Thank you for your support as I have gone over 4 years without dialysis (and still getting better). Watch the latest videos and learn how you can slow or even halt the progression of Kidney Disease at th-cam.com/video/VHyn2IZSdQs/w-d-xo.html
Thank you both for your invaluable advice and information. Love having your book, Dr Ro, to refer to regularly. It’s clearly written, easy to understand and extremely helpful. I highly recommend it. I’m a 81 year old Australian, diabetic with heart failure, osteoporosis and CKD stage 5. I have a superb nephrologist who works for me and with me ,with my best interests first and foremost. Following his advice and yours, my eGFR has come up from 15 to 17, and I’m hoping to further improve on that number. Best wishes and sincere thanks. Meryl from Queensland.
Thank you Doctor Rosansky andJames for a very good and clear information about CKD condition. I live in Indonesia and I am 74 years old with GFR stage 3 B ( 39) MyCreatine are1.68 . Albumin 25 (+1), Now I fell no stress any more and continue with better life and happy with not so much worried. GOD Bless You both
The best lecture I’ve heard, my husband 84, his gfr is 44, he is diabetic with A1c 6.1 on Farxiga. On Losartan 25mg low side of BP , boarderline Hemoglobin, protein in his urine. His Nephrologist asked him to reduce animal proteins. Need your opinion.
This is one of the most valuable episode (I am 75 years old with CKD stage 4) I have watched. A big THANK YOU. to both dr Rosansky & James for the valuable tips and suggestions.
Our pleasure! Dr. Rosansky is full of so much helpful information. If you haven't read his book, it's a MUST read for CKD patients ( go.dadvicetv.com/book ).
Awesome! Thank you! Here is a cool fact - the Philippines was the first country I lived in outside the United States (and it is where my younger brother was born).
Thank you do much Dorothy! All of the co-host I have on are great, we try to keep things light, a little humor, and easier to understand than the normal "doctor talk".
I'm very concerned about not only dialysis but also unwanted surgery. I recently learned that my right kidney has a roughly 7 mm cyst and 2 smaller ones. The left kidney has a roughly 5 cm cyst and 2 smaller ones. It's sad I didn't find out about this problem much earlier than I did. I'm 70 and male with no history of diabetes, and about 40 pounds overweight. Standard urine tests don't show a problem,
Said I had to 20 kidney. And don't tell me this year 2023. Only tell me this year 2023. But I had it saints 2020. I don't know if I had 20. I Yes, I I have three it and yes send they sent me to Especially Specialist, tell me what can I do. The phone is not working properly. The phone is not working property not bringing out the right words. So you can understand what I'm saying. So sorry about that, but can you tell me about my tree? A 45 to 60 or something to tell me. Babe to 60. Please please Please tell me everyone, especially please.
I was diagnosed with CKD about 17 years ago. A few months ago I got an abscess that was treated with amoxicillin. This antibiotic never caused problems before, but I had to complete two courses of it, and my kidneys took an unexpected hit, dropping to an eGFR of 23. My nephrologist immediately told me if it dropped another 3 points, to TWENTY, we would install a fistula! "NO. We WON'T." I said. She was incredulous, tipped her head to one side and made a face. "Do you want to DIE?!" She asked. My answer surprised her. I am not afraid of dying but I am more afraid of how I get there. I have fibromyalgia and any time I stay in one position for too long, it can trigger major muscle spasms! I cannot imagine anything more detrimental to my sanity than being tethered to a machine for hours at a time, three or four days a week! After fourteen years of narcotics for the pain, I weaned myself off them, finding more natural ways to provide relief. I have no intention of changing my mind until there is no longer a choice!
Dr. R said "there is no reason to restrict protein" (53:00). Do you have more information about this? Everything I read and hear is low protein, including the dietitians I listen to on your channel. What am I not understanding? :( Or are they just different philosophies?
The thing about protein is mostly the type from animal tissue ... have to be careful about potassium, but my understanding is that plant protein is easier for our kidneys to filter...
Oh my goodness, I am thrilled to find this channel as I've gotten so many mixed messages about CKD across the Internet - even my very own PCP's ONLY advice was, "drink lots of water" the day he informed me I had stage 3b CKD (gfr 39.37, creatinine 1.6 - I'm 76)! He said nothing else - no nutritional advice whatsoever. I look forward to learning so much here! One quick easy question if you don't mind: isn't it smart to take one of these "Juice and Veggie" supplements you hear so much about, along with my Plant-based diet? I hope you find time to respond - at your convenience, of course. :) Thank you!
Renal dietitian Jen Hernandez has talked about the juice (or juicing diets). While they sound good - you are missing the fiber and getting the concentrated sugars, potassium, and natural phosphorus. Fiber is very important and it’s best to smoothie things vs juicing. Even better is actually eating the foods since smoothies can be packed with calories in a small serving.
Great question! Reply answered some questions/thoughts of my own. Hope your on your path to a healthy joyful life Remember our Heavenly Father has our life plan mapped out. Prayers Blessings Joy

