Difference between a document and record

To Plan and Check the process is an integral part of PDCA which is a methodology adopted by ISO standards. Therefore, making documents (Plan) and formats (Check) are very important activity in order to comply with any standard. People are confused in between documents and records and don't understand where to make what according to the standard requirements...!!!
