As a medical doctor, I've created this informative video to help patients who are researching ways to tighten loose skin on their arms and stomach after significant weight loss. In this video, I discuss the science behind intermittent fasting and autophagy, and how these two processes can potentially improve the appearance of loose skin.

    16:56 Conclusion
First, let's discuss what intermittent fasting is. Intermittent fasting is a dietary approach that involves cycling between periods of eating and fasting. It's not so much about what you eat, but rather when you eat. There are various methods, including the 16:8 method (fasting for 16 hours, eating within an 8-hour window), the 5:2 method (consuming 500-600 calories for 2 non-consecutive days, eating normally for the other 5 days), and alternate-day fasting.
Now, let's talk about autophagy. Autophagy is a natural process in our body where cells break down and recycle their own components. This process is crucial for maintaining cellular health, as it removes damaged proteins and organelles and helps the body adapt to stress. Research shows that autophagy can be stimulated by nutrient deprivation, such as during periods of fasting.
The connection between intermittent fasting, autophagy, and tightening loose skin lies in the fact that autophagy can help improve skin elasticity. When we lose weight, our skin may not shrink as quickly as the fat beneath it, resulting in loose or sagging skin. By stimulating autophagy, we promote the breakdown and recycling of damaged proteins and other cellular components, which can lead to an improvement in skin quality.
In addition, intermittent fasting has been shown to have other benefits that may indirectly contribute to tightening loose skin. For example, it can help regulate hormones, reduce inflammation, and improve insulin sensitivity. These factors can all contribute to better overall skin health.
However, it's important to remember that the effects of intermittent fasting and autophagy on loose skin can vary from person to person. Factors such as age, genetics, and the amount of weight lost can all impact the degree to which fasting and autophagy can help tighten loose skin. It's also crucial to consult with a healthcare professional before starting any new dietary regimen.
In conclusion, intermittent fasting and autophagy offer a promising approach to potentially tightening loose skin after weight loss. By understanding the science behind these processes, you can make informed decisions about whether this approach may be right for you.
If you've lost a lot of weight then you've probably dealt with the resulting loose skin on the arms and stomach that make you wonder if you need to have surgery for it. There is some research that suggested that autophagy from intermittent fasting can help improve fibroblast function and increase collagen and skin elasticity, and as a result, reduce the amount of loose skin from weight loss.
Find out what the research and science suggests on how effective fasting (and autophagy) is for dealing with loose skin from weight loss.

Lost 75 lbs. It took two years to lose about 80% of loose skin. Fasting/autophagy, a lot of repeated 48-72 hr fasts did the trick. Even if it didn't help I'd rather look like a hanging sack of skin than drag around 75 lbs of lard again.

You'll want to stay on top of your electrolytes though as it will make it easier and won't break your fast. Sodium is the big one with potassium and magnesium to a lesser extent. The first 48 hours are brutal so if you want quicker results, go for a five day fast. On the third day and beyond you should lose your hunger and feel really good. If you're hungry or feel bad, it's because you're low on electrolytes. You can also take psyllium husk tablets to keep your gut flora happy which should also help. A couple a day should be fine.

The 24-36hr figure is quite conservative. There's information out there suggesting that autophagy peaks between 48-60 hours. If you remove the cellular stress that triggers autophagy, the signaling molecules and phosphorylation of the relevant gene pathways resolve in a short time frame. Considering serum levels, metabolic pathways and a healthy GFR, you can estimate that the hormonal and metabolic changes induced by a 24 hour fast will mostly resolve within 4 hours after breaking it, and it's unclear how long autophagy continues after that point.
Theoretically, ideal fasting time appears to be between 4-5 days, with some utility for longer fasts when used sparingly. Obviously you can't maintain these types of fasts without significantly extending the refeed window to avoid malnutrition, and as you extend fasts beyond a week, refeeding syndrome becomes a serious consideration. Also, daily intake of carefully measured electrolytes is a must, particularly in regards to K+, as too much or too little can quickly become life-threatening. The other concerns are activity and hydration, for if you push yourself too hard physically during a fast or fail to consume enough water, AKI, acidosis, rhabdomyolysis, and hyperkalemia become real concerns.
Also, be weary of long fasts when on medication. For example, if you are hypertensive and on medication to treat it, engaging in extended fasting without modifying dosages can result in a sudden, dangerous loss of blood pressure, tachycardia, arrhythmia, angina, and death.
Finally, for dry fasting, I recommend simply avoiding them as they're extremely dangerous. If you do go for dry fasts, 2-3 days is an appropriate limit, and you should probably never exceed 5 days. You should do them only when you are already in good health and well-hydrated, not taking medications, can avoid physical activity, and can remain in a cool, comfortable environment. Even then, there's a very high risk of damage to the internal organs, particularly the kidneys. If you engage in physical activity or can't stay in a cool environment, heat stroke and brain damage is very likely to result. You're really better off just never doing them.