Yes I can and it was all through diet! For me, I needed more iron so my body could make more red blood cells and hemoglobins. My first wife was being told she would need a port is she went below 20. She was 72 an insulin dependent diabetic some over weight on blood pressure meds. This went on for over a year that I know about. Suddenly she started having bad back pain and she denied it as from her CKD saying she pulled her back. In about 8 weeks she died after 4 weeks in critical care unit put on dialysis 24 7 because her urine production was too low. They did start with the arm then went to the neck port. She was sedated almost 24 7. Kidney function went to nothing. The kidneys could not start producing urine again no matter the treatments. They tried to move to every other day like an out patient. It was too late. That did not work. Within two weeks she died in December.
Godbless her soul I pray she is at peace now I know she was scared to death from the shocking reality, and Godbless you aswell for mentioning this sweet lady’s life cut short
I'm so sorry, my hubby is below 20, high blood pressure, insulin dependent, do you think she should of done anything differently, my hubby is trying to eat better, get off the meds & reverse his ckd I pray he can we don't want dialysis
Im new here & to the situation, my hubby's Dr never talked about if your kidney function was 50 & below lets work on eating right & changing your lifestyle, his Dr says his kidney function is @ 15% now, i was floored & worried but i watch one of your other videos & it gave me hope about reversing it back to stage 2 or 3 maybe higher, we did a kidney class last week out comes the papers about we need to get you setup with a port in the arm which dialysis would you prefer home or in a center, how about neither, my hubby is so nonchalant, i dont fall for these tactics, its sad money is more important than the patient, please tell me if my hubby's kidney function is this low we dont have to rush to dialysis, we are working on the blood pressure, using herbs & vitamins you give us great hope its like God lead me here 😊

I started dialysis at 20 percent but since then I have been eating better and taking care of blood pressure and blood sugar I don’t gain any extra fluids and feel great I’ve been cutting the time that I spend at the clinic to once or twice per week because I don’t feel like I need it anymore I’m 74 years old , would it be wise to stop going!
My hubby is anemic & have to get a shot every 2 weeks if it's below 10.5 ,can you share what you ate to fix your anemia please, a year or so my hubby's Dr said his potassium was high & now he have him on potassium pills & its hard as heck to get his pressure down could it be his other meds thats keeping it high, the hospital couldn't even get it down back in January we are trying healthy foods, beetroot & other options
Hi sir james. My mother was diagnosed with ckd stage5 with egfr of 5 she is 66 years old, and we do not want her to go on dialysis. Can you give us advice on what to do to improve her egfr. Thanks
Around age 30 I began feeling terrible so I went to many, many doctors who all told me nothing was wrong. I bought test strips which indicated protein in my urine. Went back to doctors and they said it was natural for weightlifters to have protein in urine. Am now 60 and seen over 50 doctors complaining about kidney pain. They all told me my tests were normal. Found a new doctor who looked at all my tests and told me the other doctors were lying to me. My kidney tests were getting progressively worse for 20 years. I now havestage 4 kidney disease. I can only imagine the system does this on purpose because they make billions off diayisis !!! Near age 60 one doctor even told me to take twelve IB Profen per day for a month and my kidney pain would go away !!!
I'm 56ys old living with stage 5 I know so dyliss is my last resort and I just started my work started drinking plenty water eating vegetables fruits fish chicken breast and some other changes my diet doing my exercises extra walking around I don't want no one to limit my life I will be in touch with getting my health back in shape
@@pisageegee2424 Your nephrologist makes exactly 0 cents from your being on dialysis. First, the vast majority of dialysis patients receive Medicare which pays your nephrologist far, far less than commercial insurance. Second, the vast majority of US based dialysis patients receive treatment from either Fresinius or DaVita, both publicly traded corporations and NOT owned by your nephrologist. All in all, kidney failure means your nephrologist receives a tiny fraction of the money he receives before you suffer kidney failure. Kidney failure is the last thing your nephrologist wants for you.
Hi Dave & Dr Rosansky: I would like to know where is Dr Rosansky, I would like to consulta him, I’m at stage 5 CKD, and supposed to start dialysis soon, and I I’m struggling no to go to that toruture, I’ll highly appreciate the information about contacting him, also his book sounds if great help and all the information, incredible advice! Thanks so much for your help.
Hi Dave, can you please share your Nephrologist ph #? I know that you don’t live in NJ , but I would like to seek medical treatment somewhere else, my Nephrologist here is a dead end( just mention dialysis and or transplant! I’m desperate, thank you so much!
Please instruct if one must join Dadvice to comment or pose a question, as I had posted something earlier then note it disappeared. Naturally, I want to abide by the rules! Or perhaps there is a time process for review. I'm learning! Thank you very much...

