TBH - I thought Dororo was a girl when were 1st introduced to the character. During the same episode they call her a him ( or boy). So i just went with the flow assuming Dororo was a boy. I should have followed my instincts.

Dororo was raised as a boy... Possibly for safety... and it is likely safer to continue that way
But she is shown dressed as a girl right at the end of the last episode.
Bethany Brookes lol true, I just meant after the death of her parents. I didn’t mean that Dororo considers herself a boy (especially in this anime), just that it is clear that even someone that knew child-Dororo thought Dororo was male. Which suggests that Dororo was not treated/raised as a girl in any overt way... probably for safety reasons :D

The red flower shown in this episode and Demon Slayer (and many other shows such as "Jigoku Shoujo") is called a "higanbana," and was often seen growing near cemeteries. It came to be associated with death or the path to the afterlife, and thus it is a frequently used trope to symbolize death.
I feel you on the translation things, and it definitely didn't help how the subs early on default to 'boy' instead of 'kid' since that's how English is. It IS one of the twists though!!
It wasn’t too hard for me to pickup on due to the way she interacted with some of the other characters in the past and the time era they are in.

I'm different. Since the first episode I knew Dororo is a girl and I thought it was a wrong translation because it write "he". Through out the episode I'm doubt and thinking is Dororo a girl or a boy but after watched this episode it was confirm Dororo is a girl 😂

I still call Dororo by "he", because it seems like he prefers he/him-pronouns 😕
Some say Dororo just says he's a boy because of safety, others say he's trans. There are things that speak for both sides and because the anime doesn't give a straight answer, I think everyone can think for themselves what they want to call Dororo.
Biologically, Dororo is a girl yet identifies as a boy. It is not often you see trans characters being handled subtly and maturely in anime but Dororo is an example and possibly a rule. Dororo’s sex is hinted at through his slight tilt towards feminity in scenes where Dororo’s body was integral.
not a trans character. Dororo is a girl. just like Mulan, hiding her true nature was key to survival.
Manga was also written in the 1960's I agree with 9822703, warring states period too meaning women were treated awfully.