It’s my understanding that the window for 48 to 60 hours is only for those not on a low carb diet. Being in a state of ketosis allows you to enter a state of peak autophagy much earlier. At that point, an individual in a state of ketosis peaks between 24 and 36 hours.

There's not significant autophagy up to 36 hours of fasting, especially if you're doing a normal to high carb diet up to the start of your fast. Between 36-72 hours is when autophagy gets started. Three days of low carbs before the fast and you are at least 24 hours ahead when you stop all calories. Not everyone can fast right away. If you're insulin resistant or have other chronic issues like high blood pressure, you will need to monitor your ketones, bp and blood sugar as you go. Everything should be steady and you should be feeling really good. If not, you're mot ready for fasting. If you aren't ready, start with 24 hours and add around 4 hours per attempt until every indicator looks good. In addition to starting with 24 hours when you go for extended fasting, establish an easting window (intermittent fasting) religiously, every day. These together, not just just one of them alone, will get you ready for extended fasting. It's no casual thing. Ease into it and watch your body for signs of trouble.

It seems best to do longer fasts and not just doing IF while eating so much protein, the more protein you eat the less skin breakdown there will be , because during autophagy the body breaks down skin protein of excess skin but can’t do that if your eating every day.

I have lost about 25 lb on carnivore. I go for walks every day with my dog, and get physical exercise running a business. I have never had issues with loose skin, and as my fat deposits are shrinking, my skin is just kind of shrinking along with it. The only place that isn't shrinking enough in my opinion is my cleavage skin lol! That never seems too tighten up enough to my liking. 😊.
I am 59 years old, and have rheumatoid arthritis. But on the carnivore diet most days I feel like I'm in my 20s and have very little pain (I am on medications for the rheumatoid arthritis, but the carnivore diet has helped additionally in this regard).
I have heard that slow weight loss allows the skin to shrink along with the fat. I don't know if there is a point at which the skin gets stretched so much that this is no longer the case. Judging from my cleavage, I would say that there is a point at which the skin cannot shrink back to its prior state without intervention.

I eat two to three carnivore meals a day and no snacks. Weight loss has been gradual but consistent. I have mixing in some longer stretches between meals, AKA eating one meal a day, for example. I think I've been having good autophagy doing this, as the skin sag is almost non-existent except in the cleavage area.

Think of it (autophagy) as a means to an end. Autophagy "cleans up" intracellular debris, then allowing cells to function better. In the case of fibroblasts, its thought that its the better functioning fibroblasts that are then able to increase elastin and collagen production in the skin.

That’s the plan , but I really have know idea how much to eat & how much protein to have or how much protein is to much .
My omad meal is basically broken with raw , almonds & cashew nuts , a steak & 4 eggs & usually some turkey mince or protein powder or can of tuna for the extra protein. I have coffee with butter during the day & hemp seed oil .

I've been doing it now for almost a year and have noticed no difference in my skin. Are you saying that autophagy doesn't start until 23 hours of fasting? Because I'm doing 16 hours. Hair loss happens sometimes with women. Toxins dissolve in water or your fat, and are released into the body as the fat is broken down. Women are also hardwired to hold onto fat for the sake of surviving pregnancy during a famine. I wish you the best in this... Make sure you load up on your vitamins and hair supplements as you fast, including your fasting days. Fasting flushes minerals as you eliminate daily without the constant consumption. Hair loss is linked with general shock. Sasha Snake Diet spoke about this during her fasting weight loss process. Her channel might still be up. Keep going to lose the day because it's necessary, and you need it for your health. Your hair will come back, especially if you've never had problems with it before. From what you've described it probably isn't getting worse, so you'd rather be fit instead of heavy having to recover from the same hair issues.