It will vary person to person, but the current recommendation is around eGFR 5-7 unless there are other health issues that require starting dialysis early. Your doctor will be able to explain the best time for you based on your health.
At 57:35 they discuss Joan's question about diabetic medications that can help with kidney function. James gave some directions for finding the video but I had some trouble locating it. I believe this is the video & the medications named there: th-cam.com/video/SqDn5uabpT8/w-d-xo.html "Episode 164: Managing Kidney Disease: Dr. Rosansky discusses medications that can help you live longer . . . Dr. Rosansky will discuss the latest medications ( including Canagliflozin, Dapagliflozin, & Empagliflozin) that have been shown to improve the quality of life and help people with kidney disease live longer."
Dr. RO - I turned 67 a couple months ago. I have high blood pressure, however the #'s were in the normal range in April. Last November, my GFR was 52.33, HDL was 101 and protein was negative. This April my #'s are 51.69 and 105 (no unrine test) and was told I'm Stage 3a. Is the GFR change considered fast or slow. Does the HDL correlate to the GFR readings? As a side note, my Mom has had high blood pressure as long as I've been alive, has not watched her diet (but now she's in a n nursing home), and is 92 1/2 and still very cognitive. There is longevity on both sides of my family with the average life age being in the low 90's. Some have lived into their low 100's. With this background and my #'s above, I am concerned about dialysis in the future. Do I need to be?
Thank you so much for all the information! Could you please let me know your thoughts-if my uncle was at stage 3A over a year ago and now in a week from 3.7 to 4.2 within a week? Should this be considered acute renal failure? He has anemia (hemoglobin of 9.5) and fluid retention but has changed his diet in the past two weeks and now his swelling went down. I’m very concerned and sad. He only has one kidney.
When is the right time to start, he is 55 years old, my hubby Dr said he need to get started because if he get so low he might end up in the hospital & might die if he dont get on dialysis soon, im sorry i don't trust them to much, the nurse didn't even know what monk fruit was when he was teaching the class, he said someone told him about it about a few weeks ago wow i think these medical professionals need to go back for new updated training
I am 62 i have protein in my unine and i am stage 4, i started at age 16 with high blood pressure having kids, toxemia and preeclampsiaand over the years was not told to go to kidney doctor till 37, i did have at hershey pa a i vp in age21 after third kid i had small kidney on right might been born that way, now at age about 39 sended but never told anything about the diffenet stages till four and that both kidneys are getting smaller and told to watch my meats, protein potassium and phosphorus on lots of medication and now have afib to and on blood thinners, dont know what to do next, on Staton.
Im age 75 years young.. when my kidney infection was diagnosed.with onfection whi h suddenly became disease. Ive been told im younger sat health worrksops. O one je .
It means your kidneys are extremely failing. If you are not urinating daily, you must see a kidney doctor. It could be life threatening. You need dialysis at around Stage 15. If it is below Stage 15... like Stage 1, you are near-death. Therefore, you need dialysis. There are two forms of dialysis: Tube or Pill. Good news: Kidney Failure can be reverse!!!! Get right with God. He will heal you overtime if you obey Him. If not, He will murder you.
I finally had time to watch this episode. As an African American, I was extremely disappointed in the topic of creatinine in Black people. This across the board generalization that all of us have more muscle mass than White people is complete B.S. And what about mixed race folks? By artificially raising our GFR doctors are keeping us from reaching that 20% function number necessary to be considered for a transplant. Very disappointed that you would be pushing an outdated system like this.
The eGFR calculation is not manage or established by me, it is what the world uses and is only a rough estimate (it's actually extremely inaccurate for a calculation that gives a single number). I personally am not a fan of the eGFR at all - it really is meaningless and too many people focus on it instead of their overall health and managing symptoms. I would like to see it modified to give a range (some countries are starting to do that). I encourage you to reach out to the labs you use and discuss moving to an estimated range with them. It's those of us who speak up who help to influence change and make the world better. Bashing each other isn't the path forward, it only hurts our cause of trying to get earlier detection, better early intervention, better funding for new treatments, better treatment for those already living with kidney disease, more organ donors, etc. Good luck.
I do not agree i am 63 yrs old have been on dialysis 8 yrs you need to understand when your kidneys are damaged you need to clean the toxins in your Blood yes you get some weakness i do not pee i try to give my body the nutrition from vitamins minerals that help cardio i get albumen to get protean

