AID-clamp - a clinical paradigm for evaluating new drugs in diabetes

  • AID-clamp is a proprietary test protocol that determines exogenous insulin requirements of an individual under test conditions via analysis of the automated insulin delivery (AID) event log. A participant in a diabetes intervention trial uses an AID system for their usual insulin delivery. After overnight fast they consume a standardized breakfast while using the AID system. From Time 0 to Time +6 hours (for example), total insulin infused by AID is recorded. An insulin quantification script retrieves insulin data over the time period, providing an output of total insulin requirement. This can be readily applied to the DIY (aka open-source) AID systems in widespread use. Reductions in test-meal-related insulin-requirement serve as a proxy for increased endogenous insulin action. This approach can provide real-time results regarding the efficacy of interventions.
