  • While Olanzapine is an effective medication, it’s side effect profile can be nasty. Things like weight gain, changes in blood lipids, and changes in blood glucose are common. ALKS 3831 attempts to fix this problem by adding Samidorphan to olanzapine to reduce weight gain.

    I took Olanzapine for one month and whilst it made me feel great I gained almost 28lbs. Never again I felt like a bottomless pit and it didn't matter how much I ate I was always hungry.

      I like to use this mediation later in treatment. Options like risperidone, aripiprazole, or ziprasidone which are less likely to result in weight gain but are sometime thought of clinically as less effective. I think it’s more important to find a medication that helps but also does not cause significant side effect particularly early in treatment. I appreciate the comment.

      @@jatinsharma5024 sure, My quick response is dopamine blocking medications like pimozide work best and are most effective but have the most side effects. A safe place to start would be something like clonidine or guanfacine which are alpha-2 agonists and do help with tics as well as ADHD.

      @@jatinsharma5024 I like risperidone in this case, it's a more traditional dopamine blocking medication similar in some ways to first generation medications. As always talk with your doctor before making any medication changes. Hope this helps

    Ive been on abilify for a year now and i gained pretty much 100 lb in a year! My diet wasn’t the best but I wasn’t eating 3,500 calories a day. I went from 210 to 310 i just got prescribed Olanzapine and I’m so terrified of gaining more weight. The NP also prescribed me Bupropion in hopes it cancels the weight gain out. Dear God please don’t let me gain any more weight 🙏

      Glad to hear the medication is offering some value. It's one of the more effective dopamine blocking medications but you have to watch for weight gain and metabolic side effects long term.     Weight gain is my problem. Helps me sleep but during the day I feel down because the weight issue is getting to me.

    I have ARFID and was just prescribed 10mg zyprexa bc 200 seroquel wasn’t helping me. See my ED symptoms flare due to depression and anxiety. The meds helped but didn’t give me an appetite so today my dr decided to switch to zyprexa. I’ll be taking them both together while I taper off. I want to gain weight. That’s the goal. Two birds with one stone. Anxiety/depression and my appetite too. But now the side effects have me worried. I’ve read hundreds of people saying they developed diabetes taking it long term.

      It's a valid concern. I think it's important to adhere to a good diet possibly see my recent shorts on the antidepressant diet and exercise regularly. I have also used metformin to counteract the metabolic side effects of these medications, this is something you should discuss with your doctor. You have the information and the power to make an informed decisions about your treatment.     I don't know if my Zyprexa has the added ingredient which reduces my appetite but I know it raises insulin. I'm on 7.5 milligrams every night. I am actually losing weight by eating Mediterranean low carb OMAD every lunchtime and exercising after. I do lemon water fasting for the other 23 hours. Lost 3 pounds last week.

      I would say you should be very proud of all the work you put into diet and exercise. This is one of the most difficult things to get patients to start especially if they have never dieted or exercised before. Keep up the good work.     It is about time pharmaceutical companies come up with a drug that does not add to depression because of weight gain. Swapping out depression for weight gain and high triglycerides is just not worth it. Sadly general practitioners prescribe these drugs for their patients without a full discussion on the side effects that may ensue. Many patients would rule out taking some of these drugs before the weight gain if they knew. I think Mirtazapine is a good example, a great antidepressant for someone with depression and decreased appetite /anorexia but not worth the risk for someone already struggling with weight issues, in my humble opinion. And for those already struggling to keep their LDL and triglycerides down through diet and exercise a real no no.

    Thx for a great video, i live in Sweden and i dont think we have Samidorphan here, how ever i have gain alot of weight since i start to eat Zyprexa for over a year now, and i dont know how to stop it, when i try to stop take Zyprexa i cant sleep and feel realy bad overall. And have have tryed alot of other meds that dont work at all, Zyprexa is so nice feeling but bad at the same time.

      This is one of the major problems with most dopamine blocking medications. There are several ways to approach the problem, one would be exercise and diet starting a routine for both. Another option would be metformin which has good evidence for helping reduce antipsychotic induced weight gain. Another option would be to see a physician who specializes in obesity management who could possibly help with pharmacotherapy beyond metformin. Finally you could switch to a weight neutral medication although it seems like the benefits from Zyprexa are substantial.

    The worst gainer for me personally was aripiprazole, 40lbs in 6 months despite not increasing calorie intake and doing substantial exercise daily, switching antipsychotic and adding metformin was the only thing that stopped the weight gain for me. However I’ve been unable to lose weight even when 600 calories under my bmr and the same substantial exercise. Currently coming off antipsychotics all together due to not being able to tolerate the sedation from all 11 that I have tried, so hopefully my body will go back to how it was before and although I will have loose skin and bad stretch marks, hopefully I’ll be able to get at least somewhere near my original weight which was on the borderline of underweight before all of this.