How can we get this book
How do we test for protein loss?
This is done in a urinalysis. I get one each time I get my labs done since protein leakage is a great indicator of future kidney health/issues.
How do I get my Blood Sugars down running in 200's and have Stage 4.
I am so sorry, these doctors do that for the billions they make off of us
I'm 56ys old living with stage 5 I know so dyliss is my last resort and I just started my work started drinking plenty water eating vegetables fruits fish chicken breast and some other changes my diet doing my exercises extra walking around I don't want no one to limit my life I will be in touch with getting my health back in shape
I'm so sorry...now, you are your own advocate...I have my own story to tell but it is sickening dialysis is such a money maker to our detriment!
@oneildonegal2529 are you thinking about dialysis or you trying to reverse the kidney disease
Mr.James. I am from India watching your videos.,being patient I want the book can you help me.
I have 3 days in week dialysis, im 34 years old... Any advice please, im desperate.

Please instruct if one must join Dadvice to comment or pose a question, as I had posted something earlier then note it disappeared. Naturally, I want to abide by the rules! Or perhaps there is a time process for review. I'm learning! Thank you very much...

When does a person need to go on dialysis (stage 4 gfr very low)?
It will vary person to person, but the current recommendation is around eGFR 5-7 unless there are other health issues that require starting dialysis early. Your doctor will be able to explain the best time for you based on your health.
Dadvice...how did you improve your anemia? More meat or plant protein?
My wife cannot lower her BP to less than 160 systolic. Please advise.
eat red beets evaery day
Try Clonidine 75mcg (Generic)
At 57:35 they discuss Joan's question about diabetic medications that can help with kidney function. James gave some directions for finding the video but I had some trouble locating it. I believe this is the video & the medications named there:

I am 78 with GfR of 33 with out protein do I have Kidney Decease
DR my son in law is on Dialysis about 4mt can he stop now
Why would a person have to do manual dialysis six times a day?
I am loosing weight my egfr 54 why I am loosing weight pl help I am from Malaysia
What dietary changes he made?
Thanks for all the information
Zoha imtiaz he has adjusted his diet according to his doctors advice. Low potassium I believe ...
Thanks from indiA
Also low sodium diet
I'm 76 with GFR of 40.

I am 55 year old suffering from CKD with eGFR 48 stage 3 , What suggestion you want to give me?
When is the right time to start, he is 55 years old, my hubby Dr said he need to get started because if he get so low he might end up in the hospital & might die if he dont get on dialysis soon, im sorry i don't trust them to much, the nurse didn't even know what monk fruit was when he was teaching the class, he said someone told him about it about a few weeks ago wow i think these medical professionals need to go back for new updated training

What happens if you stop passing urine at ckd of 4?
Good luck.
I am an Older Folk 83 yrs old,lol
