In my health system, you can get a virtual visit with a specially trained registered nurse that can authorize medication for simple problems. It’s all done online. a step up from that is a telephone visit from a medical doctor/nurse practitioner/physician assistant. And a step up from that is a video visit. All three of these happen with in a few minutes to an hour.
Only once did I choose to wait till the next day (it was a holiday)
Urgent care is open every day.
Advice nurses are available 24/7/365 with physician back up for more difficult advice situations.
Of course, then there is the ER

I'm 71 years old and had severe back pain ,some chest and left arm pain also. I am not doing a commercial so I hope it doesn't sound like it, but give thanks where it's due. At 10:00 pm I go to an Adventist emergency room, not a hospital where they got me in immediately, did a scan, my back was fractured from Multiple Myeloma which I didn't know I had and I was transported to an Adventist hospital for surgery. I spent 30 days in the hospital and rehab and my treatment at both facilities could not have been better, I am grateful. I believe Doctors can tell by looking and asking a few quick questions as to how urgent your emergency is in most cases.

My 84 year old mother arrived by ambulance only to be placed in the waiting area for 7 hours they felt her vitals were not that bad but she was in severe pain and hospital was saying they don't have a bed available. While waiting they did take her for blood work and a cat scan and placed her back in waiting area but they never checked her lactid acid till much later which had reached a level of 5 very concerning. Severe Sepsis was already invading her body and her cat scan revealed toxic megacolon. Treatment with broad spectrum antibiotics where not working and was told they would have to remove all her intestines but was warned she might not make it since so much time had passed. Unfortunately she passed the very next day. It makes me very angry that she waited all that time and I will always think if she had received treatment immediately she might have had a better outcome. The healthcare system has fallen apart and things only seem to be getting worse.

I have a basic rule for visits to the ER: The longer you wait, the better, because that means you aren’t about to die. I once waited for 12 hours in an ER with a kidney stone and was glad to do it (the stone passed after 6 hours). And I agree: always treat the ER staff nicely. Always.

The scariest thing ever was when I went to the ER super short of breath when I was pregnant. There was zero wait! The doctor and nurse RAN to meet me in the room while a nurse RAN my wheelchair to them. Scared the crud out of me to see them work that fast! Once they ruled out the scariest of stuff, I learned how urgent it was for them. I was definitely grateful, but still scared me. (I’m in the US).

It is really unsettling to be the center of that kind of urgent, concentrated care in the ER. We are used to the opposite being the norm. I remember one nurse putting EKG stickers on me while another took my vitals, and a Dr asked me basic questions. I was in a CT scan within 15 minutes. Thankfully it wasn’t a blood clot like they were afraid of. But I had developed a pregnancy related heart condition that required treatment and early delivery. I had a good outcome, too. My miracle girl will be 16 in a couple months.

So many people just run to the ER for things that aren't emergencies!
We have new urgent care facilities popping up everywhere! Their goal was to keep real non-emergencies out of the ER.
Plus, Urgent Care is much cheaper than the ER in a hospital Even insurance companies acknowledge that and your copay is usually in the range of $50 but it's more like $200-$500 at the ER!
This is a great public service announcement!

My son in Oct. 2023 waited 5.5 hours for 6 stitches. Once again neither of us needed a bed or overnight stay. From friends and family I hear the same thing, even though you do not need a bed it takes 3 to 6 hours to get treated in a hospital in Ontario Canada.

Fortunately I was taken to the ER via EMS, who ran an EKG on the way and gave me an aspirin. The cardiac surgeon was there 2 minutes after I was taken into a room in the ER and then to the cath lab for a stent. I spent more time trying to ignore my symptoms than the time it took to get EMS at my door, transport me to the ER and have the stent put in place. All emergency services should be as responsive and, if properly staffed, they could be.