      Theoretically aripiprazole is weight neutral, but I have had patients gain weight on the medication. In general, I like abilify for the reduced side effect profile but it’s not to say weight gain cannot happen.       Understandable that young or middle aged people who still want to have an active life are not able to tolerate the sedation effect.
      For those who are past 65 sedation isn't that big an issue. (as in my case ,being 72).

      Right, so medications like olanzapine will affect various receptors. In this case the weight gain associated with this medication is thought to be from histamine H1 receptor blockade which is known to causes sedation and weight gain. It’s probably more complex than this but it helps provide a framework for the mechanism causing weight gain. I will do a video on the management of dopamine blocking medication induced weight gain to clarify these concepts.

    Hello there mate , I am writing from UK , i have a question as I am starting to lose faith in all meds that I am taking or took in the past , battling Bipolar treatment resistant depression , in the past had some success with lithium plus duloxetine but not so much anymore , would you say olanzapine low dose plus fluoxetine would be better over lithium plus fluoxetine, I tend to get more depressed when on antipshychotics alone and safety profile of them, would be very grateful if you can tell me your opinion.I am stuck

    ive been talking olanzapine for more then 2 years because of my bipolar, i gained 80 pounds because i took 20mg now im only taking 2.5 because i have become much better stated to loose weight lost 15 pounds in the last 2 months and started exercising, just want to know what are the alternetives to olanzapine, is there a medication with less side effects

      My favorite low metabolic impact medications are aripirazole, brexpiprazole, cariprazine, ziprasidone lumateperone and lurasidone. They all have low risk of metabolic side effects.     Nope nope nope nope. I am not continuing to take this med......I know I have gained a couple pounds. I am stopping this med. I'll wean myself off it. 100% done. I'm not taking any extra meds with it as in meds for my meds. I refuse to get fat on top of everything else. I'd rather deal with my symptoms than be fat. No thank you. Goodbye zyprexa.

      I would still recommend discussing with your doctor so they can take you off safely and possibly discuss other options that have less weight gain. what's interesting about zyprexa and clozapine and despite the many side effects (which I agree are significant and need to be weighted out) patients stay of these medications longer than other options. I think it''s ultimately because they are very effective and people can see the improvement. Best of luck with everything

    I was taking Zyprexa for 10 months and gained over 12kg. I’m now tapering off the medication gradually with a psychiatrist. I was wondering how fast do you lose weight after stopping Zyprexa?

      I ate one meal per day, but still gained 1-2kg per month. I feel frustrated with this medication. Hoping I can loss weight after stopping Zyprexa.

      I think you will still need to work on diet and exercise to lose the weight. I will often start metformin for a glucagon like peptide medication like liraglutide at the first signs of weight gain this can help mitigate some of the weight gain. It will really depend on the value of this medication and the diagnosis it's being prescribed for . If this is being used for something like sleep or depression the side effects are probably too great for it to be worth it. If a patient is schizophrenic and has not responded to any other medication and is finally stable on zyprexa you may consider alternatives ways to mitigate the weight gain as described above. There is no simple answer. Hope this helps a little

      It has taken me a whole decade to lose the weight I gained from olanzapine, and I still have more to go. I took olanzapine for six months.

      It’s not metabolism that slows down for say, it’s changes in the way the body metabolizes things like glucose. Our bodies have a relatively consistent set point when it comes to weight. If you ever tried dieting before it can be difficult to keep the weight off for this reason, at some point I will cover the neuroscience of this and how it works. If you come off the medication lipid levels and glucose metabolism should return to prior states, it may still require dieting and exercise to return to baseline weight.     Hi: I have depression and GAD. my psychiatrist prescribed sertraline 50mg and olanzapine 5mg. I am 5 feet 9 inches and I weigh 158 pounds. Is there a possibility that I can gain weight?

    It is really for profit to the companies. It is really made to usher more people into the addiction industry. It costs 1600.00 per month and was NOT SHOWN TO REDUCE WEIGHTGAIN. The company has a real hard sell to investors, and in 5 years after a lot of dead people,they will settle.

      Lab values should improve once the medication is stopped. With weight loss you may need to work on a diet and exercise program to help get you back on track. The best way to deal with medication induced weight gain is primary prevention, healthy diet, and daily exercise can work wonders. Hope this helps

    I had my first hospitalization at 23 a month after the first real date of my life didn't turn out so well.
    I ended up in a Mental Hospital in Minnesota.
    After my symptoms were under control a nurse went up to me and said "take this if you want to get out of here sooner"
    and I (because I had done nothing wrong and there seemed to be nothing wrong with me physically) refused at first
    But being in a cramped hospital I decided to take the Abilify.
    I exited the hospital but the medication did a number on my brain.
    When my dad went to the Mayo clinic within that week of getting out of the hospital I was in their relaxation room where I had a panic attack where I had to breathe consciously because my normal breathing wiring wasn't working. And when I said "breathe consciously" I mean I had to force myself to breath because my body wouldn't do it on it's own.
    I ended up taking Abilify for about 10 years and then one day while I was on a low dosage I woke up and went for a nice walk with my mom
    I came back home and because it was my routine I took the abilify
    I instantly got a headache and had to lay down. It was then I realized that the medication was no longer good for me.
    I stopped taking the medication then and haven't had to get back on it. I had a good year without having to take any psych meds.
    I'll leave it at that for now.

