on week. It's a slow recovery. I'm allowed to drive my car but no motorcycle. No squats or any move that will twist my knee.
Following information & questions helped me decide if I should go for surgery,
      1. Is the pain consistent over the next few months after bucket handle tear?
      2. If I continue without surgery and emphasize on PT, the flipped meniscus is not going to get repaired by itself.. rather it will loose it's biology over the years, making it not suitable for repair in case I had to in future.
      3. Over the years there is a possibility that torn pieces might shift and get stuck in the middle of the knee further limiting my knee range of motion. In such cases surgery is the only resort and in such cases menisectomy is the only option, mostly.
      4. How many years of active life do you imagine you have remaining? Do I want my active life back? even if it takes close to a year to recover.
      5. Is the doctor the best in the business? Do I have full confidence in him/her? It's very important since a lot of decisions are made on the table when they see the situation inside your knee. So an experienced and reliable doc is so very important.
      6. You need good support for initial 4 to 6 weeks at home since you are on crutches or walker or walking stick.
I hope this helps. Good luck 🍀

I have a undersurface horizontal tear of the posterior horn and body medial meniscus with flipped flap into the inferomedial joint recess.
It's not a bucket handle tear, but is it something I should consider having surgery for since there is a flap - but the flap is in the outer joint space? Right now I don't have much pain and can almost straighten my leg all the way which has been progressing with exercise. It's been 4 weeks since the injury.

Hi Derek. That's the hope I had before my partial medial meniscectomy when I did some research about 14 weeks ago. My surgeons advice was initially correct.
It'll have to be removed and especially if you've been walking around like that for months the bucket handle meniscus that tore off is probably all damaged from flopping around and possibly getting compressed.
It sucks no matter what but if you're young it's even worse because you have a long ways to go. Eventually you can have arthritis of that knee. When you're much older you might not get arthritis very badly.
But there are other various factors that figure into this whole situation. Your age, your weight (overweight) etc.
Just make sure you undergo extensive physical therapy after surgery. It should help a lot.

Hi. I'm 38. Had my ACL reconstruction surgery 3 years ago and had a meniscus repair at that time. Been playing intense badminton and my knee locked up 6 months back, but it was all right in 3 days and I started playing again after a break of 3 months. One month later my knee locked again and cleared up in 3 days. Now I don't have any pain and have started going to a gym to strengthen the muscles in my leg and I've been working out intensely. But I am scared that once I start playing badminton my knee will lock up again. The doctor was not sure if my meniscus could be repaired this time around. I'm having nightmares when I think about meniscectomy, getting my meniscus trimmed as it leads to OsteoArthritis. Abstaining from playing badminton is already stressful. Just wondering whether I could continue to engage in sports till there's pain in my knee or it gets locked up or am I causing more stress to the ligament even though my bucket handle tear doesn't hurt as of now.

Hello. I injured my knee when I was about 15 years old in track. I went to stand up really quick from the ground and put outward pressure on my knee and it felt like it slipped out and right back in as I walked. It was sore for a few days. It ended up slipping again a few more times within the next year or so. I was always able to get my knee unlocked after a few minutes. The last time I had pain in the outer part of my right knee was in 2016. It didn’t lock but I felt slight pain when my knee was bent while trying to tie my shoe with my foot on the arm of a couch. That’s the last time I felt actual pain in my knee. I’m cautious with it to this day but it’s been over five years. Is it a bucket handle tear still? It is a flap tear? Did it heal? Just looking for some advice. Thank you.

I felt it again some 10 years after when I started to get into running and weight lifting. I ran 11 miles and saw an ortho and he told me maybe small meniscus tear, probably not worth the MRI. 5 years after I'm now 31 and my knee pain is getting bad again but without me doing all the running I did before. I got an MRI and was told I have a buckethandle meniscus tear. Thr doctor says it's impossible this is from when I was young or I'd have no meniscus left. I gotta follow up with a surgeon to see what's next for me but I'm so scared of the outcome.

@Mohammad Abubakar that's correct, removal of any menis us part will cause degeneration. I've had both knees tear, in tbe right a very small one but after trim I ran for 4 years on it and its ok but now has new tear. My left one was destroyed 3 months after 60% removal. It should be a law that tge surgeon must inform you if he has the complex skill to sew back the meniscus. I heard from a good surgeon that many of his colleagues cannot do it, its time consuming and not worth the money. Knee replacement is good for them so they decide to specialise in that

@@PrecisionMovementCoach hello I’m somewhat in that same situation as far as not proceeding with surgery. Hurt my knee playing basketball landing awkwardly and MRI confirmed bucket handle tear 2 years ago. Watching the videos I assume my condition is a chronic tear with the bucket handle flipping back and forth to wear it’s in place occasionally. As of now I can fully extend my knee/leg. Knee locks ups for a few seconds once a month if anything. Want to avoid surgery as much as possible since the orthopedic surgeon did mention it’s not a high chance of getting repaired and won’t fully know until they start the operation which was one of the reasons why I’m not confident pursuing surgical repair to the meniscus. To the this day I’m working hiking but not running full speed just mentally afraid to try. Any suggestions or possible workout tips to strengthen the knee??
Also have full range of motion and no pain until it casually locks up for a few seconds and once it unlocks pain subdues in about a few minutes.

      @PrecisionMovementCoach  3 ปีที่แล้ว

If you know you have a bucket handle tear even if your knee does not lock or catch and you can actively or passively fully extend your knee, get crutches and head to the doctor.
The problem of moving around with a bucket handle tear is two-fold. If it’s a displaced bucket handle tear, the displaced piece of meniscus can damage the articular cartilage.
If it can be repaired, the more you walk around on it, the meniscus can be damaged to the point where it can’t be repaired as well or not at all.
After your surgery the sooner you can start the program the better (Dr. B often started people on it in the recovery room!).
However, due to the individual differences in the type of surgery you have, follow your surgeon’s guidelines. When you’re cleared to move, start the program right away!