Six weeks ago I wasn't feeling myself, it turned out to be a reaction of my afib to my fever. Ambulance came, these people are so strong (you try to lift some 300+ pound 6'4" giant). I was completely out of it. Went straight to a room and put on antibiotics. Blood work came back, the doctors were sporadically showing up. Before I knew it, it was morning and I felt completely fine. I had been getting up by myself the later part of the night. They wanted to keep me another day, I convinced them I was doing much better. Hug for the doctor and my wife was ready to drive me home where I got the sleep I needed. I have been waiting in the hospital for 16 hours and know what stress these nurses and doctors go through daily. See for us, it is only one crappy night, for them, it is every night. 10 or 12 hours shifts, try to keep up with that. I know the government is telling us we will get more beds but who will staff these beds?

This lack of "flow" has been foreseen for DECADES... I trained as a gerontologist 35 years ago when we took lectures to learn how demographics and politics would likely combine to pose problems in not only health care but housing and other age-related issues as well. Unforeseen challenges such as COVID and inflation and climate change have only exacerbated the situation.
Changing our institutions is like trying to get a stubborn elephant to turn around while herding cats. We have a huge, but entirely FORESEEN cohort of seniors moving through now. And what is our short-sighted leadership spending time and resources doing? This brain trust is FIGHTING AMONG THEMSELVES when they're not obsessing about such pressing issues as whether some students should be allowed to call themselves by a different name.
When the ship is sinking it's standard practice that at some point the "strongest" will collude to lighten the load by heaving the weakest and most vulnerable overboard. Good luck, seniors!

I always feel so guilty attending the Emergency Dept at a local hospital when I know that it’s not a matter of life or death, but rather something that needs to be dealt with in a time-sensitive fashion. However, I cannot get an appointment to see my doctor for literally weeks. A compounding problem is that if I attend a walk-in clinic, I risk my Doctor de-rostering me. My situation is not likely life-threatening, but I could get back to work much more quickly perhaps if I could see my family physician. An observation that I share is that “Emergency” rarely translates into “in a hurry”. In attending a smaller Emergency room I can usually observe and hear the banter going on between docs and nurses. Things seem to occur at a much more casual pace than I would expect. My best guess is that the documentation/paperwork are taking an inordinately large part of their time?

when i gave birth a number of years ago way before the pandemic i was told there arent enough beds. its been a problem in some places for many years. especially in the inner cities where population is dense and more care is needed. and the demands are high.
there are a variety of reasons for wait times.
change in shift when nurses and doctors leave and next one comes create a slower pace. its just the way it is.
    not enough doctors and nurses
not enough beds! in dense neighborhoods where there arent enough hospitals
too many illegal coming for care where its free and dont have money to pay they block the emergency patients time bec everyone deserves to get care.
if there is a large amount of very sick patients coming at the same time. waiting time is longer. there are levels of care in emergency situations. a car accident crash victim that needs emergency surgery right now will come before someone that has a broken foot. and has all thier vitals and is stable. just needs a cast and x rays. a woman needing a caesarean emergency will come before stitches usually depending on the level of severity.
there are levels in emergency and that can add to waiting time.

Hey Docs its interesting that you have long waits in ED ,here in Adelaide South Australia our ED wait times are bad ,i waited 5 hours in ED with a gaul blader attack which had to be removed ,the level of care is second to none ,and its hard for the medical staff they are run of their feet.Mental health is a big problem taking up bed,s sadly most caused by Drugs ,most of our GP,s dont bulk bill anymore so people go to ED for free care,we have a shortage of GP,s and most young medical graduates are choosing specialist fields as trying to run a GP practice is financially very challenging, add to all this tight government budgets not being able to afford to employ enough Doctors nurses and general medical staff,i personally pay to have private health cover which lets me go to my own choice Doctor and private Hospital, case in point last June i had my TKR done within 2 weeks of seeing my Surgeon ,if i was on the Public waiting list it like a 2 year wait ,.Hope ive given you some interesting overview of our system 😊😊😊

The delays are indicative of a poorly functioning healthcare system in Canada. Without getting too political, it's clear that the current federal Government must assume a large portion of the blame. Indeed many pateints are showing up to the ED when in fact their ailments could easily have been handled by a PCP, but that seeminly rudimentary reason grows into a conundrum when 6 million Canadians don't have and can't find a family doctor.
To say there's a problem is a drastic understatement. Growing doctor patient loads and time restricted visits are meaningless and in fact would exacerbate the problem. The solution certainly won't be found in some agitated comments, but the fact remains the Government is dropping g the ball with Canadians healthcare. It's a heartbreaking scenario when we have good well meaning immigrants settling in Canada and then find the!selves searching endlessly for a doctor, that is spirit crushing.
Bottom line here is this... those with money get care one way or another, but if you don't have disposable income you go without getting proper care.
This system is broken in so many ways.