    Could you mention your opinions about people saying that lybalvi doesn't make you lose that much weight and that it only works 40%? Could they be wrong?

      There is still going to be some risk for weight gain. It seems to be about a 50% decreased risk of >10% weight gain over olanazapine alone. You are right it's not perfect but it's much better than the alternative. This study looked at patients baseline and after 24 weeks of treatment.     Doc i'm really worried if I am bipolar.None of the antidepressants have worked and some made me feel a little better for a few days and then down again.Also there is a period where I was taking Agomelatin and I felt this huge agitation for a few days and then down again.Could this be bipolar?

    Hello I’ve been on seroquel for like maybe 5 years went from a healthy 210lb to 278lb is why I am here since I’ve searched for byvaldi. I am going to ask my doc to switch me to this med because this extra weight has been a problem for sometime. Heavy harder to move and tired a lot. I even train 4 days a week. I wonder if seroquel withdrawal is an issue? Also have you heard anything new about the TAAR-1 agonist drugs like Ulotaront and this one I am participating in the research Ralmitaront which is currently phase 2. Will have to get back to you

    Hi! Is it possible to avoid the weight gain with diet and exercise while on zyprexa? My son just started it a month ago and is worried about weight gain. We were thinking of the Mediterranean diet. This sounds promising

      I personally was unable to avoid weight gain even with a 1200 calorie diet and exercising 1-2 hours a day, the only thing that helped it stop was switching antipsychotic and adding metformin.

My husband is suggesting that I ask my doctor to prescribe Metformin because Olanzapine is making me crave carbs and never feel satisfied. I eat like 3 bowls of cereal at night and it’s terrible, I’ve never eaten like this before in my life.

      I love the mediterranean diet for mood disorders. It has the best evidence and can help reduce the risk of weight gain. I believe it's possible to prevent weight gain with diet and exercise. It also might be worth looking at different options is weight gain is a serious concern and potential reason to stop treatment. Ziprasidone is least likely to cause weight gain.     I am off olanzapine but i want the weight gain. Do you have any weight gain medications to increase appetite? . I don’t want to take this medication as it messes up my sleeping patterns

      I've used dronabinol in the past with geriatric patients with significant weight loss and appetite supression. Depending on the diagnosis mirtazapine has been used (it's best if the person has a diagnosis of depression otherwise it's off label). I would not use olanzapine primarily for weight gain under any circumstances, there needs to be another indication for using it. Hope this helps, appreciate the comment

    I gained around 50 lbs on olanzapine.. went from 145 ish to 180 over the course of a few years. If you have extreme resilience and discipline with eating and working out you could be fine, but I worked out3x a month and it wasn’t enough. Your appetite is insatiable on this shit! Worst feeling ever to constantly be this hungry

      Metabolic side effects are price for improved efficacy in this case. Olanzapine is one of the more effective medications but it depends on the diagnosis and reason for treatment. If someone has schizophrenia and was not stable on other medication then they try zyprexa and stabilize it might be worth trying to treat the weight gain with metformin or semaglutide.     I take olanzapine 5mg only, you think this low dosage will bother my weight?

      The risk for weight gain is still possible but less likely at lower doses. Although people have disagreed with me on this channel, if you have a good diet and exercise program in place much of the risk for weight gain can be mitigated.       @@ferdinarghoshi4614 I prefer at night but I have dosed it both ways. Sedation is common so thats why nighttime is usually better, it will help with sleep and make the risk of daytime sedation lower provided you do not take it too late

    Listen to yourself. “We’re going to talk about adding a medication to combat some of the side effects of this crap” and if it’s so great, you take it. Yes, the side effects are awful. I didn’t gain weight. That would have been easy.

    I just started olanzapine. Is there a way not to gain weight when taking it? Diets? Does it help if you count calories? My Dr. is trying to get Lybalvi. I was told less weight gain.

      Same as well. I gained over 12kg in 10 months with a low dose of Olanzapine 5mg.

      My experience is most psychiatrists do not do enough to intervene prior to substantial weight gain. Things like metformin or liraglutide should be started way before 70 pounds is gained. Diet, exercise, possible consult with a nutritionist etc needs to be done early on

      @@ShrinksInSneakers my son is now up to 150kg - baseline 90kg - (and still gaining) and its heart breaking to me. This issue is not addressed by his psychiatrists - revolving door here- locum after locum. He takes 30mg Olanzapine daily. We try to stick to healthy diet and walk regularly. His tummy is now starting to fall down over the front of his body. He seems to not care. He was once a fit athletic talented footballer. I feel saddened on a daily basis and fighting for assistance.