try apple cider vinegar combined with increased protein at meal time, and focus on increasing nutrition. Apple cider vinegar acts like stomach acid and improves protein breakdown, meanwhile nutrition will get your stomach acid back up to speed. Eventually you can stop ACV.
I did a series of prolonged fasts and ended up losing a lot of my hair too, and that is what i did to correct it (1-2 tbsp ACV in very tall glass of water, straw to back of throat to avoid contact with molars). I had bald patches, losing hundreds of strands per day. Now none of my hair comes out and its been a year.

Doctors don't want you to fast because that makes you a person that's not going to take the medication if they don't take the medication then the doctors don't get the perks from all the pharmaceutical companies and that is generally what the case is in fact I went ahead and did fasting, and I am no longer diabetes too. I am clear because of fasting you can find more on my channel.

Most doctors are there only to prescribe drugs!!! Which will in turn make your health worse. They need much more training on Fasting and healthy eating.

Thank you doctor. I went through MTF hormonal therapy which made my body to redistribute weight which left me with a slightly saggy belly. My doctor suggested a mini tummy tuck surgery but I'm still skeptical about the permanent scar it's going to leave.

Going into fasting too rapidly, especially if you have diet above lo carb level. Headaches could be salts deficiency or sugar withdrawal caffeine withdrawal.

???? Can someone help ??? Water only fasting has always been very easy & pleasant for 42 hours three or four days per week. But more recently, whenever I try to make it to 42 hrs, I now hit a problem at exactly 39 hrs. Vision darkens when I stand up. Low blood pressure?? Low blood glucose?? Low electrolytes ??? Dehydration ???

It's due to low blood pressure and nothing to worry about. just take your time standing up from seated or prone position. I've done a 21 day fast and it began about day 3-4 and continued until I refed. Hope this helps.

Recorded at too low sound volume. I just can't hear it well enough on any of my devices. It's a shame because it sounds interesting (although I had to speed up playback to 1.75 cause delivery is too slow for me). in the future, LOUDER please. I'm not super hard-of-hearing; a lot of people hear at my level or worse. Food for thought.

In addition to adjusting the playback speed (I almost ALWAYS speed people up! 😅) you can also enable the captions. Just click the little CC button near the tool icon. Hope this helps! ☺️

Your advice about talking to your doctor before beginning a diet program sounds pretty boilerplate. Please find me a doctor that knows anything about nutrition. I haven't found one and a doctor is the last person I'm going to talk to about nutrition. Sad but there it is. I'm sure I could get a prescription for a weight loss medication if I wanted one, pharmaceuticals are always available.... Sad!

Doc, how long should I fast? Answer: "OMG, you'll kill yourself----grazing is best, so eat at least 7x a day." Ouch. I really wish there was good data on fast time vs autophagy. I'm on a 3-day fast right now. At 18 hours my blood ketone level was 2.7, at 48-hours 2.8, at 62-hours 4.8. Therefore IF ketone levels are indicative of autophagy for me, at least a 3-day fast is best. What is really fascinating is that once I'm at 48-hours, I could go, literally, for a week. I suspect the trick is get blood ketones above 2.4 or so, they are the "Ozempic" of the body.

Fasting isn't new for yogis tho. They've done extended fasts for religious reasons and the positive changes to the brain chemistry. The West has a habit of dismissing everything as woo woo but we now know you can reset your gut serotonin with a 72 hr fast. And they promote raw dairy products for gut biome health.

I do exercises and it seems to help. Including tongue lifts with the mouth closed, giraffe stretches and such. If you exercise your face muscles, they bulk up, and fill the loose areas a bit.
Everyone has to make their own decisions about how they want to address aging in the face and neck. It all depends on what you're comfortable with, knowing that none of us are going to look 20 forever. If you can get comfortable with aging gracefully with health, a good grooming, you'll be a happier person overall.
For everyone else, that's not quite ready to do that yet, there are other options. Personally, I never want to look like Madonna or any of the faked faced people in the media. But again, it's a matter of personal taste.
Whatever one chooses, is up to them.