Also if you are at a hospital on University campas (ie Western University) ALL students are told to go to the ER not Urgent Care. Either way I appreciate all that the doctors and nurses in all medical establishment do. Keep up the great work.

I am sure you will get inundated with stories, including mine.
I was in Alabama, in the center of the city, surrounded by hospitals.
I came down with a serious flu bug around 1 A.M. . I froze in the ER for over 2 hours. I came down with walking phnumonia in the ER. THEN, I was misdiagnosed after I corrected the staff. I KNEW I had walking pneumonia, but they stated that the xrays stated otherwise.
As I got up the next few days from the hotel room, I knew I was in trouble.
    I could not breathe.
JUST as I had that thought, a call came in from the front desk.
It was the hospital. They had mis read my xrays. I DID have walking pneumonia.
I walked to the hospital right in the center of town, and was asked to SIT on an ER bed corner.
I was given medication, and when I got my bill from the ER, they wanted $2,000.00 for using the bed in the ER.
    I fought that and won.
Just thought I would share and vent.
    BTW.
    I have to admit.
I was seen by a beautiful blonde, in the hospital when I first was admitted.
    THEN,
seen by even a hotter blonde, who gave me my intravenous.
    Just sayin.

The lack of homecare is a major issue. Both of my elderly parents had strokes and both times the hospital tried to send them home with no homecare plan started. Dad had a stroke, was sent home and less than 48 hours of being home still had no homecare and was back in hospital with stroke #2. The PSW started and either showed up without calling or didn't show up at all. It was really hard to manage. Family members are not equipped in most cases to deal with medical care.

They saw me quickly, did a CT scan, and said "just a stomach virus". Discharge within a few hours. 3 days later I was still in severe pain, saw my Primary Care Doctor.... Who sent me straight to the ER! That was an 8 hour visit but I got a diagnosis that made sense, IV antibiotics and was sent home after 8 hours. But yah, the second hospital was CROWDED and my "pod' of emergency care was getting medical treatment in a public area. I got my IV in a closet. Literally. My second IV bag was put up while I was in the public area. No privacy.

I really like your guys and I recently had an experience with our adult son having had seizure and being transferred to the ER via ambulance. My experience with the ER was, they were more interested in getting a urine sample thinking he was a drug addict then treating the symptoms of a seizure. They insisted on a overnight hospital situation where I stayed with him and then actually wanted him to stay a second night. They even tried to convince me not to stay with him; I would never leave a loved one that was so drugged up by the medical system alone. In regards to the second night, we refused and left. We are regular people and having the ER treat our son like he was doing drugs versus that he had a seizure was very, confusing and scary. Before we left, I requested what the results were of the urine sample they insisted on taking via catheter because they had so many drugs in our son's system that he couldn't even stand up and the urine sample was negative. This makes me really really angry with her medical system in general.

I don’t know how old your son is or where you are from but generally, the only person that can have access to lab results is the patient themselves, so mom asking for the results isn’t gonna happen.
And, part of the differential diagnosis for seizure disorder is to rule out medication as the cause so doing the urine drug screen is very important; in fact, if not done, it’s malpractice.
It sounds like they treated the seizure if his mind was altered by the medications they gave him.

In my community the long wait is caused by understaffing and the hospital's refusal to turn away patients that are not experiencing an emergency or that may only need urgent care. I've seen patients come in for a pregnancy test, because they are lonely and want attention or have soap in their eye. These things are not even urgent care issues and can be treated at home. Hospitals need to either add additional staff and rooms to their emergency dept, turn non-emergency patients away or make them wait until the more urgent patients are seen.
